Dear Prospective Resident or Family Member, A safe, comfortable step between independent living and a skilled nursing facility, our assisted living services at Dove Healthcare - Barron are designed to provide you a helping hand with every day activities, while you continue to live as independent as possible. Within the community of assisted living providers, we strive to be known for our inviting and comfortable atmosphere, friendly caregivers, and the quality of our services. As a certified Residential Care Apartment Complex (RCAC), Dove Healthcare - Barron will serve those age 60 and older that are in need of some assistance and supervision with activities of daily living and or medication management and administration. Assistive devices such as canes, wheelchairs, and walkers are welcome. We are able to care for mildly confused residents as long as unsafe wandering or elopement is not a problem. A few of our most appreciated, unique features include: Competitive, affordable monthly rate: Your monthly base rate will include three hours of care per week. An assessment prior to admission will determine your care level rate. Professional nursing services: A nurse consultant is available to address general health concerns. Conveniently located next to the Barron hospital: You will have peace of mind knowing in the event of an emergency or illness, help is right next door. The hospital is connected to the assisted living by an enclosed walkway, making the hospital and clinic easily accessible. Physical, occupational, and speech therapy: Out-patient therapy services can be easily coordinated and provided on-site by Dove Healthcare therapists. Today, Dove Healthcare is comprised of six skilled nursing facilities (Dove Healthcare Barron, Bloomer, Osseo, outh Eau Claire, West Eau Claire, and Wissota Health & Regional Vent Center); four assisted living facilities (Dove Healthcare - Barron, Osseo, Orchard Hills Assisted Living, and Rutledge Home); and one rehabilitation company (Transitions Rehabilitation). Construction is currently in progress on a skilled nursing facility, set to open summer 2016 in Rice Lake. Our talented and diverse workforce of 800 employees serves an average of 360 residents and patients every day. Dove Healthcare - West Eau Claire, our first acquisition in 1997, is licensed for 160 beds and is located on Truax Boulevard in Eau Claire. This facility specializes in skilled nursing, short-term rehabilitation, long-term care, and end-of-life care. Dove Healthcare - Orchard Hills Assisted Living opened inside Dove Healthcare - West Eau Claire in 1999 and then a new 26-unit facility was built attached to it in 2005. A second expansion in 2008 brought total capacity of Dove Healthcare - Orchard Hills Assisted Living to 39 studio apartments. Dove Healthcare - Rutledge Home, a 92-bed skilled nursing facility in Chippewa Falls, was added as a third affiliate in 2004. Dove Healthcare - Wissota Health & Regional Vent Center, a 135-bed skilled nursing facility with 36 certified ventilator beds, was purchased as a fourth affiliate in late 2005. Page 1 of 2

In early 2007, Dove Healthcare - Rutledge Home merged operations with Dove Healthcare - Wissota Health & Regional Vent Center. The merger allowed for more efficient operations at both Chippewa Falls facilities. ince then, Dove Healthcare - Rutledge Home has been transitioned into a 63-unit assisted living facility, with its primary focus on Memory Care. In addition to featuring the Regional Vent Center, Dove Healthcare - Wissota Health specializes skilled nursing, short-term rehabilitation, ventilator and tracheostomy care, long-term care, and end-of-life care. Dove Healthcare - outh Eau Claire opened in eptember 2009 on Mall Drive in Eau Claire. The 50-bed, state-of-the-art skilled nursing facility, featuring aquatic therapy and exercise, is an expansion of our short-term rehabilitation services. Transitions Rehabilitation consists of over 60 licensed therapists that provide services to all of our locations. We have invested heavily in our staff, therapy gyms, and equipment to ensure our patients receive the most advanced and progressive treatment. In October 2015, Dove Healthcare became the licensed operator of skilled nursing facilities in Barron, Bloomer, and Osseo, as well as assisted living facilities in Barron and Osseo. Future plans for these communities include the following: Dove Healthcare - Rice Lake will open summer 2016. The Dove Healthcare - Barron skilled nursing facility will move its operation into the new Rice Lake facility. All Barron skilled nursing facility residents and employees are invited to move with this transition. The assisted living facility in Barron (formerly Maplecroft Assisted Living) will continue to operate in its current location. Dove Healthcare Osseo is currently a 21-bed skilled nursing facility and 14- unit assisted living facility attached to Mayo Clinic Health ystem Oakridge in Osseo. A new state-of-the-art, 40-bed skilled nursing facility with attached 24-unit assisted living facility will open in fall 2016, replacing the current facilities. The skilled nursing facility provides both short-term rehabilitation and long-term care. Dove Healthcare Bloomer is currently a 35-bed skilled nursing facility (formerly Maplewood Nursing Home) attached to Mayo Clinic Health ystem Chippewa Valley in Bloomer. A new 40-bed, state-of-the-art skilled nursing facility providing both short-term rehabilitation and long-term care will open in spring 2017, replacing the current facility. Through the years since Dove Healthcare's origination, it is clear our steady growth is a result of our consistent approach defining what we do well, doing more of it as often as we can, and growing as our resources allow.
