1 Introduction to the Quick Reference Guide to Inpatient Rehab/CCC/Palliative Care Programs in the GTA Inpatient Rehab/LTLD/CCC Quick Reference The attached document is a Quick Reference of adult Inpatient Rehabilitation, Low Tolerance Long Duration (LTLD), Complex Continuing Care (CCC) and Palliative Care programs (within CCC) throughout the GTA. It is current as of April 212. It was designed to function as an adjunct to Rehab Finder, as a printable, easy to read overview of programs for referrers, families and clients. It intentionally reflects only those programs that accept external referrals and, as such, is not an exhaustive list of all beds/programs in the GTA. The information provided is intended for the purpose of easily locating an appropriate bed for a specific adult client. The Quick Reference Guide provides a summary of the number of designated rehab, LTLD and CCC beds by organization and population. The information is organized in tabs according to hospital type (Rehab/CCC Hospitals; Acute and Community Hospitals) We ask that you assist us, at the GTA Rehab Network, in keeping this document current, by informing us when there are changes to your program that will affect admissions/referrals. The GTA Rehab Network will work to actively request updates from organizations on an annual basis. 2 Definitions High Tolerance Short Duration Rehabilitation (HTSD) Low Tolerance Long Duration Rehab (LTLD) Complex Continuing Care (CCC) Definitions This type of program is an inpatient rehabilitation program that may be a general/mixed rehab program or a population specific rehab program (see below). It is a post acute, intensive rehab program, for patients following illness, injury or surgery who need 24 hour hospital care and concentrated interprofessional rehabilitation. It may be located in an acute care or a freestanding rehabilitation hospital. (Definition from GTA Rehab Network) Low Tolerance, Long Duration (LTLD) rehab is most typically located in complex continuing care beds situated within acute care, rehab or Complex Continuing Care hospitals. Low Tolerance, Long Duration (LTLD) rehab is suitable for individuals in need of an interdisciplinary rehab program who may also have a chronic/complex condition requiring 24-hour hospital care over an extended period of time and who are expected to benefit from a slower-paced rehab program for a longer duration than is offered in dedicated or mixed rehab programs. (Definition from GTA Rehab Network ) Complex continuing care (CCC) provides continuing, medically complex and specialized services to both young and old, sometimes over extended periods of time. CCC is provided in hospitals for people who have long-term illnesses or disabilities typically requiring skilled, technology-based care not available at home or in long-term care facilities. Any rehabilitation component of care is focused on improving or maintaining quality of life. CCC provides patients with room, board and other necessities in addition to medical care. There may be a co-payment required that covers the cost of room and board. Any co-payment is facility specific.
