We know switching banks can be a bit intimidating, but at Mainstreet Community Bank of Florida, we ve made it as simple as possible. In fact, we ve done some of the work for you already! Our Switch Kit provides everything you ll need to make the transition easy - we ve even included pre-written forms to assist with the transfer of your existing automatic payments and direct deposits. Simply follow the steps below to ensure a smooth transition from your previous bank. Remember, our Customer Service Representatives are here to help you every step of the way! Contact us if you have any questions or need assistance. 1. Open a Mainstreet Community Bank of Florida account. Stop by any Mainstreet Community Bank location and speak with one of our friendly and knowledgeable Customer Service Representatives. We ll help you choose the account that best meets your financial needs. Once your account is open, let us give you a fast and easy demonstration on how to access and manage our online banking and bill pay services. 2. Stop using your old account and redirect your direct deposits and automatic payments. Important: Keep your old account open until you have verified that all outstanding checks, final direct deposits and/or automatic payments have cleared. Review your old account for direct deposits and automatic payments. Take a moment to review your old account. Use the attached Account Review Checklist to make sure that you ve identified all of your current automatic payments and direct deposits. Each company you list on this checklist will need to be notified of your switch to Mainstreet Community Bank of Florida. Switch your automatic payments and direct deposits. If you have existing automatic payments and direct deposits linked to your old account, you ll need to switch them to your new Mainstreet Community Bank account by contacting each affected company. We ve designed the following forms to help you with this process. Use the attached Direct Deposit Change Notification Form & Automatic Payment Change Authorization Form to notify each company of the switch and that you would like to redirect automatic withdrawals and direct deposits from your old account to your new Mainstreet Community Bank account. Note: If you currently receive direct deposits from Social Security, you ll need to call (800) to make the switch. They ll need to talk to you directly prior to making a change to your direct deposit. 3. Close your old account. Once all outstanding checks, final automatic payments and direct deposits have cleared your old account and have been successfully redirected to your new account, you ll need to notify your old bank that you want to close your account. Use the attached Account Closing Authorization Form to notify your old bank that you want to close your account with them. Be sure to destroy all old checks, deposit slips and cards (ATM/Debit) associated with the old account.

3 Account Review Checklist Use this form along with your most recent bank statement to identify all of your automatic payments and direct deposits. Each company you list on this checklist will need to be notified of your switch to Mainstreet Community Bank of Florida. Some of these items may be setup in your current online banking/bill payment system and will need to be added to your new Mainstreet Community Bank NetTeller online banking account.
