This map shows immediately the actual astigmatism. More then 3 colors present a suspected high astigmatism. 1
2 Axial Absolute Normalized Tangential Tangential Absolute Normalized 4 combinations of mapping Axial Keratoconus Apical Curvature. This index repre- sents the power of the apical cornea. Apical Curvature Gradient. AGC higher kerato values than 2 D/mm show s possible compatibility with keratoconus. SI is a vertical asymmetric index. Positive values means top hemi- sphere more curved than the bottom one, and vice versa. KPI: Keratoconus Probability Index. Value in Green: Not compatible with Keratoconus. Value in Yellow: Suspected keratoconus. Value in red: Compatible with keratoconus. Elevation Maps of the cornea (BFS) Steep center Ectasia Diagnosis and Management Post-op Flat periphery The normal cornea is prolate, Q < 1, meaning that meridional curvature decreases from center to periphery. Prolateness of the normal cornea causes it to rise centrally above the reference sphere. The result is a central hill. Immediately surrounding the central hill is an annular sea where the cornea dips below the reference surface. In the far periphery, the prolate cornea again rises above the reference surface, producing peripheral highlands. 7 Corneal Cross-Linking First introduced by Theo Seiler MD Involves saturating the cornea with riboflavin (Vit B2) Expose cornea to UV light (370 nm) for 30 minutes Riboflavin absorbs UV light and produces singlet oxygen Causes cross-linking of collagen fibers and extracellular matrix proteins To protect the endothelium: Soak cornea for 30 minutes prior Debride corneal epithelium 37 Ensure riboflavin has penetrated to the AC Corneal Collagen Cross- Linking Riboflavin prevents penetration of uv light Older corneas vs. younger corneas and progression of keratoconus CXL appears to be the first technology than can halt the progression of ectasia 38 CORNEAL CROSSLINKING with Riboflavin 0.1% (Vit B2) increases corneal rigidity by 328,9% CORNEAL EPITHELIUM = LYPO-SOLUBLE BARRIER can accept only lypo-soluble molecules smaller than about 500 Dalton RIBOFLAVIN (Vit B2) =376 Dalton HYDRO-SOLUBLE MOLECULE Increased rigidity of the cornea caused by intrastromal cross-linking Ophthalmologe Dec;94(12):902-6 Spörl E, Huhle M, Kasper 39 M, Seiler T. 40 RIBOFLAVIN cannot pass beyond the CORNEAL EPITHELIUM Corneal cross-linking in vivo and ex vivo Riboflavin concentration determined by HPCL chromatography in corneal stroma exposed with and without epithelium (Caporossi A. et al.
