7 A Time to Heal 12-week educational program designed to help men and women regain physical, emotional, intellectual, psychological and spiritual health after undergoing treatment for cancer.

8 Cosmetology/Personal Appearance The Appearance Center at Faith Regional offers patients a positive place of support where they can obtain wigs, hats, and expertise on looking good and feeling better. FRHS hosts the American Cancer Society Look Good, Feel Better program

9 Breast Prosthesis Fitting Program Previously through the American Cancer Society Continuing at FRHS Trained prosthesis fitter Foundation funded

10 Educational Services Educational programs are offered throughout the year on cancer prevention, screening and treatment. Smoking Cessation Kits ACS Resources Colorectal Cancer (FOBT) Kits Cancer-Related Brain Fog Inservices Breast Cancer Therapy Inservices

11 Home Health Care Home health care is available for those patients needing specialized treatment while wishing to recover at home with a medical condition or following surgery.

12 Hospice Services Hospice Care is available to patients with a lifelimiting illness and a life expectancy of less than 6 months as determined by the physician. Medical, emotional and spiritual services can be provided in a nursing or assisted living facility, hospital or the patient s home.

13 Lymphedema Services Patients with lymphedema can obtain assistance in managing this condition from a certified lymphedema therapist.

14 Nutrition A registered dietician is available to assess patient nutritional needs and educate and counsel patients on maintaining or improving nutritional status.

15 Pain Management/Palliative Care Patient/family centered care Individuals with a chronic or life-threatening illness Multidisciplinary holistic team approach toward body, mind, spirit and culture.

16 Patient Library The health resource library is located in Faith Regional University of Medical Offices West and includes healthrelated books, journals, videos and access to online resources.

17 Physical, Occupational and Speech Therapy All 3 therapies are offered at Faith Regional Health Services as part of the STAR Program

18 Therapy Referrals Physical, Occupational & Speech Therapy 2011: 56 referrals 2012: 88 referrals 2013: 106 referrals

19 Physical Wellness/Exercise Following completion of the STAR therapy program, cancer patients have access to an exercise facility to use for a maintenance exercise program that is staffed by exercise specialists.

20 Social Work A social worker is available to assess any barriers to treatment and healing and can refer patients to the appropriate resources. Finances Transportation or accommodations Support needs such as counseling, patient-to-patient programs and support groups.

21 Spiritual Support Spiritual Care representatives are available to guide patients through emotional challenges and spiritual questions that may arise upon diagnosis or during/after treatment.

22 Support Group Caring Hearts Cancer Support Group meets twice monthly in the Health Resource Center

23 Volunteer Services Reach to Recovery American Cancer Society (for breast cancer only) Hand-in-Hand Faith Regional s newest cancer patient connection program

24 Staff and Patient Education Regarding Survivorship Services Physician and Staff Inservices Meetings with Physicians Brochures Patient Education Inservices Referral Pads Cancer Specific Flip Charts Cancer Specific Patient Screening Hand-outs
