Code: 10640 01 3 320 1000000 01 1000 2. PURPOSE Main reason why the position exists, within what context and what the overall end result. The Acute Care Registered Nurse is a member of the patient care team that determines patient status and responds to actual or potential health problems; plans and performs interventions; and evaluates patient outcomes following standards, guidelines and scope of professional nursing practice with the intention of maintaining or regaining patient health. 3. SCOPE Describe the impact the position has on the area in which it works, or if it impacts other departments, the government as a whole, or the public directly or indirectly. How does the position impact those groups/individuals, the organization and/or budgets? What is the magnitude of that impact? The incumbent takes direction from the Nurse Manager, In-Patient Care of the Kivalliq Health Center and is responsible for providing nursing services and collaborating with other health care professionals, paraprofessionals and others involved in providing care. This position has a direct impact on the delivery of services to patients, and the incumbent s decisions and recommendations affect the way that services are provided, and affect not only the provision of services, but the cost of the services being provided 4. RESPONSIBILITIES Describe major responsibilities and target accomplishments expected of the position. List the responsibilities that have the greatest impact on the organization first and describe them in a way that answers why the duties of the position are being performed. For a supervisory or management position, indicate the subordinate position(s) through which objectives are accomplished. 1. Provides direct patient (individual, families and community) care nursing services to residents of the Regional Health & Social Services Department with a multi-disciplinary setting in order to prevent and restore incidence of disease, disability and death. Function as a team member in the following units/areas with the expectation that they must do so on short notice; intensive care, pediatrics, obstetrics, emergency, out-patients, medical/surgical, psychiatry, palliative care, and continuing care. Use the nursing process and completing an individualized nursing history by interviewing the patient or family, reviewing transfer notes and previous charts in order to complete a thorough patient assessment. Upon admission of the patient initiates individualized patient care through recognizing and respecting cultural differences, utilizing standard care plans, formulating individualized care plans for each patient and revising the care plans as patient s needs and progress dictates. Evaluates the nursing care giver and making recommendations and alternative plans for improved care by consulting with the appropriate physician. Assists CNA s to formulate patient care plans. Ensures medications are safely distributed to all patients when assigned while being aware of all side effects of all prescribed medications. Coordinates care with other agencies within the health facility and within the community as well as arrange the transport via medevac carrier of any patient requiring such service. Demonstrates knowledge and skills in implementation and the varied roles in a Code 99 procedure. Demonstrates knowledge and actual skill in performing a delivery in the absence of a physician. Maintains a working knowledge of use of all equipment and systems in the varied work areas, i.e., obstetrics, nursery, pediatrics, medicine, psychiatry, intensive care and palliative care. Ensures that provisions are made for all issues identified in the discharge planning for patients. Exercises effective cost control in the use of equipment, supplies and necessary staffing. Documents daily, all observations, nursing intervention and individualized patient outcomes achieved according to predetermined approved procedures; while maintaining complete, accurate, concise records according to hospital policies and procedures. Assists on-call physicians with the assessment and admission of patients arriving to the Health Facility for admission from the region. 2. Collaborates with other members of the health care team in providing health care services according to accepted nursing practices in order to prevent and restore incidence of disease, disability and death. Communicates and consults with other members of the health care team about the patient s care. 3. Participates in orientation/development of self and other staff members while increasing nursing knowledge and skills by self studies and pursuing professional activities in order to ensure competency for multiple skills required. Continually seeks and incorporates new knowledge and acquired skills into nursing practice. 4. Performs administrative duties. Assume a care role for administrative coverage in the health facility when assigned the In-Patient Manager role after regular hours and weekends, working during off hours which includes, liaison with Community Health Centers within the region, authorization of overtime which includes the decision to call back staff who are on standby status (radiology, social worker, pharmacist, laboratory technician, etc.) medical records and transportation functions, assess level of acuity for In-Patient Care Unit and call-in extra staff as needed, in charge of the in-patient health centre as a whole. 5. Adheres to the ethical standards of the nursing profession. 6. Other duties as required. Answers telephone and/or pulling charts from Health Records during the absence of the Communications Clerk. Maintains all patient care areas in a state of readiness at all times. Maintains a working knowledge of all procedures used in the work area. Completes and reports incident reports of unusual events, equipment/staff/patient/visitor incidents in a timely manner. 5. KNOWLEDGE, SKILLS AND ABILITIES Describe the level of knowledge, experience and abilities that are required for satisfactory job performance. Knowledge identifies the acquired information or concepts that relates to a specific discipline. Skills describe acquired measurable behaviors and may cover manual aspects required to do a job. Abilities describe natural talents or developed proficiencies required to do the job. These requirements are in reference to the job, not the incumbent performing the job. Bachelors of Nursing or Diploma in Nursing. RNANT/NU registration. Basic CPR with annual re-certification is required. Knowledge of current trends in health promotion/disease prevention practices and programs. Well-developed skills in order to communicate/teach effectively on an individual or group basis within a cross-cultural setting. Incumbents must be willing to successfully complete all certification programs that are offered in work setting. Incumbent must be willing to work in a multi-faceted environment. Possession of one or more of the following would be preferable but not required upon initial hiring: Basic Trauma Life Support (BTLS), Advanced Cardiac Life Support (ACLS), Neonatal Resuscitation (NALS), fetal monitoring, pelvic examination, labour and delivery experience with advanced obstetrical training. Cross Cultural training and computer skills an asset. Palliative Care Experience an asset. 6. WORKING CONDITIONS List the unavoidable, externally imposed conditions under which the work must be performed and which create hardship for the incumbent. Physical Demands Indicate the nature of physical demands and the frequency and duration of occurrences leading to physical fatigue. There can be the requirement for long periods of standing. Lifting of patients and equipment. Fatigue related to 12 hour shifts. Environmental Conditions Indicate the nature of adverse environmental conditions to which the jobholder is exposed, and the frequency and duration of exposures. Include conditions that disrupt work schedules and travel requirements. Exposure to infectious diseases, hazardous and/or toxic chemicals and drugs. Sensory Demands Indicate the nature of demands on the jobholder s senses to make judgments through touch, smell, sight, and hearing and judge speed and accuracy. Incumbent s two or more senses to make judgments and the use of senses require considerable sensory attention. Mental Demands Indicate conditions within the job that may lead to mental or emotional fatigue.
