1 LEVEL I PACKET -GLEN OAKS COMMUNITY COLLEGE NURSING DIVISION INFORMATION SHEET THE NEXT LEVEL I NURSING CLASS STARTS IN SPRING OF 2016 AND THE NURSING APPLICATION IS DUE ON DECEMBER 1, 2015, ALONG WITH THE ENTRANCE TEST SIGN UP. The test will be given in 3 sessions, on December 16 & 17, 2015 and sign-up sheets will be posted on the Pre-Nursing board on November 1, The Glen Oaks Community College Laddered Associate of Science in Nursing Degree Program begins yearly. The laddered program consists of Level I (practical nurse) and Level II (registered nurse). In addition to meeting the general requirements of the College for admission, there are requirements pertinent to each level of the program. It is recommended that you work with the nursing office to plan your prerequisite courses prior to applying to the program and you MUST meet with the Director of Nursing before you turn in your Nursing Program Application. The curriculum has been designed to accommodate beginning nursing students at Level I. Graduates of Level I may progress directly to Level II if requirements are met. It offers those students, as well as current Licensed Practical Nurses, the opportunity to continue their education at Level II to meet the requirements for R.N. licensure. Glen Oaks Community College admits to both levels concurrently. Qualified applicants may enter either of these levels as a means for meeting their educational goal. Completion of each level requires three semesters, with the first semester being a COMPRESSED 7 ½ week semester in Spring of each year. There are basic prerequisite courses, which must be completed with a grade of 2.5 (C+) for each class. You must also complete the COMPASS evaluation (given free at Glen Oaks. Prerequisite courses: Take algebra prior to taking chemistry!!! Some of these nursing prerequisites have prerequisites of their own be advised properly to avoid conflicts! **1 credit of high school Algebra AND placement into MATH 104 Intermediate Algebra on COMPASS(score of 46 or higher) OR **MATH-ACT (score of 23 or higher), OR MATH 100 Introductory Algebra with 2.5 or higher grade ** 1 credit of high school Chemistry or CHEM 130 Chemistry with 2.5 or higher within the past 5 years BIO 210 Principles of Anatomy & Physiology I BIO 211 Anatomy and Physiology II PSY 210 Human Growth & Development ENG 121 English Composition ALH 218 Medical Terminology **These courses may be completed in high school or in college (1 semester) Acceptance into Level I of the nursing program is based upon: #1- successful completion of the pre-requisites for the program (listed above); #2-being ranked into the top 40 (there are a limited number of seats set by the Michigan Board of Nursing (at this time: PN-40 seats)). Ranking is based on the entrance test score (percentage) and the GPA of the prerequisite courses for Level I. The ranking score formula is a combination of 75% of the entrance test score, and 25% of the GPA of all prerequisites for Level I entry. The pre-admission test for Level I is free. The test is comprised of four sections: English, Reading, Mathematics and Science and takes about four hours to complete. #3-All health, including medical clearance, background check, drug screening requirements, etc. must be met by due dates established. Applicants who have met the prerequisites in high school or who are taking the prerequisite courses at Glen Oaks may utilize the time prior to admission to take non-nursing courses, which are part of the nursing curriculum. They are identified on the course sequence sheets for each level of the program (in the Level I & II packets online). Taking courses prior to admission will not insure acceptance to the Nursing Program. However, if accepted, taking courses in advance will reduce the number of credit hours the student will take each semester once the program begins. A grade of 2.5 (C+) must be achieved in each prerequisite course. NOTE: You are cautioned to work with the nursing office to keep from over or under loading each semester as it relates to financial aid. Please Note: nursing students are responsible for their own expenses including, but not restricted to, travel, health care, and emergency treatment. Nursing students may be assigned to evening (3-11 p.m.) and night (11 p.m. 7 a.m.) clinical rotations, as it may not be possible for all students to be assigned to days (7 a.m. 3 p.m.). Travel is required to clinical sites in Indiana as well as various cities in Michigan.
2 GLEN OAKS COMMUNITY COLLEGE LEVEL I ENTRY CHECKLIST *GOCC application for Admission, high school & college transcripts (official-transcripts must come in the mail from your high school & college to Glen Oaks and be here before you sit for the entrance test).see attached for transcripts. *Completed Nursing application (received no later than December 1 st, or nearest date previous to it, of the year preceding entry into program). You must meet with the Director of Nursing before submitting your application. Sign up for the entrance test for Level I from November 1 through December one of the year it s being given. After ACCEPTANCE into the nursing program these things will be required: Criminal Record Background Check & Drug Screening (after admission to program). Information provided to you by the nursing office after admission to program. Up to date American Heart Association CPR card for the Healthcare provider (after admission to program). ONLY AMERICAN HEART ASSOCIATION MEETS REQUIREMENT. (A current copy must be in your file throughout the program) Completed health (physical) form. Must be completed by the attending physician and all information on front of form must be complete, including emergency contact information. Information provided to you by the nursing office after admission to program. IMMUNIZATION POLICY RECORDS ARE MAINTAINED BY THE NURSING OFFICE TO MEET REQUIREMENTS OF CONTRACTS WE MUST MAINTAIN WITH CLINICAL SITES THAT PARTICIPATE IN THE EDUCATION OF OUR STUDENT NURSES, AND MUST BE ADHEERED TO BY ALL STUDENTS. ENTRY INTO THE PROGRAM WILL BE DENIED TO ANYONE NOT WILLING TO FOLLOW THE POLICY SINCE WE WOULD NOT BE ABLE TO PLACE YOU IN CLINICALS ; THEREFORE YOU WOULD NOT BE ABLE TO COMPLETE THE NURSING PROGRAM. Proof of Hepatitis B immunity through submission of a health record showing the 3 Hepatitis B shot series OR Hep B Surface (AB) Anti-Body *titer is REQUIRED. Immunization record with 2 MMR shots. In lieu of 2 MMR proofs you may submit titers showing immunity for Rubeola, Rubella and Mumps. Negative Tuberculin test (this is required yearly in health professions), or negative chest X-Ray-required every 2 years. Verification on a health record of Chicken Pox as a child or Varicella Zoster vaccination or titer validating immunity. TDap within 10 years. Influenza vaccination(s) completed in season (October deadline). *Please note: no file will be started in the nursing office without your application to GOCC AND your NURSING application being completed 9/8/15
3 1. Meet with the Director of Nursing (DON). ADMISSION CRITERIA FOR LEVEL I 2. Submit a nursing program application to the Glen Oaks Community College Nursing Office. (You may only submit your nursing application after meeting with the DON. The deadline for submitting an application is December 1 st or the nearest business day before December 1). 3. Complete a COMPASS evaluation (given free at Glen Oaks), administered by the Testing & Tutoring Center. 4. Submit a transcript verifying high school completion or scores of the General Education Development (GED) examination. The transcript must be mailed directly from the institution to the Glen Oaks Admissions office. Submit an official transcript from all other institutions of higher education you attended. High school algebra (1 credit) or its equivalent; for example NSM 100 at Glen Oaks Community College. Placement into NSM 104 Intermediate Algebra or higher on the COMPASS (score of 46 or higer) evaluation is needed for both high school algebra and college algebra that is older than 5 years. Algebra, NSM 100/104 must be taken prior to taking Chemistry. 2. High school chemistry (1 credit) or its equivalent; for example NSC 130 at Glen Oaks Community College (within the past 5 years). 3. BIO 210 Anatomy & Physiology I 4. BIO 211 Anatomy & Physiology II 5. PSY 210 Human Growth & Development 6. ENG 121 English Composition 7. ALH 218 Medical Terminology 7. Take the Kaplan Entrance pre-admission examination administered by the Nursing Division of Glen Oaks Community College and achieve the minimum score or higher to be considered for admission. There are a limited number of seats set by the Michigan Board of Nursing (enrollment in Level I is limited to 40 students) and applicants are ranked for admission. The ranking score formula is a combination of 75% of the entrance test score, and 25% of the GPA of all prerequisites for Level I entry.
4 ADMISSION CRITERIA-LEVEL I (CONT.) Priority for admission shall be given to highest ranked applicants. If the number of equally ranked applicants exceed available seats, the following criteria will be used to determine admission, in the following order: 1) In-district applicant. 2) Date Nursing Application was received. 3) Date Glen Oaks Admission Application was received. Acceptance into Level I is conditional until documentation supports the following: a. A physical exam that reveals no medical reason which would exclude the student from participating in the nursing program. b. A record of current immunizations per immunization policy. c. A current CPR for the health care provider card from the American Heart Association. Both the CPR card and the immunizations must be kept current for the duration of the program. d. A Negative Drug and Alcohol screen will be required. Any criminal felony or misdemeanor charge(s) on a public record must be reported to the DON with submission of your nursing application or earlier. Acceptance into Level I is conditional upon a negative criminal background check, as defined by the Glen Oaks Criminal Background Check Policy; and negative drug and alcohol screen.
5 Glen Oaks Community College LADDERED ASSOCIATE DEGREE NURSING PROGRAM LEVEL I EXPENSES The following are the APPROXIMATE costs of a student enrolling in the Nursing Program at Glen Oaks for the academic year Tuition per contact hour*: In district $103, Service Area $158, State of MI $166, Out of State, $200 General Fees - $25 per contact hour* - Approximately $1450 Student Activity - $4.00 per contact hour Approximately $192 Individual Course Fees Approximately $833 Books Approximately $500 (required texts) per semester Other expenses include but are not limited to: Physical examination yearly, American Heart Association CPR for the Healthcare Provider, and required immunizations prior to beginning nursing clinical courses Background check and drug screen 1 st year and drug screen repeated 2 nd year, close to $100 initially Student uniforms (2-recommended), white shoes, watch with sweep 2 nd hand, small note pad. Costs vary because of student preference. School graduation pin - $46 to $95 (costs vary) Optional expense State Board Application fees at this time $54 (MI) New in 2008 states (MI & IN) require fingerprints before you sit for your test at an approximate cost of $63 done through the state, not through Glen Oaks NCLEX Examination each year ($200) Transportation costs that depend on student needs *A contact hour is equivalent to the time the instructor spends in the classroom or at the clinical site per week. In district students are: St. Joseph County residents Service area students are: Cass County residents in the following school districts; Three Rivers, Constantine & White Pigeon. Branch County residents, LaGrange IN County residents, Elkhart County IN residents and Steuben County IN residents State of MI students are: Students outside of St. Joseph County and outside of Service Area who are residents of Michigan. Out of State students: Students outside of Michigan and Service Area, including International Students

9 STUDENT EXPENSES OTHER THAN TUITION, FEES AND BOOKS In addition to the current in-district or out-of-district tuition, nursing students shall be responsible for the following: - Personal health requirements/physical examination and history - Current TB testing, immunizations and required titers - Current CPR from American Heart Association, for the health care provider; status validated with CPR card - Fit Mask Testing - Drug and alcohol screening - Criminal background check - Transportation costs - Clinical equipment; stethoscope, scissors, penlight, pocket notepad, black ink pen, BP cuff (available in bookstore) - Uniforms, shoes, socks (all white) - Watch with second hand or digital capability to display seconds - Nursing Pin/Lamp - Sitting fee for composite picture of graduating class (optional) - NCLEX (National Council Licensure Examination) Application - Nursing License application fee, Live Scan Fingerprint scan now required before you can sit for the NCLEX test - Health insurance and expenses related to illness, injury or *random drug/alcohol screen are the responsibility of the individual student and/or the student s family Other fees may apply
