1 Breaking Down the Barriers - Masterclass training package and modules Assessment of a patient with urgent mental health needs in an emergency department Module 1 Module 2 Mental health awareness training in emergency and urgent care departments Assessment of a patient with urgent mental health needs in an acute emergency and urgent care setting - assessing risk of harm to self or others, Mental Health Act (MHA), Mental Capacity Act (MCA), deliberate self-harm pathway, acute psychosis and Mental Health Act assessment. Training format: Presentations, classroom and/or case based scenario re-enactment videos Drugs and alcohol Referral of patients with drug or alcohol problems to appropriate services, understanding the scale and complications of alcohol and substance misuse. The aim of this module is to improve collaboration, effective management and sign-posting of people who present to the emergency and urgent care units with urgent mental health needs. To provide doctors, nurses, multi-disciplinary and multi-specialty teams with the framework and practice guidance in assessing selfharm and risk in the context of personality disorder and ensuring safe management To provide doctors, nurses, multi-disciplinary and multi-specialty teams with framework and practice guidance in assessing for the presence of psychotic symptoms To provide doctors and nurses with framework and practice in assessing physical health problems in those with severe mental illness Provide awareness and information on avoiding diagnostic overshadowing that can come with a diagnosis of severe mental illness To improve working knowledge of MCA and MHA legislation and its application in A&E To improve confidence and competence in management of behaviourally disturbed patients, including consideration of pharmacological treatment. The aim of this module is to improve collaboration, effective management and sign-posting of people who present to the emergency and urgent care units with drugs and alcohol problems. 2 Module 3 Training format: Presentations, classroom training and case study discussions Psychotropic drugs and adverse effects Awareness of medications used in mental illness, assessment of side effects of antipsychotic medication To recognise and understand how to refer patients with drug or alcohol problems to appropriate services. The aim of this module is to improve collaboration, effective management and sign-posting of people who present to the emergency and urgent care units prescribed psychotropic drugs experiencing adverse effects. Training format: Presentations, classroom training and case study discussions. Awareness of medications used in mental illness Knowledge of possible side effects of these medications Case-based discussions around adverse effects of medications Managing acutely unwell patients Module 1 Physical health awareness training in mental health settings Managing acutely unwell patients in a mental health setting (Assessment of the acutely unwell patient and NEWS/MEWS scores, interpretation of an ECG, Acute neurology fits and funny turns). Training format: Presentations, classroom training and case study discussions The aim of this module is to improve the assessment and effective management, care and sign-posting of the acutely unwell patient in mental health settings. Identify and carry out an initial assessment of the acutely unwell patient Initiate some immediate life saving measures Understand the use of the NEW score Carry out an assessment of a collapsed patient Identify some common acute neurology presentations. Module 2 Case-based learning on serious incidents including sudden death, cardiac arrest, neglect cases, etc. Training format: Presentations, classroom training, videos and case study discussions The aim of the module is to: Promote a culture of learning through review and reflection of serious incidents and near misses Promoting awareness of a consistent approach across the organisation in the reporting and management of incidents Enabling the effective reporting to ensure that lessons can be learnt and improvements made reducing re-occurrence of similar incidents, improving the safety of service users, staff and visitors,
3 Managing patients with long-term conditions Module 3 Management of a collapsed patient - simulation training Training format: Simulation training, presentations, classroom training and case study discussions Module 1 Managing patients with long-term conditions - physical health checks who does what? National Audit of Schizophrenia health check, Lester tool and Health Improvement Profile (HIPs) Training format: Presentations, classroom training and group based discussions and activity Discussing the human, organisational and financial impacts of incidents Enabling the identification and correction/ improvement of weaknesses in practices, systems or equipment The aim of this module is to: Provide basic life support manoeuvres and practical skills on Situation Background Assessment Recommendation (SBAR) communication. To Improve the timely, collaborative and effective assessment and management of the collapsed patient covering the following areas: o Anaphylaxis o Cardiac arrest o Collapsed patient o Over sedation o Hanging o Hypoglycaemia o Choking The aim of this module is to improve collaborative and effective management of patients with Long Term Conditions (LTC) specifically physical health management of people with SMI. The module will: Highlight the excess physical health morbidity and mortality associated with severe mental illness Develop confidence in undertaking physical health checks and intervention in people with SMI Provide clarity on roles and responsibilities relating to physical health care of people with SMI Promote awareness of the physical health and long-term conditions in mental health settings Promote awareness of the National Audit of Schizophrenia Introduce the Lester tool for cardio metabolic health monitoring
4 Module 2 Module 3 Managing patients with diabetes in a mental health setting - screening of new patients, lifestyle management, screening for long-term complications, working with specialist diabetes teams, care systems and documentation Training format: Presentations, classroom training and group based discussions and activity, diabetes equipment demonstration. Managing patients with Chronic Obstructive Pulmonary Disease (COPD) including smoking cessation in a mental health setting. Training format: Presentations, tutorials, videos, classroom training, group based discussions and activity. The aim of this module and training is to improve collaboration, effective management, care and sign-posting of patients with diabetes in mental health settings. This module addresses how to: Recognise and understand the high prevalence of diabetes amongst patients with mental health problems and the vital importance of screening all new patients for diabetes, unless already known to have diabetes Recognise and understand that diabetes is a complex condition requiring treatment with lifestyle management, in addition to oral medications and often injectable medications, including insulin Recognise and understand the need for all mental health patients with diabetes to have a clear management plan for their diabetes. Include monitoring of blood glucose and blood pressure control and screening for long term complications. Recognise and understand the need to seek formal advice and input from specialist diabetes teams for most patients with mental health disorders who have diabetes. Explore and discuss what care systems and documentation is required to support all of the above, particularly in an inpatient mental health setting, when the patient is admitted, and also for ongoing care. Understand the factors that might make it harder for patients with mental health disorders to control their diabetes This module aims to: Provide an awareness on the assessment and management of Patients with COPD in mental health settings. Provide an understanding of the prevalence of smoking related lung disease, particularly COPD, with an emphasis on patients with mental health needs and the importance of screening Inform about the spectrum of COPD and disability from it. Recognise and understand the need for all mental health patients with COPD to have a clear management plan for their condition,
5 Module 4 Managing sexual health of patients contraception and abortion, assessment and prevalence of STIs, signposting to local sexual health providers Training format: Presentations, case-based studies and scenarios, classroom training, group based to include all treatments including successful smoking cessation strategies, treatments for COPD and patient self-management. To recognise and understand when urgent help is needed and how to seek formal advice and input from specialists. This module will cover the management of sexual health of patients in a mental health setting including contraception, abortion, assessment and prevalence of STIs, and signposting to local sexual health providers. This module will equip attendees to: Be able to confidently discuss sex, contraception, and abortion with patients Be able to risk assess for sexually transmitted morbidity and refer appropriately Be able to develop and maintain a mutually supportive relationship with local sexual health service providers to the benefit of your patient population. Children and adolescent mental health awareness in primary care GP practices Module 1 Child and adolescent mental health awareness training in primary care GP practices Whole GP practice mental health awareness training Training Format: Presentations, case-based studies To provide a broad training model, building awareness, knowledge and skills to identify mental health issues in children who engage with staff within a general practice surgery; Raise awareness of young people s mental health and emotional wellbeing and to promote practice within a risk and resilience framework To understand the resilience framework and the factors that have been evidenced to build resilience in young people and their families, with a view to making the practice more resilient To understand young people s perspectives and what they say is important to them when interacting with health professionals To explore how participants can develop some systems and structures in their workplaces to support young people who have mental health issues.
6 Module 2 Module 3 Self-harm awareness in children and adolescents Training Format: Presentations, case-based studies Eating disorder awareness in children and adolescents Training Format: Presentations, case-based studies The aim of this module is to increase understanding and awareness of selfharm in children and adolescents, how to assess initial risk, plan management in primary care and onward referral practice. To recognise and understand the high prevalence of self-harm in children and young adolescents Understanding self-harm, factors, triggers associated with selfharm. Be able to assess initial risk and onward referral process The aim of this module is to increase understanding and awareness of eating disorders in children and adolescents, how to assess initial risk, plan management in primary care and onward referral practice. To recognise and understand the high prevalence of eating disorders in children and young adolescents Understanding factors and triggers associated with eating disorders Be able to assess initial risk, create onward or ward referral where necessary including a management plan Learning disabilities awareness in children and adolescents Module 1 Child and adolescent learning disabilities awareness in primary care GP practice Learning disabilities awareness in children and adolescents The aim of this module is to increase understanding and awareness of learning disabilities in children and adolescents, how to assess initial risk, plan management in primary care and onward referral practice. The module will help attendees to:
7 Training format: Presentations, interactive case-based studies and scenarios, videos, classroom training, group based Develop improved confidence in communicating with young people with learning disabilities Gain an understanding of the factors that lead to behavioural disturbance, gain skills in assessing and managing challenging behaviours in young people and understand the relationship between learning disabilities and mental health. Classroom based Module 1 Mental health awareness Mental health training in primary care - practice nurses Training duration: Three hours Designed to give primary care practitioners a good understanding of the various mental health issues presented within primary care. This module will cover: Module 2 Training format: Presentations, interactive case-based studies Changing patients behaviour Training duration: Three hours Screening patients for depression and anxiety Talking to a patient who has just divulged that they are distressed or concerned about a mental health problem. Carrying out a biopsychosocial assessment Assessing the severity of depression and anxiety Assessing suicide risk Supporting patients, medications used in mental illness (including depot injections) and the services for people with mental health problems. This module teaches primary care practitioners how to help and support their patients to set appropriate goals and actions. Attendees will be taught to: Training format: Presentations, interactive case-based studies Know how changing behaviour can affect the individual and others Be aware of the types of behaviour healthcare professionals may be in a position to help patients change Be confident in reviewing a patient s progress.
8 Module 3 Physical health in mental illnesses Training duration: Three hours This module covers best practice guidance in carrying out physical health checks and how to use the appropriate tools. Attendees will be taught to: Training format: Presentations, interactive case-based studies Understand the definition of mental illness Know the signs and symptoms of mental illness Have an awareness of the impact of mental illness on physical health, particularly the increased risk of CVD. Module 4 Module 5 Wellbeing management Training duration: Three hours Training format: Presentations, interactive case-based studies Co-morbidities: Using a psychological approach Training duration: Three hours Training format: Presentations, interactive case-based studies Helping people to learn to keep well reduces both physical and mental health problems. This module will teach attendees: Practical advice and techniques that they can use with their patients To understand the definition of mental wellbeing and the factors which contribute to mental wellbeing. To be confident in measuring the wellbeing of patients. To understand how stress can affect mental wellbeing. To be able to proactively help patients maintain their mental wellbeing. People with long term conditions are more at risk of developing depression and anxiety than the rest of the population. This in turn affects the outcomes of their physical condition. This module teaches attendees: Guidance to deliver care using a holistic approach An awareness of the common co-morbidities and an understanding of how having a long-term condition (LTC) can affect people physically, psychologically and socially The prevalence of psychological distress in patients with LTCs and possible reasons To be able to confidently screen patients with LTCs for psychological distress.
9 E-learning modules via the BMJ website and learning portal Module 6 Module 7 Module 8 Alcohol and drug awareness for practice nurses Training duration: One hour Training format: E-learning, Medication for mental health problems in primary care: a guide for practice nurses part one and two Training duration: Two one-hour sessions Training Format: E-learning, Your patient s journey Training duration: One hour This module teaches primary care practitioners how to screen for alcohol and drug problems and how to support patients who may have an alcohol or drug issue. Attendees will learn: To understand the difference between alcohol misuse and alcohol dependence To know how to screen for alcohol problems To have an awareness of the management options for people with alcohol problems To recognise when a patient may have a drug problem To know how to support a patient with a drug problem. This module teaches primary care practitioners to understand which medications are prescribed for dementia, severe mental illness, depression and anxiety. It also looks at the side effects of medications and knowing how to complete a side effect rating scale. Attendees will learn to: Understand which medications are prescribed for: Depression and anxiety, aiding sleep, dementia and severe mental illness Understand how to help patients adhere to treatment Be aware of the side effects and be able to complete a side effect rating scale. Assist s primary care practitioners to help patients with mental health problems navigate their pathway of care by explaining how services are generally provided in England. Attendees will learn to: Training format: E-learning, Understand the current commissioning structure of the NHS. Be aware of treatment and advice that may have been accessed by the patient prior to attendance in primary care or mental health service. Be able to identify any gaps in the patient s care pathway.
10 Module 9 Module 10 Care planning A guide for practice nurses Training duration: One hour Training Format: E-learning, Specific mental health conditions: a guide for practice nurses Training duration: One hour This module enables healthcare professionals to develop an awareness of how they can assist the patient to plan their care and to be aware of the different types of care plans available. Attendees will learn to: Understand how helping a patient plan their care can promote self-management Know the best way to organise the consultation to enhance the care planning process Gain an awareness of the different types of care plans available Feel confident in helping a patient with a long term condition(s) plan their care. This module provides a brief overview of some, but by no means all, mental health conditions that practice nurses might meet in primary care. Attendees will gain: Training format: E-learning, An awareness of some of the mental health conditions likely to be seen in primary care and their effect on patients lives An understanding of how to help a patient with a mental health condition The ability to recognise when and where to refer or signpost a patient to further help or advice.
