The Community Consortium meets quarterly (a group of individuals bringing a community perspective to perinatal issues) July 25 th Consortium meeting, Same Old Gap, at Unity Health System West Auditorium. September is Infant Mortality Month, Save the Date for Upcoming Events. PNMC Coalition meetings: The Perinatal Coalitions meet quarterly (Oral Health, Perinatal Loss & Mood Disorder and Perinatal Smoking Cessation). 2 Programs and resources in the Community: Perinatal Smoking Cessation Coalition meeting is September 6 th at UHS. Perinatal Oral Health Coalition meeting is September 13 th at UHS. PNMC Annual conference & Doula workshop Save the Dates, November 14, Doula workshop, November 16, PNMC Outreach Coordinator educates women in the community about pregnancy and helps them to get into care as early as possible. A Healthy Baby Begins with you and Breastfeeding campaign Advertisements have begun running in the inner city of Rochester, on the radio, on billboards, in the buses and at bus stop shelters. PNMC Health Educator is available to do presentations in the community. 1. Healthy Start Center at Unity Health System, 89 Genesee St. Offers help to pregnant and parenting women with children up to age 2 that live in 14608, 14611, 14619, 14605, 14609, 14621, 14607, 14606, or Center provides educational services, smoking cessation, stress reduction, and mental health therapy, prenatal yoga classes. Offering "Incredible Years" baby and toddler program for 1-3 years old. 2. Comienzo Sano (Healthy Start) Clinton Family Health at Upper Falls Boulevard Offers prenatal education, childcare, centering pregnancy yoga classes, and parenting programs. A Refugee group meets at the Clinic on Fridays. Women must be pregnant, or have children under the age of 2 and live in zip codes 14605, 14606, 14607, 14608, 14609, 14611, 14613, 14619, or Action for Better Community & Faith-based community FOCUS PLUS program Promotes and provides opportunities for low-income individuals and families to become self-sufficient. 4. MCLAC Free civil legal services to Monroe Co. residents. Topics include fair housing, employment rights and public benefits. Food stamps can be used at the Public Market to purchase fruits, vegetables, breads, baked goods, meat & fish etc. There are no age limits to qualify for food stamps. You can own a home/and or a car and get food stamps. Must meet income eligibility. Community Health Advocates program connects family to health care and health insurance. 5. Ther-RX Corp. Offers the FDA-Approved drug Makena, for women with preterm birth risks who are expecting again. The drug is covered completely for uninsured patients. As of June 15, private insurance covers injections at no charge. 6. Court Appointed Special Advocates (CASA) Advocates for abused and/or neglected children since 1981.They train volunteers to advocate for children who are risk for being removed, or have been removed, from their homes. Their goal is that all children have a safe, permanent home. 7. Ontario County Public Health Offers services that includes maternal child health, prenatal care, infant/child health, breastfeeding education, lead testing, referrals and resources. Their maternal child resource guide is available as an online resource at 8. Chemung County Health Department MCH program Promotes wellness for women, children and families. RN s provide individual care to mothers, newborns and children. Services include prenatal and postpartum care, infant/child health, referrals and resources, breastfeeding education, growth and development of newborn/child. 9. Birthright of Rochester Offers free, confidential and nonjudgmental support to girls and women who are distressed by an unplanned pregnancy. 10. Rochester Area Birth Network Advocates for health, safety and informed childbirth choices. Journey Through Grief Offers perinatal loss support, grief counseling, seminars and lectures. 12. March of Dimes Helps Moms have full-term pregnancies and healthy babies. Offers regional program services, scholarships, grants awards to Agencies to fund small programs and RFP grants are available for larger programs. Coordinated Care Services Inc.(CCSI) The goal is to ensure that uninsured families (eligible adults, teens and children) have access to affordable health insurance. There have been significant changes to Medicaid, FHP and CHP programs. 15. ABVI Goodwill FingerLakes, 211 Lifeline Trained call counselors assists people with non-emergency questions. Offers referrals for emergency food, shelter, clothing, crisis counseling, substance abuse issues, employment, financial and legal issues, physical and mental health needs, and more. Provides educational presentations in the community. 16. Parents as Teachers/FRC Hillside Provides parents with child development information (focused on prenatal to five years) and involves them in parent-child activities that encourage language development, intellectual growth, social development, and motor skills. PAT is a home visiting program for all families based on understanding that parents are their child s first teachers. The program offers personal visits, group meetings, screenings, and linkages to community resources if needed. The Parent Group curriculum includes family talk; Incredible years parent training, Los Ninos Bien Educados, Effective parenting skills, Parenting Wisely, STEPS to early brain development. 5 Next Meeting Parents As Advocates is a home visiting program for all families with children entering their 1 st formal school experience. The program will focus on your child s transition into school life and will help the child have a successful, positive school experience. 17.
