1 The Personal Care and Support service How we can help Making Surrey a better place
2 The Personal Care and Support service offers Surrey residents a front door to information and support. Our aim is to help people and their carers live as independently and safely as possible, remaining in their own homes for as long as possible, if they wish, or other accommodation of their choice. The teams offer a range of special skills to deliver personal care and support services and are based locally in the 11 District and Borough Council offices throughout Surrey. 3
3 They also work closely with partners in health, the local councils and independent and voluntary sector organisations to provide the right support residents need in their local community. Each locality has different needs and all agencies work together to respond with services that make the most positive impact on residents lives. Our local managers know their local area which ensures they understand what support is available in their communities and can help people to access those services easily, regardless of who provides it. We constantly seek people s views on what they need locally to live and age well. The information in this leaflet explains more clearly the roles and functions within your local teams. 5
4 Social Care Locality Teams The Locality Team is responsible for dealing with your initial enquiry, arranging your assessment, agreeing support plans and reviewing your needs. The 11 locality teams are made up of social care workers including qualified social workers and occupational therapists, assistant practitioners, support brokers and business support staff, who contribute to arranging information and advice, assessment and support. After initial contact, the first step is to have an assessment from a social care worker in the locality team so that your needs can be identified. This initial assessment will establish whether or not you are eligible to receive social care services. All residents are entitled to a free assessment of their needs, which also includes carers who provide unpaid care to a family member or friend. 7 Social Care Locality Teams
5 Staff from the Finance Assessment and Benefits Team will also meet you to ensure you receive any benefits you are entitled to and complete a financial assessment to determine if a contribution towards the costs of care is required. The Supported Self Assessment is a full assessment where you give a detailed description of your particular circumstances and needs. You can complete this on your own or with the help of someone who knows you well, like a friend or family member, or one of our staff. We often post or this in advance so you can have time to think about your needs and discuss with your family. After completing your Supported Self Assessment, we can use this to determine how much money for a personal budget you can use to create a support plan to meet your needs. Our staff can work with you to do this, see page If you already have a support plan, our staff will check how you are managing and review your situation. This will take place three months after your support has been put in place and is a chance for you to review how well things are working. Parts of your support can be changed, if necessary, to meet any changing social care needs. If you are not eligible for funded support, our social care workers will provide free information and advice and help you find support, possibly provided by another organisation. If you need longer term support, the In Touch Service within the locality team provides professional support, advice and information for people who are open to the service via face to face meetings and regular telephone contact and will review your needs at least once a year. Our locality teams are open from 9am to 5pm on weekdays. Our out of hour s service (5pm-9am weekdays and weekends) covers emergencies outside this time. See page 21 for contacts information. 9
6 Our Reablement Service works alongside the locality team to provide an assessment and reablement service, helping people regain skills for independent daily living. Our specially trained reablement assistants help you to learn how to accommodate your illness or condition and maximise your level of independence by learning or relearning the skills necessary for daily living and to remain independent at home. The reablement service is time limited and varies depending on each individual s needs, focusing on maximising an individual s independence, preventing admission into hospital, following discharge from hospital or to support someone through a short term crisis. This service is available for up to six weeks without charge. Our reablement assistants operate seven days a week between 7am and 10pm. 11
7 Keeping people safe Our local Safeguarding Advisors help to keep people safe from harm. They work with families, partner agencies and locality managers in reviewing any allegations of harm against an individual and taking necessary action to protect people, also bringing any criminal matters to the attention of the police. Being harmed is anything that hurts or injures you or another person, whether it is in the form of discrimination against a person, financial exploitation, physical, emotional, sexual or verbal harm, or domestic abuse or neglect. 13 Keeping people safe
8 Our teams increasingly work with the Surrey Fire and Rescue Service to keep people at high risk safe from fire hazards in their own home. We also work closely with the local district and borough councils to encourage the use of telecare as a great way for keeping individuals safe at home and independent while giving reassurance to families and carers. 15
9 Arranging your support Once you have agreed with the social care worker how you intend to meet your needs they can arrange for funds to be paid into a bank account so you arrange your own support. This is known as a direct payment. Our staff will explain how this works and your responsibility to use the funds in a lawful, effective and affordable way. If you ask us to arrange your support we will agree the choice of provider and agree a start time for the service. 17 Arranging your support

A support broker provides independent support to help you identify your needs, find out what resources are services are available, work out what support will best meet your needs and preferences and organise and manage this support. Support brokers are either based in locality teams and the transition team (for younger adults with disabilities) or work for independent organisations that have a contract with Surrey County Council, see page
