UnitedHealthcare is pleased to announce the UnitedHealthcare Online All-Payer Gateway solution, a secure, Web-based connection that you can use to connect to a leading clearinghouse where you can submit batch claims to most commercial and government payers. UnitedHealthcare has negotiated reduced pricing on your behalf with three leading clearinghouses. Using your existing UnitedHealthcareOnline ID and password, you can seamlessly link to any of these clearinghouses: Electronic Network Systems (ENS), ProxyMed, or Per-Sé Technologies. To review the unique payer lists and price sheets, just click on the UnitedHealthcare Online All-Payer Gateway link located on the UnitedHealthcare Online homepage. In the vendor drop-down box, select ENS, ProxyMed, or Per-Sé Technologies and you will be taken to their respective marketing pages. Register Today! 3 What is the UnitedHealthcare Online All-Payer Gateway solution and is it a good fit for your practice? UnitedHealthcare Online All-Payer Gateway is a web-based connection that you can use to connect to a leading clearinghouse website ENS, Per-Se Technologies or ProxyMed - where you can submit batch or single claims to most commercial and government payers. Do you have a Practice Management or Billing Software System? If you do and you (or your software vendor) can locate the root claim file, or you are able to currently send a file to print, then you can use the UnitedHealthcare Online All-Payer Gateway link to upload a multi-payer claim file via the internet to the clearinghouse much like sending an with an attachment! If you do not have practice management software, you can still use the UnitedHealthcare Online All- Payer product to access the clearinghouse s Direct Data Entry (DDE) solution. Both ENS and ProxyMed offer DDE solutions where you complete an online HCFA and submit it electronically over the internet. The DDE solution is similar to UnitedHealthcare Online s claim submission functionality, however, you can also use if for most of your other payers. You will not need to download or purchase any additional software whether you upload a batch claim file or use the DDE option! A High Speed Internet connection is highly recommended!! Getting Started Step 1: Open web browser and type Enter your User-ID Enter your Password (this field is case sensitive). Step 4: Click SUBMIT. Step 5: You know you are logged in successfully when the log on box disappears. If you do not have an existing UnitedHealthcare Online User Id & Password, call us toll free at UHC-FAST ( ) to have a User Id & Password generated. If you would like to receive it via , please let the representative know and it will be ed to you within 24 hours! Select a Preferred Clearinghouse Step 1: Click on the UnitedHealthcare Online All-Payer Gateway icon located on the right-hand side of the home page. The typical health care provider can submit 95% of his or her claims electronically! Click on the vendor drop-down box and select each one of the three clearinghouse vendors (ENS, Per-Se or ProxyMed) to view their payer list, price sheet and overview documents on their marketing pages. Once you select the clearinghouse that best meets your needs, begin the registration process by clicking on the Register Now link on the clearinghouse marketing page. Remember, if you do not have practice management software and want to use a Direct Data Entry (DDE) solution, your clearinghouse options are ENS or ProxyMed. Most payers have multiple claims addresses for paper claims. Most payers have ONE payer ID for electronic claims! Information Needed to Register Before starting the registration process, be sure you have the following information available: Tax ID Number(s) Physician name(s) Medicare/Medicaid, Railroad & Champus numbers (group and/or individual) Your office address, phone, , and fax numbers (physical address, not P.O Box.)

4 A form of payment, which can be either credit card or Bank ACH Practice Management Software, if applicable (ie. Lytec, Medisoft, etc.) Current Clearinghouse if known Estimated monthly claim volume (all payers) Current Claim Format, if known (e.g. Print Image, NSF) Registering with ProxyMed Step 1: After clicking on Register Now link, you will be taken to the UnitedHealthcare Online All-Payer Gateway Agreement page. Please review & select the I Agree, Continue button at the bottom of the page. You will be taken to ProxyMed s registration page. Helpful Hint: If you are not directed to the clearinghouse website registration page you probably need to disable your computer s pop-up blocker. To do this, hold down the CTRL key on your keyboard while clicking on the I Agree, Continue button.. Step 4: Step 5: Step 6: Step 7: Enter your demographic information and answer the five questions (starting with Number of Providers) regarding your practice: Helpful Hints: If you do not have Practice Management Software (ie. Lytec, Medisoft), select None. If you re not sure of the software name, select Not Sure. If you are currently submitting claims electronically but you re not sure of the clearinghouse, select Not Sure. Products of Interest Most offices select Professional Claims Submission, (Professional claims are billed on a HCFA; this includes HCPCS codes), Claim Status Inquiry and Eligibility Verification. Click on Submit Enrollment Request Print the Confirmation Page. Go back to click on the UnitedHealthcare Online All-Payer Gateway link on the home page. Select the Complete Your UnitedHealthcare Online All-Payer Gateway Profile link located on the left-hand navigation bar. Select Edit my Profile and select your clearinghouse vendor and then select Save My Profile. You will be returned to the home page and a new UnitedHealthcare Online All-Payer Gateway link will appear under the Notifications link in the middle of the home page. The next time you need to enter the clearinghouse website, you ll be able to click on the UnitedHealthcare Online All-Payer Gateway link and be taken directly to the clearinghouse website. Registering with Per-Se Technologies Step 1: After clicking on the Register Now link, you will be taken to the UnitedHealthcare Online All-Payer Gateway Agreement page. Please review & select the I Agree, Continue button at the bottom of the page. You will be taken to Per-Se s registration page. Helpful Hint: If you are not directed to the clearinghouse website registration page, you probably need to disable your computer s pop-up blocker. To do this, hold down the CTRL key on your keyboard while clicking on the I Agree, Continue button. Step 4: Step 5: Step 6: Click Start Registration. Enter your contact information Select payment option Credit Card or Bank ACH (auto withdrawal), complete all fields & select Next. Practice Registration/Maintenance form Enter Practice/Group name and demographic information & click Save If you are registering two Tax ID numbers, click on Add New Practice to enter the second Tax ID. Or if you re registering one practice, click on Maintain Providers.

5 Step 7: Step 8: Step 9: Step 10: Step 11: Click on New Provider and input the name of the doctor in your practice. If you have more than one doctor, click on New Provider each time until all you ve added all doctors, then select Update and then Exit. You can either Update Practice or Maintain Providers if you need to do so, or if you re finished, select Finish Registration. Print the Confirmation Page Go back to click on the UnitedHealthcare Online All-Payer Gateway link on the home page. Select the Complete Your UnitedHealthcare Online All-Payer Gateway Profile link located on the left-hand navigation bar. Select Edit my Profile and select your clearinghouse vendor and then select Save My Profile. You will be returned to the home page and a new UnitedHealthcare Online All-Payer Gateway link will appear under the Notifications link in the middle of the home page. Registering with ENS Step 1: Step 4: Step 5: After clicking on Register Now you will be taken to the UnitedHealthcare Online All-Payer Gateway Agreement page. Please review & select I Agree, Continue at the bottom of the page. You will be taken to ENS registration page. Helpful Hint: If you are not directed to the clearinghouse website registration page, you probably need to disable your computer s pop-up blocker. To do this, hold down the CTRL key on your keyboard while clicking on the I Agree, Continue button. Verify the Tax Id number and the Primary Contact information. (If your contact information does not load, ENS (Cheryl Kuzemka) at ckuzemka@enshealth.com with your name and phone number.) Then update the rest of your contact information. Cheryl can also be reached at (719) , ext Answer the next six questions and select Continue. Helpful Hints: If you do not see your Billing Software or don t have one, click Unknown. Most IT platforms are Windows-based. Most software systems can create a print image file. Most offices do not need the EDI translation software. Nearly all professional claims are billed on a HCFA and Institutional claims are billed on UB92s. Patient Statement processing is NOT included in the monthly fee Select No. You can sign up for this feature at a later date. Estimate your claim volume and indicate the number of practicing providers who will appear on a claim form. (This includes PA s, nurse practitioners, or anyone else that will appear on the claim form.) Level of Service The default is always Gold. This is the option you want! Select Continue. Step 6: Product Selection IMPORTANT: PLEASE REVIEW BELOW CAREFULLY 1. Health-e Web Entry/DDE Only choose this product if you do not have a Practice Management System/Billing Software. 2. Health-e Claim most practices with Practice Management Software will choose this product. Select this option if you can create a print image file format using your Practice Management Software. 3. ANSI-X12/NSF Select this option if your software can create an ANSI or NSF type format. If you re not sure which option to select, contact ENS at , press 0 and ask for Sandra F. Step 7: Review fees for accuracy and then enter your credit card or bank routing information & select Buy Now. NOTE- You will only be charged for the amount you see on the screen and your payment clock will not start until you submit your first batch of live claims.

6 Step 8: Step 9: Step 10: Print your contract as you will not be able to move to your next page until you do so. Select Continue. If you will be submitting claims for the government payers noted in the Payer fields, please enter your group or individual Payer ID numbers and select Save and Continue. Note: If you do not have the payer Ids available at this time, you can provide them to ENS at a later date. Load each Doctor/Practicing Provider (as they would appear on the claim forms) in the fields provided including their governmental IDs, UPIN numbers & SSN (for Tricare/Champus). Select Continue. Step 11: You can either print out your EMC agreements at this time or ENS will mail these to you. Most offices wait for ENS to mail them. You will receive a letter in the mail from the carrier telling you that you have been approved to bill electronically. You then notify the ENS Enrollment (719) Step 12: Step 13: Step 14: Step 15: Step 16: Select two dates/times for training at least one week from your registration date and Continue. Print the Order Confirmation page and select Confirm this order. You will receive a Purchase Complete page. We recommend printing this page as well. Go back to click on the UnitedHealthcare Online All-Payer Gateway link on the home page. Select the Complete Your UnitedHealthcare Online All-Payer Gateway Profile link located on the left-hand navigation bar. Select Edit my Profile and select your clearinghouse vendor and then select Save My Profile. You will be returned to the home page and a new UnitedHealthcare Online All-Payer Gateway link will appear under the Notifications link in the middle of the home page. The next time you need to enter the clearinghouse website, you ll be able to click on the UnitedHealthcare Online All-Payer Gateway link and be taken directly to the clearinghouse Web site. What happens after registration? Step 1: The clearinghouse implementation specialist will contact you within 48 hours to verify your registration and prepare you for your upcoming training. You will go through a testing process before going into production. You will typically be in production within 3-4 weeks after registration for your commercial payers and within 4-6 weeks for most government payers.
