3 Careful consideration of a client s needs is required to produce a total telecare solution. The table below is a good starting point. A more detailed care plan can be developed by monitoring then assessing a client s behavioural patterns within their own home. Once the results are analysed, an appropriate package can be planned to reduce risk to the client. This could be through the installation of a combination of environmental control systems, telecare solutions or sensors that trigger a call if an emergency occurs resulting in an immediate response to the situation. 4 Lifestyle Monitoring QuietCare monitors lifestyle activity and is unique as it predicts normal behavioural patterns and reports significant changes, warning of developing problems and prompting early intervention. It can also be used in conjunction with other procedures as an assessment tool for telecare. Such timely, accurate and actionable information enables Service Clients and their carers to make better decisions relating to their safe, independent and dignified living. It provides peace of mind to carers and helps care professionals make assessments and improve the quality and efficiency of care they provide. As QuietCare is a passive system that requires no behavioural change or intervention on the part of the Service Client it is particularly suited for use with vulnerable and at-risk user groups, such as people prone to falling, or with dementia, diabetes or other chronic conditions. Product QuietCare QuietCare motion sensors The standard QuietCare kit, comprising of five small motion sensors (there is no video or audio) and base unit, is easily installed and simply plugs into the Service Client s telephone and electrical supply. The base unit dials in on a freephone number to the QuietCare central computer every two hours, or more frequently if necessary, and downloads the data. The QuietCare system then analyses the raw motion data and automatically develops a baseline of each Service Clients normal daily activity. Noteworthy changes are identified and reports are generated that can be viewed via a secure internet connection by persons authorised by the Service Client. Alerts can be delivered by , pager or via a call centre. QuietCare Programs QuietCare is the entry service level and is geared towards the individual, self-funded Service Client. The QuietCare service level enables carers to view information for the Service Client via an Individual Summary Screen. Automated alerts are delivered to the Emergency Contacts via or pager. QuietCare Alert offers the same service level as QuietCare with the added benefit that urgent alerts are delivered to Emergency Contacts via a Call Centre. As the name implies, QuietCare Professional is designed for the professional sector, such as Social Services, Housing and PCT s. Carers can view information for each Service Client via the Individual Summary Screen. In addition, the QuietCare Professional Service enables professional carers to case manage a group of Service Clients utilising the QuietCare Group Summary Screen. Automated alerts are delivered to the Emergency Contacts via or pager, or via their own call centre. QuietCare Professional Alert offers the same service level as QuietCare Professional with the added benefit that urgent alerts are delivered to Emergency Contacts via a Call Centre. 03
5 Environmental Control Solutions Environmental Control is a term used to describe the remote operation of household equipment and appliances that feature wireless technology developed primarily for use by the physically disabled and elderly to aid independent living. Typically these include door locks, alarm and intercom, power sockets, lights, curtains, electric bed and chairs, TV, satellite, DVD, stereo, central heating, windows and doors, telephone, warden call system and carephones to name just a few. Integrated with telecare sensor activation, it can also allow remote access to the call centre, automatically switch off lights or turn the water or gas off if these sensors are activated. Doors may also be remotely opened allowing emergency services or care professionals into the home if the user is unable. These solutions give the individual greater control over their home environment and allow the carer to develop a total care package by using the solutions to reduce risk within the home and, in conjunction with a telecare package, enable emergency response if needed. Giving this independence to users helps people to live at home for longer and allows older people and people with a disability to maintain independence, thereby promoting well-being and reducing unnecessary admissions to hospital and residential care. Product Possum-SRS Wireless Wall Switch The Wireless Wall Switch is typically used to switch a lamp on/off (e.g. to light a pathway to the bathroom at night) and to switch mains powered appliances on/off standby using the unisocket. Operating at MHz providing an on/off remote control operation. The Wireless Wall Switch is configured by setting a channel on the two dials beneath the rocker cover with a choice of 256 channels. Unisocket The Unisocket is a mains plug socket with a radio receiver which when triggered (e.g. from the wireless wall switch or the Wireless Passive Infrared Detector) will switch power on/off to whatever appliance is plugged. 6 Product Wireless Passive Infra Red Detector Wireless Voice Alert Motion Detector The Wireless PIR detector detects movement and transmits two radio signals to activate a separate receiver (such as a Unisocket) and effect a response; the first radio signal will switch the Unisocket on and the second will switch it off with the time interval being adjustable at between 4 seconds and 5 minutes. 9 Product Verso Portable Button The Verso Portable Button is a light-weight and discreet personal alarm trigger which can be worn on the wrist, around the neck with a wide and comfortable strap or on a belt clip or item of clothing. When pressed, an alarm is raised. Operating on the 869MHz European frequency dedicated to social alarms, it allows emergency calls to be raised even if the button on the carephone is out of reach or in another room. Battery Life 5 years average, depending on usage. The battery is self tested every 48 hours; after 3 consecutive Low Battery states are detected a Low Battery code is transmitted to the control centre. The Fall Detector has an integrated Trigger and Fall Detector meaning that separate devices are not required. A fall condition occurs when the Fall Detector has been in a position of 60 degrees or greater from a vertical. When a fall condition is detected, the IntelliLink sounds a two tone warning. If the Fall Detector is put back in its upright position this warning tone will stop. 10 Product Natural Gas Detector The Natural Gas Detector will raise an immediate alarm when it detects gas. Once the alarm is raised, an optional gas shut-off valve can isolate the supply to the property, ensuring the user is safe while assistance is summoned. The valve is used to shutoff the gas supply when the specified level of natural gas is reached. The valve must be installed by a Corgi Approved engineer. The gas detector must be used in conjunction with the universal transmitter, which provides automatic signalling to the IntelliLink unit. Dimensions 43x154x76mm WxHxD. Item No. - FG Natural Gas Detector Smoke Detector Heat Detector The majority of people now recognise that it is essential to have a smoke alarm fitted within their home, and the reassurance this provides can now be greatly enhanced with our smoke alarm. When a fire is detected, the unit provides a loud audible warning to the user automatically sending an emergency alarm signal through the IntelliLink. When the signal is received at the call centre, the alarm is recognised as a priority call, and the operator can contact the resident to double check it is not a false alarm if there is no response or the alarm is confirmed, the emergency services can be alerted. The smoke alarm is supplied In certain locations of a property i.e. the kitchen, smoke detectors may give too many false activations. Installing a dedicated heat detector can solve this problem whilst retaining a high level of protection to provide reassurance for the user. Should a fire occur, causing a rapid rise in temperature, the unit will activate once 58 o c is reached raising a local alert. Intruder/Inactivity Monitoring using PIR Detection Inactivity monitor The PIR can look for movement within a defined time. If the user has not moved within that period, due to a fall or feeling unwell, for example, the carephone will send an alarm to the carer/monitoring centre. Intruder monitor If the user leaves their property to stay with friends or visit a day centre, a button on the carephone or the portable button can be used to set the system into away mode. The away mode will detect any intruder, giving peace of mind to the user. Low Temperature Sensor During the winter months, one of the greatest dangers to the elderly and vulnerable is insufficient heating of their property. If this continues over a period of days, or even hours, hypothermia can set in within a surprisingly short time. A thermostat located within the property allows monitoring of the ambient temperature within the dwelling. If this remains too low over an extended period, an emergency alarm is automatically raised. Time periods are fully programmable allowing the best settings to be achieved for each individual. If the temperature falls below the set level an emergency alarm is automatically raised. 11 Product Flood Detector The Flood Detector is installed within the bathroom or kitchen of a property and will alert a user when a sink/basin or bath overflow occurs. It will also detect water on the floor from other sources such as a burst pipe or from a flood from a flat above. This neat, unobtrusive detector will provide an early warning of any potential flood situations which can avoid large repair bills, inconvenience and insurance costs. Dimensions 90x55x28mm WxHxD. Bed Occupancy Sensor An alarm is raised when a person leaves the bed by a pressure mat under the mattress. This indicates that a user has got out of bed or a chair and is potentially putting themselves at risk. Carbon Monoxide Detector When gas appliances become faulty or have not been serviced correctly, a dangerous build-up of carbon monoxide can occur. As this toxic gas is colourless and odourless, it is essential that early detection is achieved. Installing carbon monoxide detectors adjacent to the appliances in question - for example, the central heating boiler or a gas fire, will ensure an emergency alarm call is raised when the gas build-up reaches a concentration which is hazardous to health. 12 Product Pull Cord In extra care situations it may be preferable to have fixed emergency call points in certain rooms of the home. Pull Cords can be ceiling mounted in convenient locations allowing the user to raise an alarm if their pendant trigger is not accessible. The alarm is raised by simply pulling down on the distinctive orange cord, which incorporates two triangular handles to provide ease of grip. Item No. - FG Pull Cord. Pill Dispenser A fully automatic Pill Dispenser takes the worry out of dispensing medication to older or vulnerable people. At pre-programmed times the Pill Dispenser carousel rotates to allow the user to take the appropriate medication. An intermittent audible alert tone is provided as a medication reminder. The unit knows when the pill has been removed from the dispenser, so if the medication has not been taken after a preset time, a radio link to the carephone will automatically call the control centre. It can also be configured to alert carers or the control centre, if the person has not got into or out of bed by a certain time. When leaving the bed during the night, the bed-side light will be switched on, lighting their way and reducing the risk of a fall. A timer can also be set to ensure the sensor will work only during night time hours. The user can switch the light on or off from within the bed with a remote switch, further reducing the risk of falling out of bed. Dimensions 650x95x20mm WxHxD. Enuresis Sensor Provides an early warning for individuals prone to incontinence whilst sleeping. A sensor pad placed between the bedding and mattress allows detection of excess amounts of moisture. If this occurs, an emergency alarm call can be transmitted. The bed sensor is designed to be easily cleaned, dried and replaced and is attached to a control unit by a very slim wire which is designed to not disturb the user while in bed. Item No. - FG Enuresis Sensor. 11 Door Contacts The security and safety of vulnerable users can be guaranteed discreetly and remotely by ensuring they remain within their property at certain times of the day or night. Using magnetic contact devices, the opening of doors and windows can be automatically monitored. This allows the call centre to raise the alarm if a person leaves the house at an unusual time. Time periods are fully programmable allowing the best settings to be achieved for each individual. Door Contacts can also monitor intruders and can be set in an away mode which will send an alarm to the control centre if access is detected while the user is out of the house, for example. 13 Product Care Sensor The Chubb Care Sensor provides a unique non-intrusive, non-bodily worn solution to provide security for older and disabled people where staff members are present on site to activate and deactivate when required. The Care Sensor alarms if a person does not get back into bed within a predefined time period; measures the time it takes to get out of bed, the time it takes to walk from the bed to a designated point, counts the number of times the bed is left during the night and detects people entering the room when it is armed. The Care Sensor is mounted on a wall over the bed, configured using an infrared remote control and linked to the carephone to trigger alarms. It allows bed exit monitoring without having pads or sensors physically in contact with the bed, giving an exceptional product lifetime with low maintenance. Any other seizure types such as partial or absence seizures without movement would not be detected. The detector is equipped with a movement sensor that is positioned between the bed base and mattress. The sensor has an adjustable level control with a movement detect confirmation LED. A further control, delay, is used to minimise false alarms caused by normal movements during sleep. Using these controls it is possible to fine-tune operation of the monitor to suit most bed types and convulsive seizure types. A built in microphone can be used to detect grunting or groaning sounds that sometimes precede a seizure from adult males. Again, an adjustable sensitivity control allows the correct level to be set. 14 Carer Alert Telecare technology from Chubb Community Care has traditionally provided support to those living independently via a carephone to a community alarm centre. However in some cases there may be a need for an on site carer to be notified immediately that an incident occurs within the home. This system can either work in isolation or with a default setting through to a IntelliLink carephone. This allows the carer to leave home safe in the knowledge that the alarm will notify them either on their mobile telephone or the Community Alarm Centre. The Carer Alert is used to indicate alarms from portable and stationary telecare solutions. The device has 8 indicator lights for which up to 8 telecare devices may be assigned. Along with the lights, there is a buzzer to alert the carer as soon as the alarm is activated. The box itself can also be semi-portable when used as a stand alone system so it can be plugged into the nearest power socket to the carer. Many formal and informal carers live on site with the people they look after. Carer Alert provides peace of mind to carers, allowing them to live an independent life and can help manage the risk of having the person they are caring for living at home. Data such as blood pressure is transmitted confidentially to a response centre or clinician, and any vital signs outside of set parameters will trigger a response. For people with medical conditions that require regular care and monitoring, Telehealth means the difference between spending time in hospital and living independently and comfortably at home. Telehealth systems will ensure that they still receive the protection and reassurance they need including being reminded about medication to clinical consultations via an automated question and answer system, videophone or direct speech. For hospitals and other healthcare providers it means freed-up beds, fewer unscheduled visits to A&E units, G.P s surgeries or clinics and more efficient use of both hospital-based and home-visiting nursing staff. The Chubb Community Care telehealth systems can also be used by public service providers such as Social Services for everything from care in the community to occupational therapy, which results with savings in valuable time, resources and cost. Product Care Companion Level 1: The basic Care Companion is a self-contained home unit perfectly suited for a person or patient requiring assessment and reminders. It is designed for patient ease of use and features large text and a touch-screen. The Care Companion prompts the person or patient to take medications and answer easily programmed questions related to their specific health needs. Data collected by the Care Companion is sent to a Care Companion Server every day via the patient s home telephone line to a web based server. Information is accessed by health professionals via a web interface, designed to support health management by providing medical data, daily answers to prompted questions (such as how do you feel compared to yesterday? ), data trends and alerts at set parameters. Care Companion with Wireless Medical Devices Level 2: Medical peripherals are added to the basic Care Companion to meet the patient s specific health needs. The Care Companion collects the data then sends it to the Care Companion Server. If the server receives data that exceeds pre-set parameters, the assigned caregivers are notified. Caregivers can access their patients medical data via a secure web browser. The data can be reported and graphed to meet the caregiver s needs. 15

19 The remote health management service for home-based patients suffering from Chronic Heart Failure (CHF), employs telecare technology to acquire on a daily basis, key physiological, lifestyle and medication compliance parameters. The acquired parameters are collected using a fit-for-purpose, hand-held terminal, which has been designed with use by a wide population of patients in mind, employing large buttons and a big clear display to aid usability, and predominately using simple graphical symbols for patient interaction. The handheld unit can collect ECG and Bio- Impedance measurements using built-in sensors, as well as a wide range of other physiological parameters from third party sensors and devices. The collected parameters are automatically transferred to a central, secure database using an 0800 telephone number, thereby incurring no cost to the patient. Furthermore, the terminal employs rechargeable batteries and is suitable for the patient to use while temporarily away from home - for example if travelling. From the central, secure database, this data is made available to the healthcare professionals responsible for the patient s care via a secure web site. Normally this is accessed at scheduled consultation times or in the case of an unexpected development in the patient s condition. The acquired information is also fedback to the patient in an easy to digest graphical report showing trends in their blood pressure and the corresponding trends in their lifestyle parameters and medication compliance. The aim of these reports is to help the patient understand how changes in their lifestyle and medication affect their condition so providing a tool for them to autonomously manage their disease. Weekly reports showing the trends for the preceding 4 weeks are delivered to the patient s terminal, while more detailed paper reports are delivered via post each month. Doc@Home Software Care providers are able to review the information recorded by the patient at any time using the Doc@Home secure web connection. The short, medium and long term trends of health status and peak flow can be immediately assessed and interpreted in the light of lifestyle, medication and compliance. Authorised users can define the criteria for Alerts and customise the questions asked of the patient each day. Events leading up to exacerbations can be reviewed and appropriate advice given to the patient. All of the information is readily available should the patient make telephone contact with the care provider in order to report a concern over any recent development or health change.
