2 Millions of people have had or are living with cancer. Here at Kaiser Permanente s East Bay Cancer Center, we want you to know that if you are one of those affected, you don t have to take this journey alone. Upon your initial diagnosis you will be referred to a cancer specialist and other providers or departments which may be new to you. We understand that navigating your way through the treatment process can be overwhelming, so we offer a variety of educational materials and support services to help you and your loved ones along the way. This booklet contains information about all of the cancer-related services, support groups, and classes we have available here. Having the information you need and working together with your medical team will help you live your life to the fullest before, during, and after treatment.

3 BREAST CARE CENTER We offer a full range of breast health services which includes exam, biopsy, and surgical treatment consultations. We utilize a multidisciplinary approach to treatment planning for patients with breast cancer. Go to for more information. v Oakland: 3801 Howe Street, 1st Floor, Suite 120 Reception/Appointments/Advice Breast Care Coordinator GENETICS The Oakland Genetics Department provides comprehensive medical genetic services to Kaiser Permanente Health Plan members residing in the East Bay. We specialize in counseling, diagnosis, and management of genetically influenced conditions. Our clinical services related to cancer include hereditary cancer risk evaluation and counseling and genetic screening. v Oakland: 3505 Broadway Reception/Appointments/Advice GYNECOLOGY ONCOLOGY Your Gynecologic Oncologist may offer both medical and surgical treatment of gynecologic cancers. 4 INTERVENTIONAL RADIOLOGY DEPARTMENT Interventional Radiology provides a range of minimally invasive procedures for cancer patients. These include biopsies, placement of venous access devices such as ports for chemotherapy, radio frequency ablation of tumors in liver, kidney, lung, and bone, as well as chemoembolization of liver tumors. We work closely with Medical Oncology, Gastroenterology, Hospital Medicine, Radiation Oncology, and various surgical services in caring for cancer patients. v Oakland: 3600 Broadway, Ground Floor Reception/Appointments/Advice v Richmond: 901 Nevin Avenue, Building 1, 1st Floor Reception/Appointments/Advice MEDICAL ONCOLOGY The Medical Oncology Department specializes in the diagnosis and treatment of cancer. 5 PLASTIC AND RECONSTRUCTIVE SERVICES Consultations with a plastic surgeon are available in the Breast Care Center. 7 Kaiser Support Groups BREAST CANCER SUPPORT GROUP This weekly group is intended for women who have a diagnosis of breast cancer. Members only; family and friends welcome. Call for location and time CAREGIVER SUPPORT GROUP This group is designed to educate and support caregivers as they can handle difficult decisions and situations associated with caring for someone with a serious illness. Open to family members or concerned friends. Call for location and time GENERAL CANCER SUPPORT GROUP This group is for any member who is dealing with a new or ongoing cancer diagnosis. Family and significant others are welcome. Meets on the ground floor of the Broadway Medical Office Building (3701 Broadway). Call for location and time or GYNECOLOGY CANCER SUPPORT GROUP This bi-monthly group provides support for women with cancers of the cervix, ovary, vulva, or uterus. Call for location and time HEAD AND NECK CANCER SUPPORT GROUP This once monthly group is for members living with with head and neck cancer, as well as their families and friends. Call for location and time or PROSTATE CANCER SUPPORT GROUP For patients and family members who are affected by prostate cancer and have questions about PSA screening and treatment. 9 Kaiser Support Services COMPLEMENTARY AND ALTERNATIVE MEDICINE (CAM) Visit kaiserpermanente.org to explore complementary and alternative care options available to you within Kaiser, including chiropractic, acupuncture, herbs, and supplements. DEPARTMENT OF PSYCHIATRY Our Psychiatry Department offers therapy for you and your family members to assist in coping with cancer. Open Monday - Friday from 8:30 a.m. 5:30 p,m, DURABLE MEDICAL EQUIPMENT The DME department provides assistance with breast prostheses, bras, lymphedema gloves and sleeves, as well as other medical equipment. You will need a prescription from your physician for all equipment and supplies GENERAL NUTRITIONAL ADVICE General cancer and breast cancer nutrition classes. Please call the Radiation Oncology department for registration HEALTH EDUCATION Visit our Health Education Center for the latest information about cancer diagnoses and treatment. Trained staff will help you find the information you need. 10 Kaiser Support Services HOSPICE Hospice provides comfort at home for patients facing the final stages of a terminal illness. Patients and their families are supported by the hospice team which includes physicians, nurses, social workers, nurse s aides, volunteers, spiritual care, and pharmacists. Office Hours: Monday Friday from 8 a.m. 5 p.m MEDICAL SECRETARY DEPARTMENT The Medical Secretary Department can assist in obtaining medical records or filing for state disability if you are unable to work during your illness. v Oakland: Located on the 5th floor of the Broadway Medical Office Building (3701 Broadway). Office Hours: Monday Friday from 8:30 a.m. 4:45 p.m. v Richmond: Office Hours: Monday Friday, 8:30 a.m. 5 p.m. MEDICAL SOCIAL WORK SERVICES A Kaiser Permanente medical social worker can assist you and your family in coping with the emotional, social, financial, and practical stressors that may accompany a cancer diagnosis. Contact your physician or nurse for more information. v Oakland: Office Hours: Monday Friday, 8:30 a.m. 5 p.m. v Richmond: Office Hours: Monday Friday, 8:30 a.m. 5 p.m. MULTIDISCIPLINARY TUMOR BOARDS We have several assessment and treatment planning panels with representatives from a variety of disciplines such as surgery, medical oncology, head and neck oncology, radiation oncology, pathology, radiology, nursing, and nutrition. 11 Kaiser Support Services NUTRITION & CANCER CLASSES A registered dietitian presents classes on proper nutrition before, during, and after cancer treatment. Please call the Radiation Oncology department for registration NUTRITIONAL SERVICES Registered dieticians provide one-on-one counseling before, during, and after cancer treatment. Please ask your provider for a referral PALLIATIVE CARE A consultative service for patients with a life-limiting illness and their families, to help them live as well as possible while they cope with their condition. We provide advance care planning, pain and symptom management, psychosocial and spiritual support in the last 1 2 years of life. Office Hours: Monday Friday from 8:30 a.m. 5 p.m. PHYSICAL THERAPY & LYMPHEDEMA CARE We offer a full spectrum of services, from post-operative classes to lymphedema care, to assist in the recovery process. Please ask your provider for a referral.

12 KAISER Online Resources EMMI These are online tutorials designed to help you understand your procedure or surgery. Visit your physician s home page and select Tools & Classes, then select Prepare For Your Procedure. KAISER ON-LINE Sign on to kaiserpermanente.org to send s to communicate directly with your providers, schedule appointments, order prescription refills, and review lab results. Your physician s home page has a variety of links, podcast downloads, and interactive programs. PODCASTS A variety of free podcasts regarding cancer and other health-related topics are available on kaiserpermanente.org. These guided imagery programs engage your mind, body, and spirit to help you reduce stress, focus on healthy changes, and promote healing. 13 Support In The Community AMERICAN CANCER SOCIETY ACS offers cancer information, treatment-related transportation assistance, peer support services, Look Good Feel Better sessions, free wigs, hats and scarves, and much more (English and Spanish), (toll free), or (Mandarin and Cantonese) BREAST CANCER.ORG An online site which explains breast cancer terminology, pathology, and treatment in an easy to understand format. CANCER.NET (Formerly People Living With Cancer ) American Society of Clinical Oncology s patient information website. Provides timely, comprehensive, oncologist-approved information intended to help patients and families make informed health care decisions. CANCER SUPPORT COMMUNITY (Formerly The Wellness Community ) Provides a wide range of free support groups and classes for people with cancer and their loved ones

14 Support In The Community CHARLOTTE MAXWELL COMPLEMENTARY CARE CLINIC State licensed health clinic providing free complementary and alternative medical (CAM) treatments to qualifying low-income women with cancer CIRCLE OF CARE East Bay Agency for Children program offering one of the nation s few counseling and support services for children and their families with a parent, caregiver, or family member who has died or is living with a life threatening illness. COMPREHENSIVE CANCER CENTER Offers a variety of classes and support groups open to the community LEUKEMIA & LYMPHOMA SOCIETY Provides education and information about blood cancers, financial support, and peer volunteer support LIVESTRONG FERTILITY (Formerly Fertile Hope ) Provides information about cancer-related fertility preservation options for men and women, cancer and sexuality, and limited financial assistance for fertility preservation. NATIONAL CANCER INSTITUTE Accurate and up-to-date comprehensive cancer information from the U.S. Government s principal agency for cancer research. STUPID CANCER National organization for young adults (ages 15-39) with cancer. Activities include advocacy, research, support, outreach, awareness, mobile health, and social media.
