Please read carefully all the information provided in this document and the relevant Explanatory Note(s) before completion of the application. 2. Please complete separate application form for each Hosting OG. 3. The completed application form should be submitted by fax at or by to and followed by telephone call at to confirm submission. 4. All existing terms and conditions including fees and charges for the application and use of Hosting OG shall apply. 5. Before Exchange Participant (EP) applies to install Hosting OG, please ensure you have already subscribed HKEx Hosting Services. 6. EP is responsible for procurement of hardware, software packages and network switch(es) for its Hosting OG, liaises with HKEx Appointed Service Vendor to install the software packages, arranges with its vendor for service delivery, installation, repair and various types of maintenance service activities. The minimum hardware requirement, software package and contact information of Appointed Service Vendor are set out in Explanatory Note on the application form. 7. The Hosting OG will connect to HKEx Service Network (HSN) in the HKEx Hosting Data Centre. EP is responsible to contact HKEx - Hosting Services directly for HSN circuit quotation, installation and billing arrangement. Or if the connection is HSN Virtual Port Service, please consult the Approved ASP for the corresponding HSN circuit number. Approved ASP is a Hosting subscriber who provides Approved ASP services. 8. EP terminates the use of Hosting OG can be made with a 1-month advance notice to HKEx Cash Trading. If EP also terminates the Hosting Services which are in separate service agreement, please contact HKEx - Hosting Services. 9.
