2 Welcome to Dr Mixer and Partners. Our doctors and staff aim to offer you personal care and attention. This leaflet has been designed to help you make the best use of the services that we provide. HOW TO REGISTER WITH THE PRACTICE Please bring your medical card to the surgery, if possible. You will not be registered with a particular GP, as the GP contract with the NHS East Riding of Yorkshire Clinical Commissioning Group (CCG), means that all patients are registered with the Practice and not an individual GP. Patients can see whichever GP they wish, however, each GP would like to follow a patient s progress through a particular illness. NEW PATIENT CHECKS You will be asked to complete a health questionnaire and will be contacted for any further tests by our staff if they are judged to be clinically necessary, such as blood pressure checks or urine tests. HOW TO SEE THE DOCTOR On the day appointments If you require an appointment with a doctor, most appointments will be available by request on the day that you contact us. We request that you endeavour to telephone the surgery (01482) for an appointment between 8.30 am and am on the day that you wish to be seen. We can not guarantee an appointment with a particular GP, however we shall endeavour to do this, if it provides continuity of care for an existing condition. Please note that we may ask whether you feel the appointment is urgent, as we need to take into consideration the number of available appointment slots on that day. We shall look to book non urgent appointments on days when appointment slots are more available. Advanced Booking A small proportion of appointments will be available for Advanced Booking two weeks ahead for those patients who would prefer to book a specific date and time. If patients wish to book in advance for an appointment they will be asked to telephone the surgery for an appointment within two weeks of the required appointment date. Saturday Mornings There will be a GP running a Saturday morning surgery, from 8.30 a.m p.m. which primarily are for advanced bookings only. BRANCH SURGERY A branch surgery is operated at Samman Road (01482) for the convenience of those patients who live in that locality. Our reception staff will be pleased to provide you with details. GP surgeries are held there up to four days per week, excluding Thursdays. There is no appointment system at the branch surgery. Patients are seen in the order that they arrive.

3 TELEPHONE SYSTEM The telephone system experiences a high demand between 8.30 am and 9.30 am from patients booking appointments for that day and visits. It would be appreciated that you do not ring the surgery between these times for other matters, such as results or arranging routine appointments. URGENT APPOINTMENTS Urgent appointments can be obtained by contacting the reception staff during surgery times. Patients requesting urgent appointments will be seen by the first available doctor on the day of request. The doctors will endeavour to see urgent cases as soon as possible. HOME VISITS Please try to come to the surgery whenever possible, as facilities here are far better for examinations and treatment. If you feel a home visit is more appropriate please telephone your request before am if possible. OUT OF HOURS EMERGENCIES AND WEEKENDS At evenings and weekends when the surgery is closed please ring NHS 111 who will direct you to the most appropriate service. If you merely need advice, you can contact NHS Direct on , both during surgery time and evenings/weekends. You can also obtain advice via NHS Directs internet site Out of hours cover is the responsibility of the NHS East Riding of Yorkshire Clinical Commissioning Group (CCG) based at Health House, Grange Park Lane Willerby, HU10 6DT. PRIVATE INSURANCE If your GP is going to refer you to a consultant and you have private health insurance cover, you may wish to make the GP aware of this and take advantage of a possible shorter waiting time. ADVICE FROM DOCTOR TELEPHONE CONSULTATIONS Patients will be offered the opportunity to speak to a doctor on the telephone, if appropriate and more convenient, rather than attending the surgery for a consultation. Please telephone the main surgery number (01482) The GP will ring you back during their surgery. The receptionist will give you an approximate time when this will happen. These slots can be used for advice on a condition you have had before, or to discuss a test result or advice on a new condition. It may save you coming to the surgery and will free up a face to face appointment. If the GP feels you need an appointment to see them, after the telephone consultation, this will be arranged.

4 PRACTICE OPENING TIMES The Practice is open between 8.30 am and 6.00 pm, Monday-Friday. It is also open on Saturday mornings 8.30 a.m p.m. for advanced bookings only. Outside these times, the surgery telephone answering machine will give you a message to call NHS 111. The Practice is closed between pm and 2.00 pm on Wednesday lunchtimes for Practice training. GENERAL PRACTITIONER TRAINING SCHEME We often have doctors in the scheme attached to our practice. These Registrars are fully qualified doctors who have come to us to gain experience of medicine in a general practice situation and are under the direct supervision of our GP Partners. As a training exercise, consultations with the registrar are occasionally recorded on video for later discussion between the registrar and one of the partners in the practice. You will always be pre-warned if this is occurring and be given the opportunity to decline if you wish. CHANGE OF ADDRESS Please inform us of any change of address, name or telephone number to help us maintain accurate records. If you move out of the practice area you will need to register with a local doctor in that area. REPEAT PRESCRIPTIONS 24 HOUR ACCESS PRESCRIPTION ORDERING If you telephone the surgery for a repeat prescription please dial the dedicated prescription telephone line number (01482) Alternatively you may also your request to [email protected]. You may also post or hand in your written request or re-order slip. A telephone answering service is available for patients who wish to place their repeat prescription orders outside of office hours. The answering service is also available during busy periods throughout the day. Unfortunately we are unable to deal with prescription requests via the main surgery number and you will be asked to redial the prescription line. We aim to process all prescription requests within two working days, ready for collection after 2.00 pm. alternatively, if a stamped addressed envelope is provided we will post the prescription to you. All the information on the computer system used by the practice to issue prescriptions as well as speed up administrative tasks and store patient details is governed by the strict laws of the Data Protection Act and the long standing traditions of clinical confidentiality.

5 PRACTICE NURSES AND TREATMENT ROOM The practice employs four part-time nurses, a health care assistant and two phlebotomists who are available by appointment Monday to Friday. The treatment room has its own appointment system and the receptionists will be happy to arrange your appointment. The following are some of the services available in the treatment room: Removal of sutures, dressing of wounds, immunisations, blood pressure checks, ear syringing, blood tests, ECG and other clinical tests (after seeing the doctor), diet and weight reduction advice, cervical cytology (smears) and advice re breast awareness and routine injections. On most occasions it advisable for patients with minor injuries to attend the Minor Injury Unit at the East Riding Community Hospital which is open Monday Friday (Closed Bank Holidays) 9.00 a.m p.m. Telephone: TREATMENT ROOM TIMETABLE Monday-Friday Mornings, am apart from Thursdays, am Monday-Friday Afternoons, pm apart from Fridays, pm

6 OTHER SERVICES AVAILABLE East Riding Community Hospital Facilities available at the East Riding Community Hospital on Swinemoor Lane, Beverley, include a Minor Injuries Unit open Monday to Friday, 9.00 am to 5.00 pm. The hospital has beds for intermediate care/rehabilitation and palliative care. MRI, mobile CT, ultrasound and other out-patient services are also available at the hospital. CONSULTANT CLINICS A monthly orthopaedic consultant clinic, carried out by Mr. Kevin Sherman, is held at the Old Fire Station surgery. Mr. Stuart Smith, ENT consultant holds a monthly clinic at the surgery. There is also a monthly Dermatology clinic, run by Shernaz Walton, a consultant, and Dr. Carruthers, a Partner in the practice with a special interest in Dermatology together with a specialist Dermatology Nurse employed by the practice. PHOTOTHERAPY As part of our Dermatology Service we provide Phototherapy treatment in a clinic run by our specialist Dermatology Nurse. Referral for Phototherapy treatment is usually via one of our GPs if you are a patient of Dr. Mixer and Partners, or from some other GP practices who refer to us from within the East Riding of Yorkshire CCG. This service is provided on Mondays, Wednesdays and Fridays from 8.30 a.m. until 2.30 p.m. Once you have been referred for treatment by a GP, your appointments will be arranged directly with the specialist Dermatology Nurse. CRYOTHERAPY TREATMENT A wart clinic is usually held weekly. Appointments are available by contacting the reception. DISTRICT NURSING District nursing services are available for those patients unable to attend the surgery, arranged via referral from the doctor. District nurse enquiries telephone (01482) MATERNITY SERVICES Individual doctor s ante-natal clinics are held at the surgery throughout the week in association with the practice s attached midwives.

7 DISABLED PATIENTS Wheelchairs can readily be accommodated in the surgery and there is provision for the disabled. If you are a carer and would like to have support and help from social services, please let your doctor know. Both you and the person you care for need to give further approval for this. NON NHS EXAMINATIONS/PRIVATE MEDICAL SERVICES Medical examinations for special purposes e.g. elderly drivers, pre-employment, insurance medicals can be arranged by appointment. Some services fall outside of the NHS and for these a charge may be made. This includes BUPA/PPP forms, private medical certificates, signing passport applications and various special medical examinations or forms for sports, school, insurance or employment in accordance with the current British Medical Association recommended fees. ENQUIRIES, COMPLAINTS AND COMMENTS If you wish to make enquires about administrative or non-medical aspects of your health or treatment, or if you have any comments to make concerning the practice and its services, please contact Mr. Timothy Johns, Practice Manager, who will be available to assist with your enquiry. If patients are dissatisfied with or wish to comment about the performance of areas outside the Practice, they can contact the Patient Relations Service at the East Riding of Yorkshire Clinical Commissioning Group on or [email protected]. Please note that the local Trusts have their own complaints department and patients can access them direct by ringing at Hull Royal Infirmary, for Castle Hill for the East Riding Community Hospital, Beverley, PATIENT INFORMATION Patient information is held in a confidential manner and is available to GP, Nurses and staff during their work activities. Certain information will be shared with hospital staff, if relevant to any investigations/appointment. Patients have a right to access the information held in the Practice records. There may be a charge for this of 10, plus extra photocopying charge, if applicable. Information given to third parties or accessed by third parties will require patient signed consent e.g. insurance companies/solicitors. 9 Statutory Instrument 2004 No.291 The National Health Service (General Medical Services Contracts) Regulations 2004 SCHEDULE 6 Patient preference of practitioner 18. (1) Where the contractor has accepted an application for inclusion in its list of patients, it shall - (a) notify the patient (or, in the case of a child or incapable adult, the person who made the application on their behalf) of the patient s right to express a preference to receive services from a particular performer or class of performer either generally or in relation to any particular condition; and (b) record in writing any such preference expressed by or on behalf of the patient. (2) The contractor shall endeavour to comply with any reasonable preference expressed under sub-paragraph (1) but need not do so if the preferred performer - (a) has reasonable grounds for refusing to provide services to the patient; or (b) does not routinely perform the service in question within the practice. Patients not seen within three years 5. Where a registered patient who - (a) has attained the age of 16 years but has not attained the age of 75 years; and (b) has attended neither a consultation with, nor a clinic provided by, the contractor within the three years prior to the date of his request, requests a consultation the contractor shall, in addition and without prejudice to its other obligations in respect of that patient under the contract, provide such a consultation in the course of which it shall make such inquiries and undertake such examinations as appear to it to be appropriate in all the circumstances. Patients aged 75 years and over 6. (1) Where a registered patient who - (a) has attained the age of 75 years; and (b) has not participated in a consultation under this paragraph within the period of twelve months prior to the date of his request, requests a consultation, the contractor shall, in addition and without prejudice to its other obligations in respect of that patient under the contract, provide such a consultation in the, course of which it shall make such inquiries and undertake such examinations as appear to it to be appropriate in all the circumstances. (2) A consultation under sub-paragraph (1) shall take place in the home of the patient where, in the reasonable opinion of the contractor, it would be inappropriate, as a result of the patient s medical condition, for him to attend at the practice premises.

10 Removals from the list of patients who are violent PLEASE NOTE THAT THE PRACTICE HAS A ZERO TOLERANCE POLICY 21. (1) A contractor which wishes a patient to be removed from its list of patients with immediate effect on the grounds that - (a) the patient has committed an act of violence against any of the persons specified in subparagraph (2) or behaved in such a way that any such person has feared for his safety; and (b) it has reported the incident to the police, shall notify the Primary Care Trust in accordance with sub-paragraph (3). (2) The persons referred to in sub-paragraph (1) are - (a) the contractor where it is an individual medical practitioner; (b) in the case of a contract with two or more individuals practising in partnership, a partner in that partnership; (c) in the case of a contract with a company, a legal and beneficial owner of shares in that company; (d) a member of the contractor s staff; (e) a person engaged by the contractor to perform or assist in the performance of services under the contract; or (f) any other person present - (i) on the practice premises, or (ii) in the place where services were provided to the patient under the contract.
