1 Jessica A. Antezano, MPA WIC Supervisor, IBCLC Jessica Antezano grew up primarily in Utah. She attended BYU, where she earned a Bachelor's degree in Nutritional Science. She began working for the Salt Lake County WIC program in She currently works as a supervisor for the Sandy WIC clinic, and she is also the Vendor Coordinator for the WIC program in Salt Lake County. She has three young boys ages 6,4, and 5 months and has breastfeed each of them while working full time. He currently serves as an Assistant Professor in the Public and Community Health Department at Utah Valley University (UVU) and teaches the Environmental Health, Principles of Community Health, Research Methods, Personal Health & Wellness, and Foundations of Nutrition courses. James primary research interest is the topic of financial health. He strongly believes an individual s financial status impacts every other aspect of their health. Dr. Bemel believes this impact is so profound that Financial Health should be included as a new dimension of health. Dr. Bemel s research has been presented at conferences for the Society for Public Health Education (SOPHE), International Journal of Arts and Sciences (IJAS), International Union for Health Promotion and Education, American Public Health Association, Health Education Association of Utah (HEAU), and Utah Public Health Association. Additionally, James research is published in the IJAS. College Student Financial Health and the Impact on Other Dimensions of Health Jenni Bloomfield, BS Health Program Specialist Jenni Bloomfield is a Health Program Specialist for the Cancer Control Program at the Utah Department of Health, working primarily with the National Breast and Cervical Cancer Program. As a Health Program Specialist, Jenni is responsible for completing grant reports and working with other program employees to plan, implement, and evaluate programs to increase cancer screening rates in Utah. She serves on the Utah Cancer Action Network s Survivorship and Quality of Life Committee and on several of the Utah Mammography Action Committee s implementation teams. Jenni has a Bachelor of Science in Public Health with an emphasis in Health Promotion from Brigham Young University. In April of this year, Adam changed positions and now work for the Utah Department of Health's tobacco prevention and control program, overseeing the work of our new ad agency. Cascade D

2 Rebecca Castleton, BS, CHES Arthritis Program Manager Rebecca Castleton has been in the Utah Arthritis Program since November She administers arthritis selfmanagement interventions throughout the state through program planning, implementation, and evaluation. She is involved with public outreach and building partnerships with other agencies to increase awareness of and participation in arthritis-related and chronic disease self-management programs. Rebecca received a Bachelor of Science degree in Public Health from Brigham Young University and was CHES certified in She brings experience in infant mortality program services, nation-wide research on reduced tobacco use in Russia, and international micro-business and health education experience in El Salvador. She currently works as an Evaluation Intern at the Utah Department of Health in the Tobacco Prevention and Control Program, and as a Health Systems Research Assistant for the Doris Duke Foundation. Hobble Creek Kim Clausing, BS Tobacco Prevention Contract Coordinator Kim Clausing is the Tobacco Prevention Coordinator for Tooele County Health Department (TCHD). Kim has worked as a Health Educator for TCHD for 11 years. Kim was a 6th grade teacher prior to coming to the TCHD. She received a Bachelor of Science degree in Elementary Education from Utah State University in Kim has served as a Board Member for the Coalition of Tobacco Free Utah for almost nine years. She was a Past President, Vice President and Treasurer for the CTFU. As a member of the CTFU she worked on legislative committees to advocate for smoke free Commissioners. She has served as the Urban Coordinator for three years. Kim is an active member and co-founder of Tooele Interagency Prevention Professionals, a nationally recognized community coalition. She has served as Chair many times and helps coordinate a variety of community activities. Kim enjoys her work as a Health Educator. Kim also advocated for smoke free parks and all the parks in Tooele County, Tooele City and Grantsville City are now smoke free. Title Tobacco Specialty Store City Regulations Emily Cook :45 PM American Freestyle Skier, 3-Time Olympian Title Olympian's Perspective on Right To Play 12:15 PM Cascade D Emily Cook is an American freestyle skier and three-time Olympian competing for the US in 2006, 2010, and Emily is a six-time US National Champion with Nine-time World Cup Podiums including three wins. She is also a sixtime World Championship Team Member. Even though Emily s main focus is the Olympics, she also has a passion to give back to children and young athletes. During her injury, she created Visa Champions Creating Champions, a mentoring program during which Olympians from a variety of winter sports worked with youngsters in the community. Emily is a dedicated Right To Play Athlete Ambassador and has visited our programs in Tanzania (2013) and Jordan (2009). We are lucky she is on the Right To Play team! Cascade C, D, E

3 Stephanie Croasdell UPHA Student Caucus President Stephanie Croasdell, is the co-president for the Utah Public Health Association Student Caucus and is a recent graduate from the Division of Public Health at the University of Utah. After receiving a B.S. in psychology, she developed a passion for women s health through her travel experiences in Central America and counseling experiences within the field of perinatal loss. Her graduate academic emphasis was focused on reproductive health and public health workforce development. Jamie Pluta Title Student Caucus Speed Mentoring Ronnie Daniel, MS Executive Director Title 2:30 PM 10:30 AM Ronnie Daniel became the Executive Director of the Utah Chapter of the Alzheimer s Association on January 6, He brings over 23 years of nonprofit leadership experience to this position. He started his career working with the Boy Scouts of America in local council positions in California, Colorado and Indiana. He also worked as the development director for the Natural History Museum of Utah located at the University of Utah and most recently as the development director at the Living Planet Aquarium where he led the effort to generate funding to build the new aquarium facility in Utah. Ronnie has a Bachelor s Degree in Nonprofit Administration from Brigham Young University and a Master s Degree in organizational leadership from Argosy University. Using the Road Map and BRFSS Data to Address Cognitive Decline & Alzheimer s Disease in Utah Lora L. Dreibelbis, ACJ, BS LEHS Lora has been employed with Salt Lake County for 22 years, 18 of those years have been with the Salt Lake County Health Department. She began her Health Department career as support staff in the Bureau of Sanitation and Safety, meanwhile she continued with her education, earning a Bachelor s degree in Environmental Studies at the University of Utah. After a promotion to Environmental Health Scientist, she joined the Bureau of Sanitation and Safety s Zoonosis team. Lora receives regular requests to provide community education and outreach on bedbug abatement issues. She also partners with the State Health Department, local Animal Control Agencies, physicians, and victims regarding appropriate response to animal bites making recommendations regarding case management to mitigate the risk of rabies transmission. Lora is licensed by the State of Utah as an Environmental Health Scientist, and is presently assigned as the Environmental Health Inspector in South Salt Lake City, Holladay City, and the Millcreek Township; her normal duties include enforcement of housing and solid waste violations and conducting inspections of personal service facilities. 4 Gary L. Edwards, MS Executive Director Gary Edwards is the Executive Director of the Salt Lake County Health Department. He has worked in public health for 32 years; his career began as a public health educator for the Southwest Utah Public Health Department. Gary spent 11 years with the Utah Department of Health in the Bureau of Health Promotion, directing the Bureau for three years. He was the Director of the Southwest Utah Public Health Department for 12 years prior to his current position. During his career, Gary has been involved in investigating and managing cases and outbreaks of Rocky Mountain spotted fever, measles, plague, pertussis, cryptosporidiosis, novel H1N1 influenza, and other communicable diseases. He has been actively involved in efforts to prevent and control tobacco use in Utah, responsible for some of the initial work in the nation addressing smokeless tobacco. His work has also included investigating suspected anthrax exposure sites and evaluating the potential public health impacts of large hog operations. He has served on numerous state and national boards and committees. In addition to his work in public health, Gary has taught as an adjunct faculty member at the University of Utah, Southern Utah University, and Ball State University. Gary received his undergraduate degree in health education from Utah State University and a Master of Science degree in health science from Western Illinois University. He has completed advanced leadership training with the Regional Institute for Health and Environmental Leadership and the Homeland Security Executive Leadership Program at the Naval Postgraduate School. Title Feeling Comfortable and Confident in the Health Policy Arena :30 AM Hobble Creek David J. Erickson, PhD Manager, Center for Community Development Investments David J. Erickson is director of the Center for Community Development Investments at the Federal Reserve Bank of San Francisco and edits the Federal Reserve journal Community Development Investment Review. His research areas in the Community Development Department of the Federal Reserve include community development finance, affordable housing, economic development, and institutional changes that benefit low-income communities. Erickson has a Ph.D. in history from the University of California, Berkeley, with a focus on economic history and public policy. He also holds a master s degree in public policy from Berkeley and an undergraduate degree from Dartmouth College. He has also been a leader in the collaboration between the Federal Reserve and the Robert Wood Johnson Foundation in bringing the health sector together with community development. To date, this collaboration has resulted in 15 conferences around the country and numerous publications, including a cluster of articles in Health Affairs in November, His book on the history of community development, The Housing Policy Revolution: Networks and Neighborhoods, was published in 2009 by the Urban Institute Press. 5 Teresa Garrett, RN, DNP, MS, APHN-BC Assistant Vice President Teresa is the Assistant Vice President for Integrated Care Management with Intermountain Healthcare. Prior to this, Teresa worked for 17 years with the Utah Department of Health in a variety of leadership roles for 17 years. She serves as a Governing Councilor for the American Public Health Association and the Past President of the Association of State and Territorial Directors of Nursing & the Utah Public Health Association. Teresa is a Fellow of the Robert Wood Johnson Executive Nurse Fellows Program (2007 Cohort) and a Clinical Faculty member of the University of Utah, College of Nursing, teaching in the graduate and undergraduate programs. She recently completed her Doctor of Nursing Practice and earned the credential of a board certified advanced practice public health nurse (APHN-BC). Title Public Health Workers & Influenza Immunization: Barriers & Facilitators :45 PM John Gaspari, LCSW Executive Director John Gaspari received his Masters in Industrial Social Work from USC in 1988 and since that time, has made the workplace the sole focus of his professional practice. John has over 25 years of experience as an employee assistance professional, mental health practitioner, organizational and management consultant and coach, workplace behavioral health educator and program administrator. He currently serves as the Executive Director of the USC Center for Work and Family Life, the University s employee assistance, worklife and wellness program, which was recently honored by the American Psychological Association in its Psychologically Healthy Workplace competition. Celebrating its 34th anniversary at USC, this program serves all USC faculty, staff, and their dependent family members, as well as retired employees. In this role, John is the administrative and clinical lead for the Center, and carries a caseload of individual and management clients across the University. He also provides field instruction and clinical training for graduate students and post-graduate practitioners. Mr. Gaspari is a Licensed Clinical Social Worker, a Certified Employee Assistance Professional, and a Board Certified Diplomate in Clinical Social Work. He is a member of the senior leadership teams for both USC Division of Human Resources and the Graduate School of Social Work and serves on several standing committees at USC including the Provost s Advisory Committee for Work and Family Life, the Personnel Council, the Employee Relations Committee, the Employee Benefits Advisory Committee. John created and leads the USC Disaster Mental Health Response Team, a multidisciplinary group of mental health professionals, emergency response specialists, academics and clergy from across the University community and was named the winner of the 2013 President s Award for Staff Achievement. 6 Olga M. Gray, MPH Olga holds a Master of Public Health and is a graduate of the University of Utah. She hopes to be able to continue to work in the area of childhood illness and help to improve the health outcomes of children living in Africa. In her faculty role, she uses social and ecological models of health to guide students in building linkages between the health of populations and the public policies which directly, or indirectly, influence health. Dr. Hossain earned her Ph.D. from the University of Utah in Sociology. She has been on the faculty at the University of Utah and Westminster College as an Adjunct Professor. She teaches nutrition and public health courses at local and online universities. Early in her career she worked as a nutrition supervisor and clinical dietitian. She is a Past President of UPHA and current president of the Utah Nutrition Council. She has worked on humanitarian projects in Utah, Colombia and the Dominican Republic and has done nutrition research on all seven continents. She recently authored a book on healthy eating for consumers titled The Five Hungers: Stealth Appetites You Can Satisfy without Overeating. 7 Sapna Kaul, PhD Post-Doctoral Research Associate Sapna is a Post-Doctoral Research Associate at the Department of Pediatric and Huntsman Cancer Institute, University of Utah. My research interests include health economics, cancer research, environmental economics and applied econometrics. I am working on a range of projects with my Mentor, Dr. Anne Kirchhoff. I am currently examining several issues associated with the late effects of pediatric cancers. For instance, what is the cost that patients and their families incur in treating late effects of cancer and what values do patients place on follow-up care for reducing the risk of developing late effects. I am also working on a project that analyzes the determinants of medical wastage among primary care doctors in the U.S. Dr. Anne Kirchhoff Title Costs of Late Effects for Childhood Cancer Survivors Deanna L. Kepka, PhD, MPH Assistant Professor and Investigator Deanna Kepka, PhD, MPH, is a Huntsman Cancer Institute (HCI) investigator and an assistant professor in the College of Nursing at the University of Utah. She is a member of the Cancer Control and Population Sciences (CCPS) research group Kepka s main research interests are the gaps in health care access and quality for vulnerable populations. She focuses specifically on preventing cervical cancer among US Latinas, who have higher rates of cervical cancer than most other racial and ethnic groups. Utah has one of the lowest rates of completion of the HPV vaccination series in the country, and Kepka s work with CCPS and the College of Nursing intends to develop more effective patient-provider interventions that will help increase this rate. She is also dedicated to improving health care services for cancer prevention and control among minority and underserved populations. Dr. Kepka has worked with teams in at-risk communities, promoting positive health behaviors in Jamaica, Chicago, North Carolina, and Washington. Since 2007, she has worked with Hispanic and Latino communities in Mexico, Peru, Chicago, North Carolina, Washington, and Utah. She has also developed a radionovela (a Spanish short story broadcast) that successfully improved knowledge and sparked interest in the HPV vaccine among rural Latino parents in the United States. Kepka was most recently a posdoctoral fellow in the Cancer Prevention Fellowship Program at the National Cancer Institute in Bethesda, Maryland, where she received the Cancer Prevention Fellowship Merit Award. 8 Karmella Koopmeiners, RN, MS, FNP Clinical Nurse Specialist Karmella Koopmeiners RN MS FNP, is a Clinical Nurse Specialist at Primary Children s Hospital. She has been a nurse for over 30 years working in both acute and primary care settings in Critical Care, Pediatrics and Women s Health. She is the nursing lead and co-chair of the Primary Children s Hospital Asthma Team since 2008 and nurse researcher with the University of Utah Asthma Team since December Karmella assisted with the development and implementation of the Pediatric Clinical Programs Care Process Model for Intermountain in 2011 and She also assisted with research on a paper-based and electronic Asthma Tracker, a tracking tool that helps patients in self-assessment and monitoring of asthma symptoms. Jennifer received her certification to teach Bridges Out of Poverty from Dr. Ruby K. Payne, Philip E. DeVol and Terie Dreussi Smith, the researchers and authors behind the Bridges work. She currently uses her knowledge and resources regarding poverty for the Intergenerational Poverty Project, the Homeless Discharge Planning Committee, the Governor s Commission on Homelessness and other government and non-profit organizations. 10 Lori Mau, MPH Evaluator Lori Sugiyama is an Evaluator with the Asthma Program at the Utah Department of Health. Lori earned a BS in Exercise Science from Brigham Young University in 2005 and a Master of Public Health from Brigham Young University in She has done evaluation and public health work internationally in Guyana and Tonga. She is a board member with the Queen Center, a Polynesian anti-tobacco group, and teaches at the University of Phoenix. Laine graduated this December with her B.S. in Public Health, and plans to continue on to graduate school and achieve her MPH. She hopes to one day become an Epidemiologist in communicable diseases on either a state or local level. Examination of Service Quality of The Health Clinics of Utah Hobble Creek Brett McIff, PhD, MSPH Community Transformation Coordinator Brett has worked in physical activity promotion for over 15 years, working in a variety of fields from personal training to policy development. He received his undergraduate degree at the University of Utah in Exercise and Sport Science. His graduate work continued with a Master of Science in Public Health and a Ph.D. in Public Health at Walden University. Brett s dissertation focused on the role of perception of the built environment among 4th through 6th grade students and their parents in physical activity participation, with a focus on making a connection between public health and urban planning. Brett works with several boards and committees at the national, state, and local levels to promote environments that encourage regular physical activity. He currently coordinates the Community Transformation Program at the Utah Department of Health. 14 April Young Bennett, MPA Health Program Specialist April Young Bennett is the epidemiologist, webmaster and media liaison for the Utah Department of Health, Office of Health Disparities. She has been a finalist for the Robert Wood Johnson National Award for Using Data to Address Health Disparities and was awarded Outstanding Young Professional in Health by Utah State University. She previously worked at the UDOH Tobacco Prevention and Control Program and completed fellowships and internships at a research center specializing in mental health, a veterans hospital and the United States Senate. Ms. Bennett received her Master of Public Administration from the University of Arizona and her Bachelor of Science in Community Health Education from Utah State University.
