Priority Project - Community Support (Aging at Home) Priority Project - Caregiver Supports (Aging at Home) Priority Project - GEM: Pursue Enhancement of Geriatric Expertise in Hospital Monitor progress reports on prioirity area wait times in the CE LHIN Hospitals in the CE LHIN will participate in the Provincial Wait Time Information System and other quality improvement initiatives Explore an "always available" critical care bed concept for ICU referrals between CE LHIN hospitals. Priority Project - Coordinated Intake Across Multiple Programs Priority Project - Support During Times of Transition Agencies and organizations are to improve integration of the Community Support Sector with other parts of the health care system to make it easier for providers and consumers to access the right care at the right time. Priority Project - Specialized Geriatric Priority Project - Medication Management Priority Project - Falls Prevention in LTC Homes and Hospitals Priority Project - Rural Transportation SCS - Specialized Geriatric CE LHIN Core - Transportation Some Activity Underway The CE LHIN will work in partnership with CE CCAC as it plans how to equitably serve the Central East area. CE LHIN Core - CCAC Build working relationship with Public Health Units CE LHIN Core Public Health / Health Promotion Designating FLS Service provision CE LHIN Core - French Language Establish Health Professionals Advisory Body (HPAC) / Quality and Safety Committee Determine appropriate engagement of Pharmaceutical and Insurance Co's. (research, planning, program initiatives, resourcing) Model-Framework: Build understanding of Model components - Educate:Providers,Steering Committee, Others Consistent guideline for specialist referral (pre-consult preparation) Shared resources and service coordination. Community mental health and addictions agencies to find ways to share resources and coordinate services Education. Agencies/organizations, along with relevant community groups/networks working on behalf of seniors, will collaborate in the development and implementation of strategies to educate/increase awareness of services/programs available for seniors, caregivers and providers. 4 Integrated Health Service Plan (IHSP) Action Items The CE LHIN, in establishing system performance indicators, will develop elder sensitive measures that encourage high quality and safe care of the seniors population. The CE LHIN and its community partners will further the concept of a local Quality Conference that would disseminate best practice approaches and facilitate knowledge exchange related to quality and safety for seniors. To enhance quality of care through opportunities for sharing resources and coordinating functions Wait time task group to facilitate inter-facility working partnerships that will improve access to provincial targets Conduct a critical care health human resources needs assessment and explore coordinated recruitment Facilitate, as needed, the timely approvals and implementation of the Community Health Centres currently under development in the area. Obtain current information on where telemedicine equipment currently resides, its capacities and the network of human resources that are providing services through this medium. Explore coordinate regional training schedule for the use of telemedicine/video-conferencing CE LHIN Core - Telemedicine CE LHIN Core - Telemedicine Links to Active Projects Assess capacity for stroke rehab Rehab Continuum. To establish a solid understanding of the rehabilitation needs of the complex geriatric population, and expand capacity where needed. Build Baseline Info/Knowledge Base 1. Cardiovascular and stroke 2. CKD & diabetes 3. Respiratory 4. Arthritis Related Conditions Stroke: Develop more accurate understanding of HHR limitations/pressures Case Management. To explore models of coordinated access and case management that follow the senior to enable them to receive the right care in the right place at the right time. Transition Points. To better support seniors and their caregivers at times of transition between levels or types of care. SCS - Navigation SCS - Navigation Develop and pilot "Green Script" for behavioural modifications. (A statement of commitment by consumer/caregiver and service provider's which outlines the behavioural modifications/activities that are recommended) Host a Transportation Summit that will bring together consumers, caregivers and providers to identify actions for improving transportation services, thus improving access to services. (MHA) CE LHIN Core Public Health / Health Promotion Education/Awareness of impact of broader determinants of health CE LHIN Core Public Health / Health Promotion Communication across LHIN boundaries Re: communication; data; opportunities Improve stroke patient transitions and follow-up particularly with LTC Homes Improve consistency of screening and assessment tools (Identify and share best practices) Link people with CD to mental health (depression) supports Regional Networking Groups for specific populations. Utilize a systems approach to service and promote regional networking groups for specific populations Reduce stigma and discrimination. Work with the Mental Health and Addictions Network, health providers, consumer & family groups to reduce the stigma of mental illness & addictions and the discrimination against those experiencing them Interdisciplinary team approach to care - within/across. Develop an interdisciplinary team approach to care across agencies within mental health and addictions and across sectors MHA - Health Human 5

Once completed, the Central East LHIN will review the evaluation of the Medication Management project through the former Scarborough Community Care Access Centre to determine future opportunities for this model. Interdisciplinary Teams. Explore opportunities to augment or partner with Primary Care Providers (e.g. Family Health Teams, CHCs, OTHERS) in the provision of preventative, maintenance and restorative services/programs Establish a FLS Primary Care Hub Establish HHR Working Group Provider education on management of complex, multiple CD's Coordinated effective method to collect/share information. across programs Client information travels with them to various chronic disease providers Identify barriers and opportunities to form interdisciplinary care teams for people with chronic disease Knowledge Exchange - CQI and Standards. Ensure current knowledge of national, provincial and inter-ministerial initiatives demonstrating best practice and new knowledge Develop safe, evidence-based alliances and linkages with complementary health practitioners/practices.. Access to adequate numbers of appropriately trained chronic disease professionals Use video/web technology to: 1. connect patients and their DR's to specialists 2. education for consumer/caregiver on their disease and managing their condition Knowledge of Best Practice Initiatives. Promote knowledge exchange focused on excellent program standards and learnings from CQI initiatives HHR recruitment, retention and training strategy.
