1 Electronic Signature Instructions Please complete ALL of the fields on the form. When you are finished, click in the Signed By field at the bottom of the page. It is important that you complete all of the fields on the form before you sign the document. Prematurely signing the document before you complete all fields will lock your answers and you cannot change them. **You must have Adobe Acrobat Reader 8.0 or higher in order to sign the Signature Instruction forms.** Instructions for Adobe Acrobat 9 or X When you click your mouse in the Signed By field, a dialog box will open. If you already have a Digital Signature: If you already have a digital signature created, you may use that under the Sign As option. Then click Sign. Your computer will ask you to save a copy of the document. We recommend saving it with a unique name (such as your last name or the last name of the person proctoring). When you are finished, click on the Submit Form button at the top of the form page.

2 A dialog box will open and you will need to enter your Name and address. Then click Send and another Dialog Box will open. If you have a Desktop Application such as Outlook, please select that option and click OK. You have now completed your form. If you do not have a Desktop Application, you will need to send the completed form to Christina Hills at [email protected] to finish your form.

3 If you do not already have a Digital Signature: A new dialog box will open: Then click Next. On the next screen, pick Windows Certificate Store: Complete the fields on the next screen with your information:

4 And click finish. The dialog box will close and you may click the final Sign button. Your computer will ask you to save a copy of the document. We recommend saving it with a unique name (such as your last name or the last name of the person proctoring). When you are finished, click on the Submit Form button at the top of the form page. A dialog box will open and you will need to enter your Name and address. Then click Send and another Dialog Box will open.
