2 WATER DEPARTMENT ACTIVITY REPORT OCTOBER 2012 Started and extended 8 water main into Phase 2 of Cobblestone Estates and crossed into Windset Ct. with 6 to be across street for road construction Installed 4 sleeves under Hampton Dr. for future service runs for road construction work to begin Met with Peavler and Plan B Construction on road construction conflicts with utilities Went back to start of 8 water main on Hampton and removed dirt and added rock to base up where trench was soft under curb Met with engineer and survey crews on Switzler to give depths of water and sewer mains Met with Bruce Kasubke on future boring sites for fiber installation and possible conflicts for Chances Dug down on top of sewer main behind Intermediate School to investigate large hole after camera work found large void in pipe installation. Corrected with piece of pipe and two repair bands Repaired two spots on primary cell at N.E. Lagoon (muskrats) with bentonnite and added clay Repaired lime pond burm behind Water Plant with clay Seeded and strawed 6 dig sites Flushed sewer main from E. Sims to Miles to open blockage Flushed sewer main at Worthington Ct. to assist resident with blockage their problem Made water tap and ran service at 453 Adams for new garage Made new water tap at 201 Switzler for Tom Hale Raised meter jar to meet grade for concrete at Jodie Laundry and added dirt Fixed leak on meter at 816 Countryland Cleaned both basins at Water Plant Changed leaking key valve at #61 Tidball Trailer Court Pulled and cleaned mop head that was stuck out of pumps at Fountain Road Lift Station Put new hydraulic hoses on to pump for lime cleaning Flushed all fire hydrants for fall season Oiled and serviced 5 hydrants that were not working well Inspected sewer capping at house belonging to School on S. Rollins 12 disconnects for nonpayment Read all utility meter for office use 25 readings for office confirmation

4 ELECTRIC DEPT. OCTOBER 2012 ACTIVITY REPORT 1. Hauled brush/grass as necessary from North St brush site 2. Mowed and weed eat at Wilson St Sub as necessary 3. Trimmed trees: #1 Leabrook 4. Tree removals: 619 Head, 422 S. Jenkins, 501 S. Columbia, #1 Leabrook, Gano Chance between Central & COLT, Sneed/Jenkins, 311 S. Jenkins 5. Repaired streetlights: 214 Sunset, Hulen/Howard Burton, Jenkins/Wilkins, 229 S. Rollins 6. Repaired security lights 7. Trenched in 150 feet of 3 PVC conduit at #4 Sunrise Circle, pulled wire, set secondary pedestal, connected services, removed overhead line. 8. Repaired pump control panel at Sims Farm 9. Southwest Circle UG project: Trenched and installed conduit for primary line, installed transformer, secondary pedestal, pulled wire and connected main lines. Trenched and installed conduit for five houses, pulled wire and connected service. Transferred two underground services from pole to pad mount transformer. 10. Tornado siren test: 1 st Wednesday of month, all working 11. Connected new underground service to pad mount transformer at 201 E. Switzler 12. Hwy 22 Welcome Sign: set new pole adjacent to sign, transferred lines, transformer to new pole. Strung new lines from this pole to pole adjacent to Subway Restaurant, reconnected all lines. 13. Electric Power Systems tested and cleaned eight circuit breakers at the substations. All OK. 14. Installed and removed temporary outlet boxes for the BBQ contest. 15. Replaced disconnect box at NEOS machine at Bicentennial Park 16. Assisted Sewer Dept with pumps/motors at Fountain Rd Lift Station 17. Repaired riser pipe at Prenger s store 18. Raised service at 616 S. Rollins 19. Pulled wire and connected underground services to new houses at 914 Hampton and 831 W. Rodney Griffin 20. Replaced dead end insulators at E. City Park teardrop drive, old ones were broken. 21. Replaced street light pole in front of CBMS 22. Installed bumper posts at the end of E. Sims St to prevent entry to ditch.
