3 The main engine on the unit for the draw-works did not have a heat guard or insulation installed for safety on the exhaust pipe and around the turbo. 4 The main generator engines also did not have heat guards or insulation installed on the exhaust piping or turbo as well. They will need to have heat guards installed for safety. It was also noted the breaker box panels located on the generators were not covered as required. The protection covers were missing and this was a safety issue. 7 The sight gauge located on the main fuel tank for the generators did not have shut off valves installed. This creates a safety issue for fuel spill. Need to install fuel shut off valve on sight gauge tube at top and bottom of sight tube. The sight tube also needs safety guard installed around the tube for protection of the gauge tube. 9 During the visual inspection of the mud pump and system it was noted the butterfly valve handles for the charge pump were broken and in need of replacement. It was also noted the safety guard cover for the coupling and shaft was not secured on one side due to the weld was broken. 10 During the derrick inspection it was noted, the low speed descend SALA block located at the monkey/racking board was installed with a screw type shackle and no secondary safety cable was installed as well. The screw type shackle will need to be replaced with a four part safety shackle and a secondary safety retainer cable installed as well. The derrick was found to be in good condition with no other major issues noted. The control panels were in good condition and clearly labeled. The crown block and sheaves were in good condition and well maintained. The sheaves were showing to be in good condition with no noted wear showing. The wire rope cables were in good condition and all installed with four part safety shackles and sheaves were in good condition. 11 The SALA block located at the crown was in good condition and installed correctly with a secondary safety cable in place. Los equipos están en buenas condiciones, el retráctil ubicado por debajo de la corona para la seguridad del encuellador cuando esta en el trabajadero.

12 During the derrick inspection, it was noted the Kelly Hose had a cut place in the hose located close to the bottom section of the derrick. This will need to be checked close for depth of cut during the rig up of the derrick system. Will check the installed date and certification of the Kelly hose with the rig Tool-pusher.
