Poole Community Transport Service Contact: Nicola Gray Community Transport & Travel Co ordinator Telephone Poole Community Transport Service offers low floor and wheelchair accessible transport, along with door to door services by Dial a Bus and the Voluntary Driver scheme, for Borough of Poole residents who do not have access to a car and are unable to use, or find it difficult to use, public transport. Am I eligible to become a member? You can apply to become a member if: you are a resident of Poole you are unable to use public transport for some journeys you do not have access to a car you need help getting into Poole and/or getting to essential appointments What does membership offer? Members may pre book a trip, giving three days notice, on either of the two following services: a. Dial a Bus for shopping trips into Poole Dolphin Centre, Shopmobility and also Asda and Sainsburys The bus operates Monday to Saturday. You may have three Dial a Bus bookings on our system at a time. You will be collected from your home from 10.45am onwards. You will arrive in Poole by 12pm. Dial a Bus stops at the Shopmobility (Dolphin Centre) and Sainsburys on Tuesdays to Saturdays, and Asda, Holes Bay on Mondays. You will have approximately two hours shopping time and can be collected at any of the pick up points for the return journey home. All vehicles are fully accessible and can accommodate your shopping trolley as well as mobility equipment such as a zimmer frame/rollator or wheelchair. Subject to assessment, you may travel whilst seated in your wheelchair. On Thursdays, Upton residents may travel on the Dial a Bus as well as Borough of Poole residents. Mobility scooters are not permitted on Dial a Bus for safety reasons. 2

Voluntary Driver Service ideal for essential journeys such as GP, dentist, church service, lunch club or to visit a friend or relative in a care home or hospital Voluntary drivers will collect members in a car from their home and take them to appointments, social events or for essential journeys. We cannot provide a voluntary driver for your hospital appointments or shopping. In some circumstances hospitals can provide a car and driver for getting to appointments please contact your GP for more information. Voluntary drivers provide transport in their own cars for your essential journeys. All drivers have had a DBS check. We have a wheelchair accessible vehicle that may be pre booked with a voluntary driver if you need to travel whilst seated in your wheelchair. A safety assessment will be undertaken before you travel. Subject to completion of a short training course, family or friends may hire the wheelchair accessible car to take you out in your wheelchair. A charge will be made based on the mileage covered. You may have three bookings on our system at a time. We will always try to accommodate your request for a voluntary driver; however, this service is dependent upon driver availability. Mobility scooters cannot be accommodated on the voluntary driver service for safety reasons How much will it cost? Annual membership is 5 per year. Dial a Bus fares are 2 each way / 4 return. Voluntary Driver Service journeys are 4 for the first five miles and 1 per mile thereafter. A minimum return trip is 8. c. Concessionary Travel If you do not have a bus pass (Connect card), or are unable to use yours, you may be eligible for vouchers to help pay for your Dial a Bus and Voluntary Driver journeys. Eligibility for vouchers will be assessed at the time of joining. If you feel you are eligible to become a member then please contact us on telephone number A referral will then be passed to the Community Transport & Travel Co ordinator who will arrange to visit you at your home to confirm eligibility, discuss your transport needs and decide how we can best help you. 3

Poole Shop Mobility mobility scooters for day hire in Poole town This scheme is operated by Wheels for Freedom (registered charity number ) who are committed to giving freedom of movement by loaning mobility scooters, power chairs and manual wheelchairs to any person who has difficulty walking. Tel: Address: Level B Multi Storey car Park, Kingland Crescent, Poole BH15 1TA How to Find Us If travelling by car: Follow the town centre roundabout signs depicting a wheelchair with a basket. When you enter the car park, take the left-hand lane and go up one level. If travelling by bus: From the bus station, take the pathway by the chip shop. Opposite is the multi-storey car park. If you walk to the right-hand side, you will see a ramp which leads you to the office. Open Monday to Thursday and Saturday 9.45am until 3.45pm Closed on Bank and Public Holidays Bookings must be made in advance on Charges Day Visitors: 5 per wheelchair or 7.50 per powered vehicle per day plus a 5 refundable deposit Trial Member: 10 per quarter and 1.50 per trip at any of our town centre sites Full Member: 20 per year and 1.50 per trip at any of our town centre sites, plus a discount if using the out n about service For users of their equipment who have never used a mobility scooter before, they will give a lesson to familiarise the customer with the controls and ensure that he/she is competent in the control of it. They will also willingly give lessons to any person who has their own mobility scooter but is unsure of the safest operating procedure. 4
5 3. Patient Transport Services for hospital appointments in Poole a. Dorset Patient Transport Bureau Open: Monday Friday 8am 6pm Tel: Website: Patient Transport Services are provided for patients who are being transported to a NHSfunded service for NHS treatment and who are deemed medically eligible. This service is for non urgent, planned transportation of patients whose medical condition is such that they require the skills or support of clinically trained PTS staff and/or their equipment on/after their journey. Eligible patients are those where their medical condition and their mobility means that they require the skills or support of PTS staff on/after the journey or where it would be detrimental to their condition or recovery if they were to travel by other means. How do I book patient transport? You or a nominated person on your behalf will book transport directly by calling the Dorset Patient Transport Bureau (Dorset PTB) on the low rate call number It would be helpful when calling if you have the following to hand: GP name GP surgery NHS number If I am not eligible for patient transport, what are the alternatives? The Dorset PTB will direct you to alternative methods of public and volunteer transport. If you are on low income, you may be able to get financial help from the Healthcare Travel Costs Scheme (HTCS). Call for further details. b. Hospital Car Share for hospital appointments to Poole Hospital Tel: carshare@pooleradiocabs.com This is a private scheme run by Poole Radio Cabs. There is a minimal fixed price for your journey, which can be quoted when you book. The scheme costs for annual membership, which covers the cost of an ID card. This can be paid to your driver, on your first booking. You may have to share with other patients; however they aim to collect you within minutes of your call. You can use this scheme as often as you like, but must use a valid ID card for journeys. The cost of the journey is based on a flat rate within bands and depends on how far you need to travel. Current approximate costs per journey are: 3 miles 6; 6 miles 9; 9 miles 12; Please check for estimated costs at the time of booking. 5

'BUSS' Bus Service Contact: Thelma Green Tel: Address: Old Rope Walk, Hamworthy, Poole, Dorset, BH15 4AU Open: Monday to Friday, 10am 12 noon This is a local service covering Poole, Dorset The service is provided by Age Concern and is very popular. It needs to be booked well in advance. The 14 seater bus is available to hire for local travel or day trips and a volunteer driver will be provided. This service is for organised groups of older or disabled people, and their carers, living in the Poole and Westbourne area only. The charge is 30 for the first 20 miles, then 85p per mile for each extra mile. Wheelchairs cannot be accommodated. 5. Wheelchair accessible Taxi Companies The following business details are for information only and are in no way recommended by the Poole Well being Collaborative or any other associated organisation. There may be other companies that offer similar services. Able Taxis, Poole Tel: Wheelchair accessible vehicle with understanding driver Dial a Cab Tel: Can take up to two passengers as well as the wheelchair Disabled Transport Services Tel: They have seven, specialist, wheelchair-accessible vehicles for transporting the disabled and those who are wheelchair bound. Travel tokens are accepted. They offer door to door transport for those with limited mobility, as well as for users of all types of wheelchairs, including electric mobility scooters. Drivers are all vetted, licensed and fully insured, experienced, caring and careful. All vehicles are rear loading with ramps or lifts. Poole Radio Cabs Tel: They have 10 wheelchair-accessible vehicles. These cannot be used as part of the Poole Hospital car share scheme. 6
