1 REQUEST FOR PROPOSALS Unit Make Ready Services for Public Housing for HOUSING AUTHORITY OF THE CITY OF MCKINNEY, TEXAS Date: 2/17/2015 (MHA) requests proposal for Unit Make Ready Services. All interested bidders are required to attend a tour of specific units with MHA representative to ensure uniformity of the scope of work needed. If any changes are made in the scope of work as a result of the tour, changes will be made immediately. A Contract amendment with the revised scope will be submitted to all bidders within twenty four hours of the tour. All bidders must submit verification of coverage for $1,000,000 General Liability and Workmen s Compensation coverage with MHA designated as loss payee for said project prior to the award of contract. All contractors must be in compliance with Davis Bacon Labor Standards. Contractors will furnish a minimum 3 past business references. (MHA will clean or install appliances as needed.) All materials and labor will be the responsibility of the contractor. Contractor will be responsible for securing any and all required permits. All sites will be maintained in a business like order. All areas outside the work area will be kept free of debris or trash. Work area will be maintained in a presentable, safe and secure manner. MHA will designate a vacant unit for storage of materials. Only the awarded contractor will have keys to the unit during the time you are working at our site. The specified contract units and the storage unit will all be cleared of trash and debris by the contractor prior to final payment.

2 Any work required outside the original scope of work must be clearly defined, and submitted in writing by MHA representative prior to changes being made. Any change orders submitted and approved will be an addendum to the original contract and payments will be made accordingly. Any change orders not approved and signed by MHA representative will be at the contractor s expense. Proposals must include a detailed listing of items covered with the proposal. Line item pricing is not required, but is certainly optional if contractor desires. Contractor will be responsible for the installation of new mini-blinds, smoke detectors and air conditioning grills furnished by MHA. All interior window surfaces will be cleaned prior to installation of mini-blinds. MHA will be responsible for changing of deadbolts and/or entry keys Site tour date is 2/23/2015 at 11:00 am. Location is 603 North Tennessee, McKinney, Texas Date for submittal is close of business or 5:00 pm 3/2/2015. All bids submitted after that date will be declined. Any questions should be directed to Ed Kirkpatrick, Maintenance Supervisor, extension 111 or Cell Definitions: Painting shall always include proper preparation and clean up. Preparation shall include removal of electrical cover plates and masking off switches and receptacles. Remove any nails left in place and fill holes. All appliances, cabinets, bathroom fixtures, ceiling fans and/or lights will need to be covered prior to painting. Paint will cover both sides of all entry doors, interior doors, walls, ceilings, shelves, window sills when applicable and baseboards. If texture is needed it should match existing as nearly as possible. Paint color is Home Depot Moonlight Semi-gloss latex enamel. All surfaces to be painted must be properly cleaned prior to painting. If painted surfaces are not properly cleaned prior to painting it will be the prerogative of MHA to require cleaning and repaint at no additional charge to MHA. When staining of floors is required, the floor must first be swept clean, treated with de-glosser and then stained. If there are light spots where the old stain has peeled off, put a coat of stain on those areas first. When the initial coat of stain is dry, apply a final coat of stain to the entire floor to cover any irregularities in color. MHA will furnish the stain and contractor is responsible for the de-glosser. MHA will furnish and install any smoke detectors and light bulbs.

3 Scope of work: RFP Units: Units 2, 28 and 110. Unit: 2 Paint entire unit walls and ceilings. Prepare floors with De-glosser and stain with MHA furnished stain. Fill nail holes and repair drywall with tape, bed and texture prior to painting. Replace two 35x64 blinds Alabaster in color. Paint cabinets inside and outside. Replace cabinet door prior to painting. Replace two 35x64 blinds Alabaster in color. Repair small hole in drywall with tape, bed and texture prior to painting. Secure drywall on ceiling with screws where it has dropped and tape, bed and texture prior to paint. Paint walls and ceilings. Paint walls and ceilings. Repair two small holes in drywall with tape, bed and texture prior to painting. Paint cabinet and caulk bathtub. Repair drywall behind toilet with tape, bed and texture prior to painting. Tub will be glazed by others. Bedroom 1: Replace knob at entry door. Repair drywall by A/C vent. Replace one 35x64 blind Alabaster in color. Bedroom 2: Replace entry door with hardware. Replace one 35x64 blind Alabaster in color. Pull nails and fill holes prior to any texture work and painting. Bedroom 3: Repair drywall over window with tape, bed and texture prior to painting. Repair small hole in drywall with tape, bed and texture prior to painting. Replace electrical outlet cover. Replace door knob. Unit: 28: MHA will change hardware at storm door. Paint entire unit walls and ceilings. De-gloss and stain floors. Replace A/C grill. Replace closet rod. Repair drywall with tape, bed and texture prior to painting. Repair drywall around flue. Remove any nails left in place and fill small holes left by staples and/or nails. Replace five doors on cabinets prior to painting. Ensure that hinges match as nearly as possible. Paint cabinets inside and outside Leather Brown in color. Replace floor tile with needed preparation prior to installation. Floor tile is to be 1/8 commercial VCT Armstrong Replace kitchen light fixture with a fixture that is similar. Replace water heater supplied

4 by MHA. Repair drywall around flue. Remove water damaged ceiling and replace with new drywall, tape, bed, and texture prior to painting. Repair water heater closet door. Repair small holes in drywall with tape, bed and texture prior to painting. Tub to be glazed by others. Replace door and lockset. Remove ceramic and drywall at tub surround and replace drywall with mildew resistant drywall. Install new 12x12 ceramic tile and seal grout. Replace vanity with MHA furnished cabinet, sink and faucet. MHA will furnish new drain lines, water supply lines and angle stops if needed. Remove flooring, secure sub-floor and install new T&G OSB flooring. Replace floor tile with needed preparation prior to installation. Floor tile to be 1/8 commercial VCT Armstrong Remove and reinstall toilet with new wax ring. Install new angle stop and supply line MHA furnished if needed. Install transition metal at entry door. Rear Bedroom: Replace door and lockset. Replace closet door. Replace fan and light. Front Bedroom: Replace closet door. MHA will furnish globe or fan & light combination. Unit: 110 Handicap accessible unit Paint entire unit walls and ceilings. Replace two 35x64 blinds alabaster in color. Replace four floor tile. Mop and wax all floors. Texture spot on ceiling around fan. Replace cabinet door. Paint all cabinets inside and outside. Replace one 35x64 blind. Repair small hole in drywall with tape, bed and texture prior to painting. Replace four floor tile. Repair soft spot in floor in washer area. Tub to be glazed by others. Bedroom 1: Replace one 35x64 alabaster blind. Repair drywall at window with tape, bed and texture prior to painting.

5 Bedroom 2: Replace two 35x64 alabaster blinds. Replace lockset with lever handle and install door stop. Bedroom 3: Replace two 35x64 alabaster blinds. Repair hole in wall behind entry door. Replace lockset with lever handle. Section 3 Requirements for All Contractors and Vendors MHA is committed to providing quality housing and economic opportunities for our residents and the neighborhoods we serve. One of the major vehicles for this is Section 3 of the HUD Act of 1968, which requires that HUD funds provide low-income individuals with a springboard for economic empowerment through direct participation in construction and other activities that are designed to physically improve and revitalize the communities in which they live. HUD considers public housing authorities to be in compliance with Section 3 if they meet the following minimum numerical goals: A. 30% of the aggregate number of new hires shall be Section 3 residents; B. 10% of the total dollar amount of all covered construction contracts shall be awarded to Section 3 business concerns; and C. 3% of the total dollar amount of all covered non-construction contracts shall be awarded to Section 3 business concerns. Section 3 requirements apply to all contractors and vendors who receive awards from public housing authorities.
