2 Welcome to the Practice This booklet tells you about the practice and services we offer, therefore please read it carefully. We trust that you will find it helpful and informative and we suggest that you keep it in a safe place for future reference. In 1968 Chelmsley Wood housing estate was built. Shortly after this the doctors set up practice in a house on Greenlands Road. After a short time the practice moved to purpose-built premises in Craig Croft, Chelmsley Wood. In June 2012 the practice moved into temporary accommodation with a move into newly built premises estimated for The partnership is contracted to Birmingham and Solihull NHS Cluster. Birmingham and Solihull NHS Cluster was formed in 2012 and replaced the Solihull Primary Care Trust. Its aim is to bring together all the people who deliver healthcare, treatment and advice in the community eg GPs, Health Visitors and Therapists. The Cluster is also responsible for arranging healthcare provided by hospitals. The Cluster can provide details of primary care services in the Solihull area. These appointments are available to book up to 2 weeks in advance and sessions are held on a Tuesday evening between pm, Thursday evening between pm and Friday evening between pm. These appointments are for routine consultations and are not intended for emergencies (the BADGER out-of-hours service is still in operation to cover emergencies during these times). Failure to attend an appointment without prior notification of cancellation (24 hours) may result in this facility being withdrawn on an individual basis (at the practice s discretion). For security reasons the surgery doors will remain locked during these sessions and access for the patient attending and, if necessary a carer, will be by knocking on the front door. No nurses will be on the premises. Staff And GP Training Birmingham and Solihull Cluster encourage the practice to have protected learning time for staff and GP development. Therefore the practice will be closed occasionally. You will be notified in advance by posters throughout the practice. During these times our outof-hours service will be in operation - please telephone Other Surgery Staff Practice Manager Ms V Parkes is responsible for all financial aspects of the practice and the smooth running of the practice, including the administration. Please feel free to contact her if you have any queries or concerns. Deputy Practice Manager Mrs S Phipps is responsible for infection control and health and safety. She deputises for the practice manager in her absence and is the administration team leader.

4 Secretary Mrs W Earl is responsible for general secretarial duties and liaison with hospital staff, Monday to Thursday 9.00am pm and Friday 9.00am pm. Administrative Team The administration team are responsible for the processing of medical patient data and reports. Reception Team Leader Mrs V Howell is responsible for the smooth running of reception, Monday to Friday. Receptionists Their aim is to provide an efficient service to the patients and to ensure smooth running of the day-to-day practice activities. Practice Nurses Available pm & pm Monday to Friday Mrs Claire Mason: Nursing Team Leader. Nurse Prescriber. Specialist interests - respiratory disease, contraception (including coil and implant fitting). Mrs Jacqui Rea: Specialist interests - respiratory disease, cancer care. Mrs Sue Nightingale: Nurse Prescriber. Specialist interests - diabetes, heart disease, chronic kidney disease. Health Care Assistant Jo Marshall NVQ 111 Rebecca Osborne The nurses can offer help with minor ailments, routine and travel injections, immunisations, dressings, ear syringing, removing stitches, etc. They are very involved in the management of chronic diseases such as asthma and diabetes, health promotion and can provide general advice on a wide range of health concerns. Other Attached Staff There are a number of staff employed by various authorities who work very closely with staff at this practice. District Nurses Responsible for the nursing of patients at home, particularly older or chronically ill patients. Health Visitors They are based in Bosworth Medical Centre and they are interested in child development assessments, care of the under fives and their families and health education for all ages. Community Midwives They are involved in the antenatal clinics at the surgery and in the delivery of babies. Other Specialist Staff The doctors can also refer you to a range of other specialist staff such as a social worker, physiotherapist or nurse specialising in mental health if needed. Appointments Appointments may be made by telephoning or by calling into the surgery or may be booked online (pick up an online registration pack.) Appointments may be made up to four weeks in advance. If you cannot keep your appointment, please inform us as soon as possible as this will assist in oversubscribed situations. Telephone Appointments Every day there is one doctor who is dedicated to dealing with telephone triage appointments. This means that your health concern can be discussed over the telephone with a doctor rather than attending the surgery. If, however, the doctor feels that you need to be seen, they will arrange an appointment in the surgery. To arrange to speak to a doctor please contact the practice. The receptionist will take your details and telephone number. It will be passed to the clinician concerned who will return your call. Minor Ailments Scheme Certain minor ailments do not require an appointment with a GP or nurse and be treated by the local pharmacist free of charge if you do not pay for your prescriptions. These minor ailments include: athlete s foot, cold sores, cold, flu, earache, constipation, cough, diarrhea, hay fever, allergies, head lice, nappy rash, nasal congestion, sore throat, temperature, threadworms, vaginal thrush, veruccas and warts. Pick up a referral leaflet at the reception desk. Home Visits Patients are requested to telephone before 10.00am if a visit is required that day. Emergency visits only will be arranged after that time. Please give the receptionist as much information as possible in order to help the doctor plan the visits. The doctor who visits you may not always be the doctor of your choice. Please ask if you wish to see a particular doctor and we will try to arrange it. If you think an urgent visit is necessary please explain this to the receptionist. Please note, home visits will only be accepted for patients who are not able to leave the house. If you or the person you are telephoning about is able to leave the house, with or without assistance, please make a surgery appointment. Remember that it takes your doctor roughly four times as long to deal with your illness on a home visit than at the surgery.

Simply call Out Of Hours If an emergency occurs and you need a doctor urgently but the surgery is closed, please telephone There is always an On-Call doctor available. You will always be given a number to contact a doctor although it will not be your own GP. The surgery uses the BADGER service. Please only use this service in emergencies. Badger clinics are held at Heartlands Hospital or Whittall Street, Birmingham. Patients will be advised where to attend. An NHS walk-in centre is available outside the front of Solihull Hospital. The Birmingham and Solihull Cluster is responsible for commissioning out-of-hours services. Repeat Prescriptions These are only given with a doctor's authorisation. If you have had regular repeat prescriptions you will be given a re-order slip which you should use to request further prescriptions. To ask for a repeat prescription either hand in your re-order slip to your designated chemist or at the surgery or you can now also order it online (pick up a registration pack). Be sure not to run out. Allow at least two full working days if you hand it in to the surgery or five working days for postal requests. If you use a chemist, ask when it can be collected. prescriptions can be collected in person from the surgery or will be posted back if you provide a stamped addressed envelope. You may be asked to see a doctor for a check up every so often before you can obtain a further supply of your medication. Prescriptions out Of hours There is a rota of chemists open on Sundays or Bank Holidays. Details will be displayed at your local chemist shop. There is also a list of late opening chemists in the local evening newspaper. Responsibility Of Patients Please cancel appointments that are no longer needed. Please arrive on time for your appointment. Please treat our practice staff with courtesy and respect. Please notify us if you change your name, address or contact telephone number. Clinics Antenatal Clinic Monday Tuesday am am Smear Clinic Smears can be done by the nurses during normal surgery hours.

6 Child Health Surveillance Clinic In conjunction with community physicians and health visitors we offer checks at six to eight weeks of age and at 39 months. In order to offer this service we require that children under the age of five be registered for child health surveillance by ticking the box on the registration form.you will be notified when your child has an appointment for child health surveillance. Childhood immunisations are undertaken by the practice nurses. You will receive notification from the Cluster when immunisations are due; please telephone the practice to book a convenient appointment. Diabetic Clinic Monday, Tuesday and Thursday Respiratory Clinic Tuesday, Wednesday and Thursday Family Planning By appointment in surgery hours The practice nurse will provide pill checks and repeat prescriptions for those patients established on the Pill. The practice also provides a coil and contraceptive implant fitting service for suitable patients. Minor Surgery Various minor procedures can be performed at the surgery. Your doctor will advise you. Stop Smoking Clinic The practice provides support to stop smoking by appointment, through Solihull's Stop Smoking Service, available by telephoning Travel Vaccinations Please make an appointment with the nurse at least eight weeks in advance of your holiday to ensure adequate cover. A charge will be made for certain immunisations and vaccinations which are not covered by the NHS. The nurse will advise you of any charges. Flu Vaccinations An influenza vaccination is particularly recommended for patients with heart, lung or kidney disease, diabetes and residents of nursing and rest homes. Please contact the reception staff in October for details of the vaccination dates and make an appointment. If you are unable to attend the surgery, a home visit will be arranged to undertake this facility. Non-NHS Examinations The doctors are happy to carry out medicals, insurance and driving licence, by appointment. Please telephone the surgery for an appointment. Please ask at reception for the charges for these services. Telephone Advice If you need to ask a doctor s advice by phone please ask the receptionist. The doctor may be busy with another patient. Hospital Reports/Blood Results etc Please ring the surgery between 10.00am - 12 noon and pm to find out if the results are available. In the interests of confidentiality certain results may not be given over the phone and an appointment may be necessary. Access To The Surgery The surgery is situated on Dunster Road. There is limited parking in front of the surgery and a car park at the back of the surgery. Facilities For The Disabled Patient The main entrance to the surgery is approached by a ramp and access is gained via double doors. There are two disabled buttons available by the front doors to enable automatic opening. The double doors throughout the building are wide enough for wheelchairs. There is a low level reception counter. A purpose-built W.C. is available.

7 Surgery Facilities Blood tests, ECG, swabs, urine tests, etc. There is a delivery of specimens for analysis to the laboratory on a daily basis at noon. Any specimens for analysis should be at the surgery by noon. Your Medical Card Please keep this in a safe place. It gives your NHS number which you may be asked to give if you seek NHS treatment. If you have lost your medical card ask for a form at reception. Please help us: Change of name, address, or telephone number? Please notify the surgery as soon as possible of any such changes. If your name or address changes please ask for a form at reception. A new NHS medical card will follow shortly. Confidentiality All staff are bound by strict rules of confidentiality. How To Register With The Practice And New Patient Health Checks If you live within the practice boundary and would like to register with the practice, please collect a registration pack from reception. All newly registered patients are able to book for a simple health check with the health care assistant who will carry this out. This gives us the opportunity to meet you and your family and tell you about the practice. Patients have the right to express a preference of doctor on their records. However, the practice may not always be able to comply with your request. In this case, we will explain why. All patients may consult with any doctor working at the practice, who will have access to your medical records. Patients Access To Health Records Patients have the right to have copies of their medical records, although in exceptional circumstances the law allows for records to be withheld. The law concerning this is contained mainly in the Data Protection Act 1998 and the Access to Health Records Act For more information please either ask at reception for a leaflet or contact the practice manager. A charge for this service may be applicable. Your Medical Records Your medical records are confidential. However, to ensure the highest quality of care, information may be shared with other healthcare professionals. Birmingham and Solihull Cluster may require access to records for audit purposes. Anonymised patient data may be provided to third parties for audit and research. If you do not want your records to be shared please contact the practice manager. Summary Care Record Summary care records were introduced in this practice in February A summary care record is an electronic record containing key health information which can be made available to NHS healthcare staff or pharmacists caring for you in a emergency or when your GP practice is closed. If you do not want a summary care record, please complete an opt-out form which is available at the reception desk. Violence And Aggression The practice operates a zero-tolerance policy towards any form of verbal or physical aggression. Breaches of this policy will result in instant removal from the practice list. Teaching We have an active commitment to medical education. Medical students are attached to us as part of their undergraduate training and postgraduate training. All students have access to medical records but are bound by confidentiality in the same way as all other staff. Patients are under no obligation to have students present during their consultations. Your consent should always be sought beforehand. If you choose not to participate you can be assured that your wishes will be respected and that your decision will not be held against you in any way. Comments, Suggestions And Complaints We aim to provide the best services possible. However, there may be times where you feel this has not happened. We operate an in-house complaints procedure for such times and hope that you will use this procedure to allow us to look into and, if necessary, correct any problems that you have identified, or mistakes that have been made. If you wish to make a complaint, please collect a patient information leaflet and complete a complaints form (both available at reception) or write, ([email protected]) or telephone ( ) Ms V Parkes, practice manager. We believe it is important for complaints to be dealt with swiftly and would hope to be able to offer a full explanation within a short time. However, this is not always possible but we will keep you informed throughout. When our investigations are complete you will be notified of our findings. Our practice procedure is unable to deal with questions of legal liability or compensation. Should you wish to use this procedure, it will not affect your right to complain to the Birmingham and Solihull Cluster. Please note that we have to respect our duty of confidentiality to patients and a patient s consent will be necessary should a complaint not be made by the patient in person. 8 Good Health Guide Your health is your responsibility as well as ours, so: Do not smoke. Exercise on a regular basis. Eat more fruit and vegetables, less fat, sugar and salt. Avoid fast and junk food and aim for your ideal weight. Drink in moderation - try to keep below 21 units of alcohol (men) per week and 14 units (women). One unit = half pint of beer, single measure of spirits or one glass of wine. Ensure you and your family are fully immunised. Try to attend at least every three years to have your weight, blood pressure and urine checked. Ladies: cervical smears are recommended every three years, between the ages of 25 and 50 and then every five years between the ages of 50 and 65. What To Do If Someone Dies If someone dies, call the doctor. There is no need to move the patient. It is a legal requirement for a doctor to confirm that someone has passed away. If the doctor has recently seen the patient a death certificate can normally be issued. However, in the case of an unexpected death, the doctor will need to notify the coroner. Freedom Of Information Publication Scheme The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the classes of information the practice intends to routinely make available. This scheme is available from reception.
