You re receiving this report as a personal thank you for your past support. Your generosity helped provide care to 51 residents in 2015 and 425 residents since we opened our doors in Your gift also gives the family and friends of our residents peace of mind knowing their loved one has 24 hour care that emphasizes compassion, respect and dignity. It is because of your gift Omega House is able to offer the quality of care we are known for. In this year s report we are once again privileged to share stories from families, staff and volunteers as they express their gratitude for the care given to their loved ones. We feature our volunteer of the year and how he was inspired to begin volunteering at Omega House. We also said goodbye to several board members as well as our Development Director, Ray Weglarz, who retired at the end of the year. Our Resident Care Assistants are also featured to give you a glimpse into the people who provide your loved ones with such compassionate care on a daily basis. With your continued support we look optimistically to the future. We remain committed to providing personalized, compassionate, quality end of life care to those in need. Please continue to include Omega House in your annual charitable giving, and I ask you to consider leaving a legacy of compassion by including Omega House in your will or estate plans. Sincerely, Michael Lutz Executive Director Ten years have passed since Omega House served its first resident in September, When I reflect, I remember people who had a vision of caring for dying patients who wanted to receive care either at home or in a homelike setting. That vision became reality thanks to their hard work and the enormous support from our communities. Also, during the past 10 years, many changes have occurred in healthcare. These include significant changes in regulations and reimbursement, and affect the delivery of healthcare in both positive and negative ways. In 2015, Omega House served 51 residents over 400 since we opened our doors in Our rare model of comfort care to our residents utilizes hospice staff and resident care assistants, who, with volunteers, family and friends allows us to provide individualized person-centered care. As we enter the next decade at Omega House, please join me in welcoming our first Executive Director, Mike Lutz, who joined us in January. The board of directors felt this change was necessary to meet our immediate needs and our long term vision, To be a community leader and resource for end of life care. 3 I cannot adequately express how much it means to be able to have my husband in your care at Omega House. He was in a nursing home for several months after suffering two strokes. The decision to bring him to Omega House was not difficult, and it has been a very rewarding transition for both of us. We have been married for only three years and all of the setbacks and separation have been difficult for both of us. However, at Omega House I am able to spend as much time as I possibly can with my husband. Omega House is the closest thing to being right at home together. All of the staff and volunteers are so kind, helpful, and friendly. They truly are a blessing to the patient as well as to me, for which we will be forever grateful. Your care and compassion and keeping him comfortable is outstanding, and I will be forever indebted for treating my husband with so much dignity, respect and love. It is, without a doubt, comforting, and I sincerely thank you for every day that you care for him. May God bless you all. Bette Lundin Wife of Don Lundin Compassionate Care Don Lundin My son was a combat medic in Iraq during war time and during that period of my life the war became painfully personal. Omega House has provided me with a purpose and given me tremendous support which enabled me to not think about the battles we fight in other areas of the world. Debbie Gagnon My brother, Dr. David Nara, spent his last thirteen days of life at Omega House. Dave had stage IV glioblastoma (brain cancer), and at the young age of 46, was not expected to live six months. Following six weeks of radiation, and one year of chemotherapy, Dave s cancer was surprisingly in remission. Six years later, he started to decline. Watching him undergo repeated seizures became too difficult; and we opted for a second brain surgery. Dave never thrived after his second brain surgery. He wanted to return home, but was unable to reach that goal. Further medical treatment for his cancer was not an option at this point, and we decided to enter him into Omega House. Dave received such loving and compassionate care at Omega House. Staff members were incredible and the facility itself is so warm and home-like. The view from his window was very calming and serene. The staff was always so helpful and made us feel welcome. Since we felt the need, emotionally, to be with Dave around the clock, staff members were very accommodating to allow us to sleep in a recliner next to him. His son Thomas spent nights with him and I spent most of A Sister s Love the days and evenings. Dave s girlfriend, Tammy, was able to visit frequently and spend quality time as well. The staff made all of Dave s friends and visitors feel welcome and not a burden. I am so glad we chose Omega House for these final days of life for my brother. The quality of care he received there was outstanding and I would highly recommend it to others. Thank you to all for everything you did for him. We will always remember your kindness and compassion. 4 Honoring Our Board Members Rev. Christine Bohnsack Reverend Christine Fryxell Bohnsack, known to many as Tina, joined the Omega House board of directors in the winter of 2005 just months before the house opened. She served faithfully with a deep sense of commitment to the Omega House mission. During her ten years of service she endeared herself to many with her wonderful sense of humor and her dedication. She served as board secretary, vice president, board representative from Baraga County and chair of the We Honor Veterans committee. Sadly, Reverend Bohnsack passed away unexpectedly in March In 2015 we saw the retirement of two board members and the passing of another. All three of these board members had been active members for many years. Mike Abbott At the end of 2015 Mike Abbott stepped down from his position on the Omega House board of directors after 10 years of dedicated service. He was one of the most active members of the board serving in a leadership position as vice president and board president for four years. Omega House is a natural fit for my caring nature. I want to help every resident have a meaningful quality of life as they complete their journey. Ruth Wanhala Dr. Jeff Huotari Dr. Huotari was also an active board member who brought passion, intellect and experience to the table in support of our mission. He was the innovator of our fundraiser, Hike for the House and was appreciated for his precise thinking and analytical skills. Farewell to Ray Weglarz At the end of January 2016 Ray Weglarz retired as Omega House Development Director and Donor Relations. He began his involvement with Omega House in 1998, when he and other community volunteers began the work that became Omega House. For 20 years, Ray devoted his work to hospice care, serving as patient care coordinator and hospice nurse for the Keweenaw Home Nursing Hospice program. During those years, he saw firsthand the suffering of those who did not have sufficient support to die at home. The experience he gained working with the dying lead to his involvement in helping to found Omega House. Ray believes that maintaining dignity and quality is crucial for end-of-life patients. He says, We can help reduce fear and anxiety too often associated with this sacred time of life. Ray authored the photo-essay book, On the Way Home Living with Terminal Illness, which shares stories of growth, connection and compassion with those nearing the end of life. At Omega House, Ray worked with the board, staff, volunteers, family members and residents to share the work and mission of Omega House. He also worked with dying veterans, and became involved with the Michigan Hospice Veterans Partnership. Ray said, Working with our community and its donors has been a wonderful experience. I was blessed to be part of this organization and to have worked closely with so many giving people who are generous with their time, talents, and resources. Ray will continue to volunteer his time at Omega House, where he will promote the importance of caring for our elders and advocating for hospice care. From the board of directors, staff, volunteers and the countless family members and residents who have worked with Ray, We congratulate you on your retirement. 5 Volunteering The best way to find yourself is to lose yourself in the service of others GANDHI Helping Others While Helping Yourself 2015 Volunteer of the Year Rich Featherly I am honored to receive the Omega House 2015 Volunteer Award. I stand on the shoulders of greatness when it comes to volunteering, but it s nice to get the recognition anyway. I really only started to volunteer at Omega House for my own selfish interests. I was beginning a career change and wanted to stay active in something that could be included in my resume. How best to do this was to associate myself with one of the most respected facilities in the area. Little did I know I would find myself working with some of the most accepting and generous members of the community, the board, employees and volunteers of Omega House. It soon felt like a family to me. I love to see how well the residents of Omega House are treated. No matter what their situation in life before moving to the House, they receive equal care, with compassion, kindness and patience. I have listened to family members and even heard a resident lately who wondered out loud what kind of people want to work in a hospice setting. The answer has to do with people who are willing to care for others, even in the face of certain loss in other words, people who really care. I consider myself fortunate to be able to surround myself with people like that. I hope to continue to volunteer at least a few hours a week and help in any other way well into the future. Your impact on the community. Unpaid volunteers are often the bond that holds a community together in a positive way, while you connect with people and expand your network of friends. Volunteering helps to provide a healthy boost to your self-confidence, esteem, and life satisfaction. Individuals who volunteer say they feel healthier. By staying in contact with others, you will develop a support system, which may reduce depression brought on by isolation. Physical activity involved in certain forms of volunteering can be good for you at any age. You have a chance to explore your interests and passions by doing meaningful and interesting work, where you might find renewed creativity and motivation. Research shows that volunteering just 2-3 hours/week (about 100 hours/year) is most beneficial to both you and your chosen cause. But remember to volunteer only the amount of time that feels comfortable to you it should feel like a fun and rewarding hobby, not another chore on your to-do-list. At Omega House we provide many meaningful opportunities to volunteer. You have the opportunity to do a variety of tasks. Your Loss & Grief For every one of us, there are many times in life when we have great loss. The emotional pain, and sometimes physical pain, that come with a loss can be some of the greatest challenges we ll ever face. From each loss, we learn and grow and each of those moments helps us on our journey through life. No matter what kind of loss you are facing today, a loved one, a relationship, a possession, a job, your identity, or your own health, you may be wondering how you ll work through it. Every loss is Working at Omega House has shown me that the last days of a person s journey can be very peaceful and loving. The quiet atmosphere and cozy nature of Omega House makes it feel like home. April Wickstrom different. Every person s reaction is different. Life does not come with rules on how hard or long you should grieve. Grieving in your own way and in your own time is best for your well-being. Right now, you may feel like healing is a long way off. That s a normal reaction. How you begin to heal is your personal choice. Take time to think about your loss and how it affects your life. Grieving isn t about pushing the pain away or getting though it fast. When we allow ourselves to feel the grief, we allow the process to naturally unfold which in time permits you to move on. Talking to family members, friends, religious leaders, and counselors about what you need may help you. Keep a journal to help you sort through your feelings. Try prayer, meditation or relaxation ideas. Wait to make major decisions until your grief is lessened or possibly speak to someone you trust if you have to make a decision now. Sudden waves of emotion, called grief bursts, are normal to have. These sudden waves occur even when everything seems to be going fine in your life. No matter what else is going on, it s important to take care of your physical health as you process your loss. Perhaps, small, easy goals may help to motivate you. Eat well, get enough sleep, do activities that you enjoyed before your loss and try to get physical activity daily. Although we never get over loss, in time, you can have a new normal way of life. Do not fear Grief. To fear grief is to fear love. To fear love is to fear life. Dr. William Michael At Omega House our strategic vision is to provide our communities with the resources and programs necessary to educate those affected by grief and loss. We will do this by partnering with other agencies in our area and the goals of the program will include: Providing grief support from trained volunteers and developing a network of grief support volunteers to those in need. Providing community-wide and focused educational and support programs for individuals, families, schools, civic organizations and faith groups. Having a designated program director, office space and resources materials at Omega House. Employing strategies to make the program self-sustaining in the years to come. 9 Meet Our Caring Staff Mary Foss I live in Houghton and have three adult children and two grandchildren. I have worked in healthcare for most of my adult life. I began working at Omega House in 2005 and enjoy working in a hospice setting. My time at Omega House has provided me valuable skills I utilize to take care of my loved ones at home. Debbie Gagnon I have been a nurse s aide for 33 years and have worked at Omega House for the past 10 years as a part-time resident care assistant. I also have been working for the past 25 years as an aide for Aspirus Health at Home and I am nationally certified in hospice and palliative care. I live in Houghton with my best friend, a little tiny dog by the name of Violet. I have two wonderful children and two beautiful grandchildren. Kayla Semmens I have worked as a CNA for the past four years and I have been working at Omega House for nearly two of those years. I love what I do here and I wouldn t trade it for the world. When I m not working, I like spending time with my family and friends. I enjoy camping, being outside, playing soccer, and adventuring in the Copper County with my fiancé and our dog Bella. Donna Wade I have worked part-time at Omega House for six years. However, prior to that I worked at Portage Health in the emergency room for 37 years retiring in My pride and joy is spending quality time with family, friends and my grandchildren along with reading and traveling. Jeanine Seratti Hulkkonen I have been a nurse s aide for 26 years and for the last five years I have been working at Omega House. I experienced hospice care personally when my mother Alyce Mae took ill and I was privileged to work alongside of the caring hospice team. I am married with two wonderful sons Michael and Alex. I am also a huge animal lover and have three dogs. April Wickstrom I have been an aide for 20 years and have been working at Omega House for almost two years on a part-time basis. I live in Lake Linden and have two teenage sons. My favorite hobbies are boating, fishing, camping and hiking the many trails this area has to offer. Alissa Keating I have been a resident care assistant with Omega House for just over a year. I am dedicated to taking care of others and I have a passion for animals and a love for the outdoors. Jackie Mace I have been employed at Omega House for over 10 years. Several years before that, I began my career in homecare but my passion was to always work with hospice patients. When Omega House opened its doors I was able to make that passion a reality. It s very gratifying to help our residents and their families at such a difficult time in their lives. I live in the Calumet area with my wonderful husband of 46 years. We have four children and six grandchildren so spending time with my family is very important to me. Ruth Wanhala I have spent 47 years as a nurse s aide spending 42 of those years at Aspirus Keweenaw Hospital. I was born and raised in Mayflower location and still live in the same house on 20 acres of land. I have always been the type of person that wants to help anyone in need. I cared for both my parents in their dying days in order to respect their wishes of dying at home. Stephanie Wolck I have been a CNA for 11 years working mostly within hospice care. I have been working at Omega House for nearly a decade. I am full-time now working the night shift at Omega House. I am married and have one beautiful daughter

We are always pursuing additional services that will enhance the quality of life for our residents. We are pleased to announce Omega House was recently awarded a grant from the Superior Health Foundation to start our Caring Hands Massage Program. This massage program began on April 8th, 2016 and all services are provided by State of Michigan licensed massage therapists. Omega House serves, on average, over 40 unique residents per year who will benefit from the Caring Hands Massage Program. In addition, family members visiting frequently who participate in their care, and our staff of resident care assistants, can receive these services. Some family members spend many hours, and at times many days, with residents in their final days and weeks of life. Our resident care assistants provide lifting, transferring and other duties that require short periods of muscular exertion. Stress reduction and relieving of muscular and emotional tensions are results from massage therapy. Research has shown that massage therapy can provide comfort and relaxation, and help alleviate the following symptoms and conditions: Pain Anxiety Loss of sleep Depression Stress Nausea Fatigue Mood disorders This service is the first of its kind in our area. Current statistics show that hospice care nationwide is increasing each year. In 2015, nearly 1.7 million patients were cared for in a hospice program. This is true for our small rural communities in the Upper Peninsula. Hospice care is growing, and Omega House feels that additional programs and special services such as the Caring Hands Program set us apart and aligns with our vision *Deceased 25
15 How can I give to Omega House? Annual Gifts Annual donations to Omega House support the daily operations of the House. These gifts are essential for us to fulfill our mission. Annual gifts can be designated in memory or in honor of a friend, loved one or someone important in your life Please consider becoming a Sustaining Donor with on-going donations via a credit or debit card. [email protected] Founders Fund The Founders Fund recognizes individual who make donations at the $25,000 level. These gifts are payable over a period of one to three years. A gift of this level provides resources to operate the entire house for a month. It helps ensure that no will be turned away. The Founders Fund is named for the early organizers who helped make the vision of Omega House a reality. Veterans Fund Since opening our doors, U.S. military veterans make up 30% of all residents. We are honored to serve them. You can recognize a veteran, a branch of service or a veteran s group with your gift. Naming Opportunities Omega House has naming opportunities available for donations small and large. This is a unique way to forever honor or remember a loved one. For gifts of $1000 an etched glass panel commemorative wall has been created that greets visitors as they enter the house. Events Events are held in the community throughout the year to benefit the house. Some events are planned and conducted by Omega House staff, volunteers and board members. Other events are planned and conducted by individuals and organizations in the community on behalf of Omega House. 16 Omega House Legacy Society You can become a member of the Omega House Legacy Society and leave a legacy of compassion by including Omega House in your will or estate plan. A Legacy Gift to Omega House may provide tax advantages for you and your heirs. Your Legacy Gift to Omega House can be recognized or you may remain anonymous. We encourage you to consult with your tax advisor, attorney, or financial planner regarding such gifts. The options listed give you flexibility, control and the enjoyment of your assets during your lifetime. Bequest A bequest allows you to support Omega House in the future without gifting away your assets during your lifetime. You can make a bequest donation in your will or living trust by designating Omega House as the beneficiary of cash, real estate, securities or by leaving us a percentage of your estate after all other obligations have been fulfilled. A bequest remains revocable until your passing, allowing you to change your mind at any time. Beneficiary Designation The simplest way to leave a Legacy Gift to Omega House is through a beneficiary designation. Some assets like retirement plans, life insurance policies, and annuities are not distributed as part of your will or trust. The beneficiary is determined by you or the beneficiary designation form provided by your account administrator. You can request to update your designation form at any time and simply name Omega House as the full or partial beneficiary of your plan s assets. You may change your mind at any time during your lifetime. Real Estate A gift of a remainder interest in your home, camp, cottage or vacant land allows you to enjoy and control the property during your lifetime, and transfer ownership to Omega House upon your death. Such an arrangement avoids probate, saves your heirs from having to maintain or sell the property after your death, and may even provide lifetime income tax benefits. This type of gift will not impact your property taxes, as the principal residence exemption will be unaffected, and the taxes on your property will not be uncapped. This gift can also be changed or revoked if your circumstances and needs change. Contingent Gifts Contingent Gifts only come into effect should something unforeseen happen to your primary beneficiaries. Contingent gifts can be made by a will, trust or a beneficiary designation form. Other Gift Options More complex gift options include: Charitable Remainder Trusts, Charitable Lead Trusts and Charitable Gift Annuities. These gifts can provide tax advantages for you and your heirs but do require the assistance of an estate or financial planner experienced in these matters. Omega House Planned Giving Advisory Council The Panned Giving Advisory Council is comprised of community volunteers who serve in an advisory capacity to the Board of Directors in areas related to their knowledge and experience.
