This information packet is designed to provide you with information regarding admission requirements, application procedures, and the admission process. We are happy to help you any way we can. Nursing, as a program of study, is among the most demanding and rigorous. As you plan to enroll, recognize that in order to succeed, you will have to dedicate a lot of time and energy to studying and fulfilling the requirements of the program, but in the end the reward is worth the effort and sacrifices. 4 Associate of Applied Science Degree in Nursing PROGRAM INFORMATION General Description The Associate Degree Nursing Program at Cuyahoga Community College provides the opportunity for students to become registered nurses. Although the course of study is designed to be completed in five semesters (2½ years), most students, due to time constraints, complete the requirements in 3 to 4 years. The aim of the Nursing Program is to provide a broad education. Approximately half of the total credit hours are nursing courses, with the remaining course work evenly divided among science courses such as Anatomy and Physiology and general studies courses such as English. Students attend classes at Tri-C s Metropolitan, Eastern, or Westshore campuses and care for patients at clinical agencies throughout the Northeast Ohio. The nursing courses consist of classroom study; practice in the college nursing skills laboratory using manikins and simulation equipment; and in various healthcare settings caring for patients. Each week, depending upon the clinical nursing course, students will devote approximately 4-5 hours in the classroom, hours in the clinical setting and a minimum of 2-4 hours in the college nursing skills practice laboratory. Laboratory time is a combination of scheduled practice, testing sessions; and, open lab time. It is an expectation that students spend approximately two hours per week viewing videos, utilizing software; and, practicing and mastering skills. There are 3 avenues for entry into the Associate Degree Nursing Program. Applicants may enroll as follows: 1. Applicants with a High School Diploma, GED, or higher may apply for acceptance into the Generic Track. 2. Applicants with a Bachelor s Degree (or higher) may apply for acceptance into the Accelerated Track. 3. Applicants licensed as a Practical Nurse may apply for acceptance into the LPN to RN Track ACCESS in Nursing (Achieving Continuing Career Education for Success and Satisfaction). Generic Track Day and Modified Evening students are admitted to the Program in the Fall and Spring Semesters. LPN to RN Track Modified Evening students are admitted ONLY in the Spring Semester. Accelerated Track Day students are admitted to the Program ONLY in the Fall Semester. The following description of the admission requirements explains how each type of applicant should complete the process. Admission Requirements The admission requirements for the Associate Degree Nursing Program consist of four parts: General Admission Requirements and Associate Degree Nursing Program requirements, Background Check, and Health Care/CPR Requirements. Part I: General College Admission Requirements The applicant must meet the requirements for general admission to the College as listed in the Catalog under the heading: ADMISSIONS, REGISTRATION AND MONEY MATTERS. 4

5 Effective August 15, 2015 Part II: Nursing Program Admission Requirements Generic Track, Accelerated Track, and LPN-to-RN Track* To be eligible for admission into the nursing program, the applicant must meet the following minimum requirements. 1. Students must have a high school diploma transcript, GED Equivalency or higher degree transcript on file in the Office of the Registrar. In addition, Accelerated Track applicants must have a Bachelor s Degree (or higher) from an accredited institution. 2. Completion of the following Core courses with a grade of C or better: BIO-1100 Intro to Biological Chemistry OR have both CHEM 1010 & 1020* ENG-1010 College Composition I** PSY-1010 General Psychology MATH-1250 Contemporary Mathematics (or higher)*** Accelerated Track applicants must complete (grades posted) the following additional courses with a grade of C or better prior to admission: BIO-2331 Anatomy & Physiology I BIO-2341 Anatomy & Physiology II BIO-2500 Microbiology Regardless of the program track, ONLY ONE SCIENCE COURSE MAY BE REPEATED ONCE to improve a grade of less than C. A grade of less than C received over 7 years prior to admission into the Nursing Program is excused from the one course repeat policy. Science course(s) completed over 7 years prior to the date of application to the Nursing Program cannot be used to meet Admission Requirements. These courses must be repeated). 3. A cumulative Grade Point Average (GPA) of If an applicant s GPA from Tri-C is less than 3.00, the GPA from an earned degree from another accredited college or university can be used. For admission purposes if the Tri-C GPA is less than 3.0, transfer GPA from an accredited institution as recognized by the college will meet admission criteria if the student has 12 credit hours in general education courses and an overall GPA of 3.0 or higher. 4. Admission to any track of the ADN program requires passing scores on the Math, Biology, and English Language sections of the entrance test. The Elsevier Admission Test (A2) is required after successfully completing core courses and an overall 3.0 GPA. Achievement of a grade of 80% or higher in Math Skills, 80% or higher in Biology, and composite grade of 80% or higher in English Language portion of the exam is required. Three separate tests compose the English Language portion of the exam. The three tests are: Reading Comprehension, Vocabulary, and Grammar. One attempt can be made per month. If a 2nd attempt is needed only the section(s) below 80% needs to be completed. There is a limit of 2 attempts per calendar year. 5. Complete and pass a background check that includes finger printing and court search (BCI) 5

6 LPNs seeking advanced standing in the ADN program must have a minimum of one year clinical nursing experience as an LPN prior to applying to the ADN program. Applicants to the LPN-to-RN track must have a current LPN license without restrictions and must maintain non-restricted status throughout the program. *CHEM 1010 and CHEM 1020 replaces BIO 1100 for students planning to transfer to a Baccalaureate Nursing Program. **Students who transfer in credits for ENG-1020 with a grade of C or higher, without having credit for ENG-1010 will have ENG-1010 waived, but the required 6 hours in Communication must be earned. ***MATH 1200 meets this requirement; MATH may not be used to meet this requirement. Part III: Background Check The completion of a criminal background check (BCI) is one of the admission requirements to the nursing program. The background check is required to ascertain (i) the ability of students to eventually become licensed in the profession and (ii) the ability of students to attend mandatory clinical rotations at external facilities in accordance with the requirements of the program. Effective January 12, 2013, the BCI process may begin no sooner than six months prior to entry into the program. (Program entry is defined as the first day of class in which the student begins the nursing program). The BCI must be completed, reviewed and cleared prior to full acceptance into the program. The BCI will be valid as long as the student is continually enrolled in the nursing program. Students who leave and return to the program after an absence of one year must complete another BCI. Students returning to any health career program after one year of absence also need to complete another BCI. Background checks completed by another vendor, employer, or while enrolled at another school/institution will not be accepted. Information on the background check requirement can be found by clicking on the Background Check Information found on the Nursing Home page at Part IV: Health Care and BLS/CPR Requirements Prospective students must complete and submit certain requirements for consideration and/or placement in the nursing program. Once accepted into the program, nursing students must maintain these requirements or they will not be permitted into any clinical setting. The maintenance of accurate and up-to-date information of all requirements are the sole responsibility of the student. These requirements include but are not limited to the following: IMMUNIZATIONS Measles -There must be documentation of: Positive antibody titer for Measles (lab report required or employer health report required) Mumps -There must be documentation of: Positive antibody titer for Mumps (lab report required or employer health report required) 6

7 Rubella -There must be documentation of: Positive antibody titer for Rubella (lab report required or employer health report required) Varicella (Chicken Pox) -There must be documentation of one of the following: 2 Vaccinations Positive antibody titer (lab report required) Hepatitis B -A positive Hep B antibody titer is required. If you are just starting the vaccination series you must follow the CDC recommended schedule and then follow the series with a titer. If the titer is negative or equivocal, you must repeat the vaccine series two weeks after the original titer. A repeat titer is required after the series. Repeat titer will be accepted even if negative. (Please see the post vaccination requirements on p. 3) TB Skin Test -There must be documentation of one of the following: 2 step TB Skin test (1-3 weeks apart) 2 tests within one year period (most recent within last 12 months) Past 2 Step PLUS all subsequent annuals 2 consecutive annual tests Negative QuantiFERON Gold or T-Spot test within the past 12 months If TB results are positive, provide a Chest X-Ray (lab report or employer health report required). If INH therapy was received, you must submit documentation of this as well. -Tuberculosis (Single PPD) A single PPD is required annually. Tetanus, Diphtheria, and Pertussis (Tdap) -There must be documentation of a Tdap booster within the past 10 years. Influenza -Submit documentation of a flu shot administered during the current flu season, starting August 1 st. -The annual deadline is October 1. BLS/CPR CERTIFICATION Students are required to provide documentation of a current professional level CPR course. The course MUST be either the BLS for Healthcare Providers course earned through the American Heart Association (AHA) or the CPR/AED for the Professional Rescuer course earned through the American Red Cross (ARC). No other course will be accepted. When submitting documentation of AHA certification, the copy must be front and back of the card. The AHA card must be signed. The ARC certificate of completion is can also be submitted. The ARC certificate of completion must show the unique ID and QR codes for verification purposes. Letters verifying completion are no longer being accepted. 7

8 APPLICATION PROCESS GENERIC TRACK, ACCELERATED TRACK, AND ACCESS (LPN-TO-RN) TRACK 1. Submit a College application. 2. Submit an official high school diploma transcript, GED Equivalency or higher degree transcript to the Office of the Registrar, P.O. Box 5966, Cleveland, OH Once the prerequisite courses have been successfully completed, a request for review can be submitted. The request can be submitted online at 4. In addition to academic requirements, successful completion of the Elsevier Admission Test (A2) is required in order to receive an application to the program. 5. Once all of the admission requirements have been met, the applicant will receive an application for admission to the Associate Degree Nursing Program and a list of available starting dates by from the Division of Nursing. 6. Submit the completed application and any required supporting documentation by to the Division of Nursing. 7. A background check (finger printing and court search) must be completed no sooner than 6 months prior to the start of your program and no later than 8 weeks prior to the start of your program. Go to for additional information. Since a waiting period is likely, the remaining non-nursing courses can be completed during the interim period while waiting to enter the nursing course sequence. Applicants must maintain a cumulative 3.0 GPA for final enrollment into the nursing course sequence. Students who accept a slot in the nursing program (with a specific starting date for nursing courses) and do not attend orientation, decline a slot after orientation, do not attend the first day of class, do not meet all medical record, health insurance, CPR, and/or background check requirements will be required to re-start the application process. Students admitted for a fall semester may reapply after Sept. 1 st ; students admitted for a spring semester may apply after February 1 st. Students must meet all current admission requirements at the time of application. Accelerated Track Applicants Applicants with a Bachelor s Degree (or higher) from an accredited institution may qualify to enter in the Fall session and complete the program in four consecutive terms. These applicants must meet all Nursing Program admission requirements and must have the following prior to beginning the first nursing course: An overall GPA of 3.00 for all college level course work. All core coursework completed All science courses completed Official college transcript(s) are to be submitted to the Office of the Registrar, P.O. Box 5966, Cleveland, OH for transfer credit evaluation. 8

9 Licensed Practical Nurses Licensed Practical Nurses who have been admitted into the Nursing Program may qualify, through the ACCESS (Achieving Continuing Career Education for Success and Satisfaction), for advance placement in the LPN-to-RN Track of the Nursing Program on a space available basis. Additional information can be found on pages LPNs seeking advanced standing in the A.D.N. program must have a minimum of one-year clinical nursing experience as an LPN prior to applying to the A.D.N. program. Applicants to the LPN-to-RN Track must have a current LPN license without restrictions and must maintain non-restricted status throughout the program. Graduates of International Nursing Schools An international graduate nurse educated in a country other than the United States who desires to become a registered nurse in Ohio, MUST contact the Ohio Board of Nursing for information on applying for licensure as a Registered Nurse in the State of Ohio by examination: The Ohio Board of Nursing 17 South High Street, Suite 400 Columbus, Ohio (614) Transfer Applicants Applicants who were enrolled in another college, but who have not taken any nursing clinical courses will follow the general transfer process as described in the Catalog. Note: Non-nursing courses will be evaluated in accordance with general transfer policies and procedures. Grade Point Average (GPA) Procedure for Students with Transfer Credit Students transferring into Cuyahoga Community College Nursing Program must meet the cumulative GPA requirement of 3.0. Students must meet this requirement, for purposes of admission, by one of the following: 1. Transfer all prerequisite courses and have a cumulative GPA of 3.0 or greater from the transferring accredited institution. 2. Obtain a cumulative GPA of 3.0 or greater at Cuyahoga Community College. 3. If the student s cumulative Cuyahoga Community College GPA is less than 3.00, the cumulative GPA from an earned degree from an accredited college/university may be used. For admission purposes if the Tri-C GPA is less than 3.0, transfer GPA from an accredited institution as recognized by the college will meet admission criteria if the student has 12 credit hours in general education courses and an overall GPA of 3.0 or higher. Note: Courses used as prerequisites or core courses for the Health Careers Programs must have a traditional letter grade. The Pass/No Pass option for prerequisites and core courses will not be accepted by the Health Career and Nursing Programs. Students are responsible for consulting with their program manager or counselor to determine Pass/No Pass grading options. Transfer Procedure Request for Advanced Standing Applicants who were enrolled in nursing courses at another college or school of nursing and now wish to transfer into the Cuyahoga Community College Nursing Program must first apply for general college 9

10 admission. Only students who are currently in academic good standing at their current/previous nursing school are eligible for transfer status consideration. Students will not be accepted for transfer who have a greater than two year absence from the program of nursing. Nursing transfer credits will only be granted for credits earned at the most currently attended educational institution. Once the applicant has met the minimum program admission requirements the student must submit to the Metro Campus Division of Nursing, at least eight weeks prior to the desired semester of entry into the Nursing Program, the following: 1. A letter from the Director of the current/previous nursing program sent directly to Cuyahoga Community College Division of Nursing. The letter must indicate academic good standing 2. A letter requesting admission as a transfer student 3. A complete syllabus for each nursing course completed at the previous school(s); and, 4. An official transcript from the previous school(s). These documents will be reviewed by the Nursing Admission & Progression Committee, which will then make recommendations regarding the student s transfer status. (The Committee does not meet during Summer Term). Admission of transfer students who meet the requirements will be placed on a spaceavailable basis. 10

LPN-to-RN Track: Achieving Continuing Career Education for Success and Satisfaction (ACCESS Program) NURS-160A, Access to Registered Nursing and NURS-160D, and Health Deviations I for LPNs were designed to meet the need for advanced placement of licensed practical nurses (LPNs). The LPN-to-RN track is offered in the Spring Semester Modified Evening section only. Enrollment is on a space available basis. Students who seek admission to the LPN-to-RN Track must meet all Nursing Program admission requirements and must have the following credentials for enrollment in NURS-160A and NURS-160D*: 1. Licensed in Ohio without restrictions. 15

16 2. Graduated from an approved Practical Nursing Education Program. 3. Achieved a grade of C or better in each Practical Nursing Course completed. 4. Credentialed to administer medications by The Ohio Board of Nursing (OBN). 5. Official LPN transcript. 6. One-year minimum of clinical nursing experience as a LPN. NURS-160A, Access to Registered Nursing, is an 8-week course comprised of lecture and a college lab. Scheduled lecture and lab typically involve two evenings per week; additional time is required to practice and test out of lab skills. Students must successfully complete NURS-160A in order to progress to NURS-160D. Students can anticipate that two evenings per week will be needed to complete scheduled activities (lecture and lab); additional time is required to practice and test out of lab skills. NURS-160A MAY NOT BE REPEATED. NURS-160D, Health Deviations I for LPNs, has two components: lecture and clinical experience. 17 HELPFUL INFORMATION, RESOURCES & LINKS Student Communications Cuyahoga Community College has established as an official means of communication with students. An official Tri-C address is issued to each student. Students must check regularly in order to read important messages and notifications. Certain communications may be time-sensitive. Failure to read official college communications sent to students official Tri-C e- mail addresses does not absolve students from knowing and complying with the content of those official communications. Students must also ensure that there is sufficient space in their inboxes to allow for delivery. To avoid junk or spam filters, students who are forwarding mail should add their Tri-C address to their address book. Nursing Student Resources Once admitted to the nursing program, many resources are available to nursing students including but not limited to ongoing support of faculty and office personnel, open lab hours for skills practice, tutoring services and more.
