Kings County Hospital Center Cancer Care Program ANNUAL REPORT 2013: EXECUTIVE SUMMARY The Kings County Hospital Center Cancer Care Program (KCHC-CCP) is accredited by the Commission on Cancer (COC) of the American College of Surgeons (ACS) as an Academic Comprehensive Cancer Program (ACAD), committed to providing quality cancer care services to the community. Certification by the COC validates and recognizes the excellent and highly regarded organization of Kings County Hospital Center, and its Cancer Care Services, the high quality of care the program provides, and its application and implementation of changes that proliferate access to Cancer Care services and lead to better outcomes. We stand proud of this recognition bestowed by the COC and the ACS. The Cancer Care Program (CCP) of Kings County Hospital serves as a major referral center to other New York City Health and Hospitals Corporation (NYCHHC) centers in Brooklyn, including Coney Island Hospital, Woodhull Hospital, and other community hospitals. The CCP of Kings County Hospital is guided by a Cancer Committee, a multidisciplinary team of physicians and non-physician representatives from multiple clinical specialties, involved in the management of patients with cancer. The Cancer Committee also includes representatives from various hospital administrative ranks and support groups. The focus and goal of the Cancer Committee is that of monitoring the Program s continuity in the delivery of excellent patient care, adherence to quality standards, overseeing research outcomes, and engagement in performance improvement initiatives. The Cancer Committee ensures that resources of the CCP are directed towards the goal of providing optimum cancer care for a diverse patient population. The multidisciplinary team integrates cancer specialists from: medical oncology, surgery, radiation oncology, pathology, radiology, urology, and other cancer related fields, to provide Kings County s patients with accurate diagnoses and the most effective treatment plan possible. The teams work together in the monitoring and re-evaluation of treatment plans, and the integration of support services for patients suffering from cancer. The Cancer Care Program s vision is to be the cancer healthcare provider of choice for the residents of Brooklyn. As such, the Program steadfastly makes every effort towards delivering quality cancer services, which include emphasis on patient safety, cancer prevention, research, rehabilitation, and survivorship programs. Cancer Care Treatment Center The Cancer Treatment Center provides multi-disciplinary care, in a healing environment that is located in one convenient outpatient facility and is designed, equipped and staffed, to offer a complete range of non-surgical cancer treatment techniques. This modern facility features examination, treatment rooms and consultation space, as well as a state of the art chemotherapy area for outpatient chemotherapy. The center is located in close proximity to the Radiation Oncology services. Other features include a pharmacy, lab and conference rooms for patient education, nutritional and social services counseling. KCHC Cancer Care Program: Annual Report 2013 - Executive Summary 2

Ambulatory Palliative Care Service The Ambulatory Palliative Care Service is an integral part of the comprehensive care provided at KCHC Cancer Center with a robust and well established reference base. The number of visits has multiplied from 53 in the first year to 256 in the third year, subsequently the follow-up rate for patients returning for care increased from 56% to 67%. The no-show rate declined from 38% to 36%. Additionally, Patient Satisfaction Surveys reflected that patients consistently rated the service with a high mark.
