3 Providence Preferred Provider Directory About Providence Preferred Providence Preferred is a preferred provider organization a network of physicians, hospitals and ancillary service providers throughout Oregon, Idaho and southwest Washington designed to manage the cost, delivery and quality of health care. You and your family can enjoy substantial savings on a wide variety of quality health care services offered by Providence Preferred through the preferred providers and hospitals listed in this directory. These participating providers and hospitals have agreed to offer their services at special rates to Providence Preferred patients. Make sure you check with your insurance carrier or claims payer to determine the benefit differences between using a preferred and non-preferred provider. As part of Providence Preferred, you receive quality medical care, plus you have the flexibility to choose your own physicians within our network of nearly 22,000 providers. Should you choose a non-providence Preferred provider, you may be responsible for additional out-of-pocket expenses. This directory lists those medical providers participating in Providence Preferred, including our mental health and chemical dependency network as well as our alternative care network. Please contact your insurer to confirm that you are eligible to access our providers as well as to confirm that an insurance benefit exists. You are eligible because your employer offers Providence Preferred as an addition to your health benefit plan. Does Providence Preferred pay my claims? Providence Preferred is your physician network but not your insurance company, and therefore does not pay your claims. Your self-insured employer, union trust or your employer s insurance company has contracted with Providence Preferred in order for you to receive discounted medical services. What are the benefits of using Providence Preferred physicians and hospitals? You will receive quality health care from some of the area s most respected private practice providers and hospitals. Your health insurance may offer a higher level of benefits when you use Providence Preferred providers and hospitals. You and your employer will save money through lower out-of-pocket expenses. Remember, participating physicians and hospitals agree to accept preferred rates for services provided to eligible Providence Preferred members. Providers and hospitals bill for you. Your Providence Preferred provider is responsible for all pre-approval requirements through your group s medical management program. We recommend making sure pre-approval is complete before you enter the hospital or a provider s office for a procedure. What if I have an emergency? In a case of a life-threatening emergency, go to the nearest hospital. If the hospital participates with Providence Preferred, plan savings will apply. For minor emergencies, visit a participating provider or urgent care facility. Can Providence Preferred answer questions about my benefits and deductibles? Please contact your employer or insurance company regarding any benefits questions you might have. What happens if my benefits plan does not cover a service I received from a preferred provider? For services not covered by your health plan, you are responsible for paying the provider s usual and customary rate. Are all provider specialties listed in this directory? A wide variety of providers are listed in the Providence Preferred network. Please check with your insurance carrier or trust to make sure the provider you have selected is covered by your health plan. A primary care provider, also referred to as a PCP, is a person who manages the majority of your primary health care needs. This provider can be a family practitioner, internist, pediatrician, general practitioner and, in some cases, certain women s health care providers, nurse practitioners and physician s assistants. ii
