6 Represents a challenge in what concerns the need of a systematic adjustment between the intervention and the assessed needs and risks; Founds the goals to be achieved by offenders and established all the actions and activities to be carried either by professional staff and offenders; Promotes the participation of offenders in the process of planning, projecting them as actors of change of their own situation and behaviours; Involves a set of professionals with different levels of responsibility working in a participatory methodology in order to create a more efficient set of social rehabilitation conditions for offenders. 6

9 PURPOSES: Provides structured information about the intervention to be undertaken Records the main areas of intervention matched to needs and underlying risks of the offender Indicates goals, and means to achieving them (concrete actions and activities including monitoring and evaluation procedures) target to specific needs Provides management with information about clients and facilitate communication among professionals 9
10 Individualized and unique (a Plan for each offender during the length of the sentence) Conducted as a significant event for the offender Requires the involvement and empowerment of various sectors and staff of the Prison; The Prison Director is responsible for its approval (and amendments) Submitted to final approval to the Execution Court Law 10
11 As a process A planned and timed-bound set of interventions anchored in the pursuit of objectives previously defined Identifies progress to achieve, and outcomes May contain multiple objectives and actions Specifies who will do what and when Is dynamic and can incorporate new objectives Is evaluated and updated periodically 11
12 Comprehensive analysis of the case, based on the information collected through the assessment tool - SARNC (acess to the offender profile) Identification of the areas to be interventioned Prioritizing the intervention (take into account risks, needs, motivation, offender s learning style, sentence s length, resources available) Evaluation (analysis of the impact of outcomes in the offender s profile) Work plan: - identification of objectives (time-bounded and outcome focused) - actions/activities to develop (i.e. support and monitoring actions; learning activities) - identification of key workers (who will do what and when) 12
