They are responsible for a wide range of health matters, including blood tests, routine child immunisation, travel advice and immunisation, removal of stitches, dressings and ear syringing. They promote good health through dietary guidance, blood pressure checks and advice on smoking and alcohol. The nurses also conduct Chronic Disease Clinics including asthma, coronary heart disease, COPD and diabetic clinics and are fully trained in family planning and HRT and cervical cytology. Reception Team, Administrators and Secretaries Our receptionists are here to help you and are your link with the rest of the health care team. If you have any enquiries, our reception staff will be happy to help you, but please remember that they have a difficult job to do with telephone calls and queries coming from every direction. They have a lot of information at their fingertips and aim to keep the practice running as smoothly as possible. To do this, they may sometimes need to ask for details of your illness, but anything you tell them will be treated in the strictest confidence, as they are bound by the same rules of confidentiality as the doctors. The secretaries deal with all referral letters, insurance medical paperwork, forms from other agencies and general enquiries along with a lot more. The administrators process all registration papers and keep your medical records accurate; they also carry out the repeat prescription service that we offer to our patients. District Nurses Our district nurses are available for nursing assessment and care of patients who require to be seen at home. They are not based at Southview surgery and cover more than one surgery in Woking but collect messages from Southview daily and can be contacted by the reception team when needed. Health Visitors Our health visitors conduct child health surveillance clinics at Westfield and communicate regularly with the doctors. They run drop-in baby and child clinics throughout Woking. They can be contacted at Woking Community Hospital on and ask for health visitors office. Midwife Our community midwife shares your antenatal care with your doctor. The midwife carries out antenatal clinics at both Southview and Westfield surgeries. They are based at St. Peter s Hospital, Chertsey. You can also leave messages for the midwife at the surgery.

Laney was taught how to treat animals by a local farmer, who was blind. That was my first experience of physiotherapy, and it s been my passion ever since, she said. Laney qualified in 1973, and has helped people with problems and complaints ranging from the aches and pains of everyday life to chronic and stress-related illnesses, as well as post-operative rehabilitation. I treat individuals of all ages, from children to the elderly, by taking a holistic approach, said Laney. Instead of taking a bad back as just that, I look at the problem to find the root cause. This can be too much time spent in the gym, or for many young athletes, overuse of their body. Playing too much sport while still growing can cause a whole range of problems, added Laney. I also treat MS patients and stroke victims to help them maintain as much independence as possible. Laney works from her own comfortable treatment room. To arrange a consultation, call (01483) ADVERTISING FEATURE Practice Information Appointments All consultations are made by appointment. We do not run drop-in surgeries. Appointments can be made: Via reception On-line - for routine doctor appointments and some nurse blood test appointments only, log onto Whenever possible, contact the receptionist well in advance to ensure seeing your doctor at a time convenient to you. We have a selection of appointments available to our patients: Routine Bookable Appointments Up To Eight Weeks In Advance - these can be booked either via reception or by using our online appointment system. Emergency Appointments, Requests For Urgent Same Day Appointments - these will be triaged by the duty doctor every morning and an appointment given if the doctor feels this is necessary, the appointment may not be with your chosen doctor and urgent appointments are allocated in time order and could be at Southview or Westfield Surgery. Telephone Appointments - If you believe that your problem might be resolved by speaking to the doctor, rather than being seen by them, you can make a telephone appointment and the doctor will call you back during the morning or afternoon to discuss your case. These can be booked in advance with a chosen doctor. The doctor will return your call, usually towards the end of their surgery. Access for the Disabled Wheelchair access is available at Westfield and consulting rooms are available on the ground floor at all times. Please inform the receptionist if you are disabled and/or unable to climb stairs, so that your appointment can be arranged at Westfield Surgery. The nurses treatment room and one doctor s consultation room (usually Dr Benham, Dr Baker or Dr Akbar) at Southview Surgery are upstairs and we do not have a lift. If you are unable to climb the stairs please try and make your appointment at Westfield. To advertise your business to our patients on low cost, easy payment terms call

4 Home Visits Home visits are for patients who are too unwell to come to the surgery. Please consider very carefully whether a request for a visit is necessary and telephone before 10.30am. The duty doctor will return your call to assess whether a visit is needed and the level of urgency. Lack of transport is not an indication for a home visit. Home visits on demand officially ceased with a change in the law in February 1995 (Statutory instrument No 80). The government and the doctors' leaders decided that it was time to adopt a higher and safer standard of medical care, by giving all doctors the discretion to see virtually all patients in their consulting rooms, with a full range of diagnostic equipment, computer, medical records and helpful letters, treatment facilities and a chaperone to hand. Extended Hours We now provide pre-bookable routine appointments outside the general surgery opening hours. Wednesday morning am at Southview. Wednesday evening pm at Westfield. Please note that during these times the surgery will not be open for general business such as collecting prescriptions and booking appointments. The telephone lines will be closed and if you have an emergency, please dial 111. Non-Attendance to Appointments Tell the surgery immediately if you cannot keep your appointment. We have a cancellation line on which you can leave a message. These messages are checked and dealt with regularly during the morning or afternoon. Many appointments each week are wasted due to non-attendance. Repeated non-attendance will result in you being de-registered from the practice. Additional Services Available at the surgery by appointment: Antenatal Care (Midwife and Doctor) Asthma Clinic Baby Clinic (including Child Health Surveillance) Chronic Heart Disease Clinic Diabetic Clinic Dietary Advice Family Planning including Mirena and Cooper Coils and Contraceptive Implants Immunisations Psychological Counselling (following GP Referral) Smoking Cessation Travel Clinic Cryotherapy Clinic Well Woman Clinic Yellow Fever Centre New Patient Health Check We strongly encourage all new patients to have a medical check, which includes weight, height and blood pressure and allows us to summarise your medical history, treatment and prescriptions while waiting for your notes from your previous doctor (this can take many weeks). Please also bring a sample of your urine for testing in a small clean bottle (preferably a specimen bottle that is obtainable from reception). Advice and Guidance The surgery website contains general medical advice and has links to other areas of interest as well as information about the practice. This is where to register for on-line appointment booking and repeat prescriptions. This is a website used by the practice and many other GPs to provide medical advice and information leaflets. It contains accurate medical advice about almost all medical conditions and can be accessed by the public. You can see your pharmacist for advice as an alternative to making an appointment with your GP for managing many minor conditions. Many medications are available without prescription including antibiotic eye drops for conjunctivitis, almost all hayfever medications and emergency contraception. For medical advice telephone 111. This is an NHS telephone helpline, which is open 24 hours a day. The medically qualified staff are able to give advice about any illness or general healthcare and can advise you whether or not it is necessary for you to see your doctor about any particular concern which you may have. Emergency Calls - Out Of Hours When the surgery is closed please consider carefully whether you can wait until it re-opens as it is best to see your own GP who has full access to your medical records. Out-of-hours medical care is only for emergencies that cannot wait until the surgery re-opens. It is not able to provide repeat prescriptions, sick notes or second opinions. For a life-threatening emergency call 999 for the emergency services and ask for an ambulance. For medical advice telephone 111. Walk-in Centre at Woking Community Hospital at Heathside Road, Woking, Surrey. The centre is open from 7.00am pm Monday to Friday and 9.00am pm at weekends. The experienced NHS nurses are able to treat minor injuries and illnesses, but not children under two years old, nor in cases where you have seen a doctor recently about the same problem.

5 Repeat Prescriptions This is only for medication that you take on a regular basis and the doctor has added to your repeat prescription list. To order a repeat prescription Register to do it on-line Faxing request, ideally white repeat list, to Use the right hand side of your prescription which contains a list of your repeat medication, tick the medication you require and send/bring the list into either surgery. Some local pharmacies will order medication on your behalf but ensure they know which items you need It takes 48 hours to process a repeat prescription and they can be collected after 2.00pm. Prescriptions can be collected: From either surgery - as long as you clearly mark on your request which surgery you wish to collect it from. By a pharmacy chosen by you, please mark this on your request and ensure the pharmacy is aware. Sent to you if you enclose a stamped, addressed envelope. The doctors often attach messages to repeat prescriptions regarding when you are due to have a review of your medication, require a blood test, flu vaccines etc. Please act on these messages otherwise your prescription will expire. If your prescription is collected by a pharmacy please check they pass on any notes to you. Non-NHS Private Fees In addition to the usual items which fall under NHS care, the doctors provide other services for which a charge is made. These include passport applications, driving medicals, solicitor s reports, fitness for occupation, travel or sporting activities, school fees insurance, private health insurance claim forms etc. Please enquire at the reception desk regarding the appropriate fee. Medical Certificates Statutory sickness certificates are not valid until you have been off work for five working days. You do not need any certificate unless you have been off work for more than three days. Following this you need to supply your employer with a self-certificate that can be accessed at or via your employer to cover you for the first five working days. Please do not make an appointment for a sick note unless you have been off for more than five days. If your employer insists on a doctor's certificate, you will be required to pay a fee for a private certificate. A receipt can be supplied to pass on to your employer. In order to obtain any medical certificate you must have been seen by the GP while you were unwell or have a letter in your records from the hospital about your condition. Results of X-rays and Tests It is the responsibility of the patient to obtain all test results - both normal and abnormal. Please telephone Westfield Surgery for test results between pm Monday to Friday. The receptionist will be able to tell you the remark the GP has attached to your result. Blood results are usually back within a week but x-rays and scans can take longer. We do not usually receive results of tests ordered by a hospital consultant. Change of Address and Telephone Numbers Please let us know your new details as soon as possible. It would also be extremely helpful if you informed any hospitals under whose care you may be. Freedom of Information Act We conform to the requirements of the Freedom of Information Act 2000, and have produced a publication scheme in accordance with the act, a copy of which is available from the practice manager. Suggestions and Complaints If you have any suggestions to make regarding the services we provide, please speak to one of the receptionists, the senior receptionist or the practice manager. Complaints regarding the running of the surgery should be addressed to the practice manager. Southview Patient Participation Group We have a virtual patient participation group which takes part in surveys and gives suggestions to the practice via . More information is on the surgery website, if you wish to join the group either ask reception for a form or contact the administrator or practice manager. We aim to achieve this by working together with you to help you to attain the best possible health. Help Us to Help You Please... Be courteous to practice staff at all times. Remember you are the person responsible for the health of your family and yourself. We are here to help and advise. Make a separate appointment for each person who needs to be seen. Consecutive appointments can be arranged. Tell the surgery immediately if you cannot keep your appointment. Try to telephone at 8.30am in the morning surgery, if you require telephone advice.

6 For test and x-ray results please telephone Westfield Surgery between 2.00 and 5.00pm. Let the surgery know if you change your name, address or telephone number. Order repeat prescriptions well before you run out of the medication. Remember the doctor only has 10 minutes for a routine appointment. Therefore, try to be realistic about what can be achieved in this time. A double appointment can be booked if necessary Thank you Zero Tolerance We strongly support the NHS policy on zero tolerance. Anyone attending the surgery who abuses the GPs, staff or other patients be it verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises. NHS Summary Care Record - Your Emergency Care Summary Today, records are kept in all the places where you receive care. These places can usually only share information from your records by letter, , fax or phone. At times this can slow down treatment and sometimes information can be hard to access. The introduction of the Summary Care Record is to improve the safety and quality of patient care. Because the Summary Care Record is an electronic record it will give healthcare staff faster, easier access to essential information about you, to help provide you with save treatment when you need care in an emergency or when your GP surgery is closed. You can choose to have a Summary Care Record: You do not need to do anything. This will happen automatically. Healthcare staff will ask your permission every time they look at your Summary Care Record. You can choose not to have a Summary Care Record: If you don t want a Summary Care Record, you need to let your GP surgery know by filling and returning an opt-out form. Opt-forms are available at or you can telephone and ask them to send you one. Discrimination The practice does not discriminate against any patient, visitor or staff member for reason of age, sex, marital status, pregnancy, race, ethnicity, disability, sexual orientation, religion or belief. Patient Confidentiality and Data Protection We ask you for personal information so that you can receive care and treatment. This information is recorded on the computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the practice team. Self Treatment of Common Illness General advice can be obtained: via the surgery website www. patient.co.uk NHS Direct If you are concerned at all book a telephone appointment for advice Colds, Coughs And Stuffy Noses Colds, sore throats and flu-like illnesses are due to viruses and are not affected by antibiotics or other prescribed treatments. You can manage them at home by the following means: Stay at home if possible. You are not doing anyone a favour by going to work as you will simply spread the disease among your colleagues and you may delay your recovery. Keep warm and rested. It is natural to feel tired because your body is diverting its energies into defeating the virus. Give your body the rest it is asking for. Drink plenty of watery liquids and don't worry if your appetite for solid food is reduced. Viruses spread by hand contact. You, and any person close to you, should wash your hands frequently. Coughs and sneezes spread diseases. Catch them in your handkerchief. You can certify your own absence from work for the first five working days. If you are concerned at all book a telephone appointment for advice Vomiting and Diarrhoea Prevent dehydration. Drink plenty of fluids. If vomiting, take small sips of water frequently at first and nothing to eat until the vomiting has stopped for several hours. For young children and the elderly, oral rehydration sachets are available at your chemist and are recommended for the first 24 hours. If vomiting persists beyond this, consult the on-call doctor for advice or at any time if you are particularly worried. Meningitis This is not common. Immunisation is routinely advised for children under four to protect against one of the three types of bacteria which cause this disease. The possible signs of the illness are: high temperature, vomiting, drowsiness, neck stiffness, aversion to bright light, unusual crying and a rash (the small red areas do not change colour when pressed with a clear glass) if in doubt phone for advice. Stomachache Most attacks are not serious and are usually caused by indigestion or wind. A hot water bottle will often relieve the symptoms and, in the case of indigestion, a teaspoon of bicarbonate of soda in half a glass of water will help. If the pain lasts for longer than eight hours or increases in intensity you should consult your doctor.
