1 The 2015 Leapfrog Hospital Survey Online Tool Quick Start Guide This guide is designed to help you navigate the online survey tool available on the Leapfrog Hospital Survey website at 1 BEFORE YOU START Before you start using the online survey tool, please review this section for some important information and helpful hints. Browser Compatibility The online survey and CPOE Evaluation Tool perform best when opened in Internet Explorer (IE) 9 or more recent versions of IE, such as IE 11. However, the online survey tool and the CPOE Evaluation Tool are also compatible with IE 8. The CPOE Evaluation Tool will only function properly in these versions of Internet Explorer (it will not function properly in Google Chrome, earlier/later versions of IE, etc.). Page Load Time It may take one or more minutes for the survey to load after you login using your security code; please wait for the page to load. Do not hit the refresh button or you will be locked out of the online survey tool for 20 minutes. Do Not Use the Browser s Back or Refresh Button or Exit/Close Features Once you have logged into the online survey tool with your hospital s 16-digit security code: 1. DO NOT use the browser s Back button. Doing so may cause you to lose any unsaved data. 2. DO NOT use the browser s Close or X button. Instead, use the Log Out button to end your session. Closing the browser before logging out will result in a 20-minute lock out. 3. DO NOT use the browser s Refresh button. Doing so may cause you to lose any unsaved data. April 1, 2015 Page 1

2 2 SURVEY LOGIN PAGE Enter your 16-digit security code on the Survey Login Page to access your Survey Dashboard at NOTE: Do not enter spaces or dashes when typing in the 16-digit security code. If you cut and paste the code from an or another document, remove the spaces between the numbers before trying to login. This screenshot points out areas of interest on the Survey Login Page. 3 3 SURVEY DASHBOARD After logging in with your 16-digit security code, you will be brought to your Survey Dashboard. The Survey Dashboard provides you access to the following navigation buttons: Button Profile Open the Survey Print Last Saved Survey Print Last Submitted Survey Description Provides access to Section 1 Basic Hospital Information of the Leapfrog Hospital Survey This section must be completed and saved before you can access the online survey. Provides access to Sections 2-9 of the Leapfrog Hospital Survey This button is only visible once you have completed and saved your Profile. Downloads a PDF version of your Last Saved Survey This button is only visible once you have saved a copy of your survey. Downloads a PDF version of your Last Submitted Survey This button is only visible once you have submitted a survey. Open the CPOE Tool Opens the CPOE Evaluation Tool This button is only visible once you have completed, affirmed, and submitted Section 2 of the Leapfrog Hospital Survey. April 1, 2015 Page 3

4 4 COMPLETING THE PROFILE & OPENING THE ONLINE SURVEY TOOL Before you are able to access Sections 2 through 9 of the survey, you must first complete the Profile page (Section 1 Basic Hospital Information). Please be sure to review and update your hospital information and contact information. Only organizational and contact information from the 2014 Survey is retained in the online survey. Please update as needed. Once you complete and save the profile page, you can open the online survey and complete Sections 2 through 9. Not all sections of the survey are applicable to all hospitals, so please read each section carefully. If a section is disabled, it means that the section is not applicable for your hospital. For example, Section 3: Evidence-Based Hospital Referral will be disabled for all pediatric hospitals. Subsections may also be disabled based on how hospitals respond to filter questions. 5 5 NAVIGATING THROUGH THE ONLINE SURVEY TOOL TABLE OF CONTENTS The survey menu/ Table of Contents on the left-side of the survey is the only way to move from one section and/or subsection to another. You can navigate to and from any section and/or subsection by clicking on section title, which expands the section, and then clicking on the subsection titles. You do not need to move sequentially through the sections. Separate sections of the online survey can be completed at different times, by different users, in different (non-concurrent) online sessions. April 1, 2015 Page 5

6 FUNCTION BUTTONS The online survey tool allows users to enter responses to questions, save those responses, and log back in at another time to finish additional survey sections. At the bottom of each survey page is a set of buttons. Use these buttons to save your data, clear the page, or submit affirmed sections of the survey and save other sections. Button Clear Subsection Save Responses and Exit Submit Sections, Save Others Description Clears all data from the current page (section/subsection). Saves entered data in all sections and brings user back to survey dashboard. Saved responses are not submitted or scored. Submits survey sections which have been completed and affirmed; saves all other entered data. ENDNOTES There are superscripted numbers indicating endnotes throughout the survey (these match the endnotes in the hard copy of the survey). If you roll your mouse over the number, the endnote pops up in a window. End Notes provide question-specific information for certain questions. April 1, 2015 Page 6

7 SURVEY STATUS While completing the survey, you will notice different colored icons next to each section/subsection title in the table of contents. The colored icons allow you to quickly assess the status of each of your survey sections/subsections. The status icons shown in this table appear in the left-hand navigation bar. Green check marks DO NOT indicate that a section was submitted they only indicate that a section was completed and affirmed. SAVE YOUR SURVEY It is not necessary to constantly save when navigating within the survey and moving between sections. However, you should always save before logging out of the online survey tool by selecting the Save and Exit button (refer to page 5). When you Save and Exit a section or subsection, you will automatically exit the online survey tool. You can come right back into the survey by selecting the Online Survey link in the menu. You will not need to log back in with your 16-digit code. SUBMIT YOUR SURVEY Once you have completed and affirmed each section, follow these steps for submitting: 1. Click on each section title in the table of content (left-side navigation) and ensure that each subsection is complete (green dot icon should be present). Then ensure that the affirmation for each section of the survey (sections 2-9) is complete (green check mark icon). 2. Go down to the affirmation in Section 9 (or the last applicable and completed section) and select the submit button. The submit button should list the number of each section of the survey that you have completed and affirmed. If a section number is not listed, it means that the section has not been completed and/or affirmed and the section will not be submitted. 3. Once you select the submit button, a pop up will appear to verify your submission. If you have not completed and affirmed every section (2-9), the message will alert you to which sections are submitting and which sections you are saving. 4. Always log back into your survey to print a copy of your LAST SUBMITTED SURVEY (see button on the survey dashboard). 5. Review the copy of your LAST SUBMITTED SURVEY for accuracy and completeness. Remember, sections that are not affirmed will not be submitted. If your Survey Dashboard does not include a button to print your LAST SUBMITTED SURVEY, it means that you have not submitted a survey. 6. Review your publicly reported results after the first 5 business days of the month following your (re)submission at starting on July 25, April 1, 2015 Page 7

8 6 CONFIRM YOUR SURVEY SUBMISSION Survey Submit Button (and the pop-up that appears after selecting to submit) should list all of the sections which are completed and affirmed, and that will be submitted to Leapfrog. If a section is not listed, it is because the section is incomplete and/or missing an affirmation. Thank you should be received immediately after your submission. Last Submitted Survey (PDF accessible from your survey dashboard) should say Submitted Survey at the top, have the correct submission date listed, and each section that you wish to submit to Leapfrog should have the affirmation fully completed with the name, title, and date. Time Stamp listing the date and time submitted should appear in bold below each submitted section when logging back into your online survey. The date and time submitted is displayed under the section header for any section that has been submitted during the current survey cycle. Submitted Sections display of date and time each section was submitted Saved Sections will display a green check mark, but the date and time will not be displayed. 7 Logging Out When you have completed the survey and are ready to log out, you need to use the Log Out option provided by the Survey Tool. Responses are not automatically saved. Please save and exit prior to logging out. April 1, 2015 Page 8

9 8 IMPORTANT SURVEY LINKS Once you are inside the online survey tool, there are three important links located in the upper right-hand corner: HELP DESK: Takes you to the Survey Help Desk page from anywhere in the survey. DOCUMENTATION: Opens a new browser window that contains the documents related to the survey. You can access these files at any time throughout the survey process. LOG OUT: Remember to Log Out of the survey before you close the browser. If you close the browser without logging out first, you will be locked out of the survey for 20 minutes. Responses are not automatically saved. Please save and exit prior to logging out. 9 Getting Help Leapfrog makes all the information you need to complete a survey available on the survey website. Please take a look at what kind of information is available on the survey website at The Download Survey Materials section of the website includes a hard copy of the survey with references, instructions, scoring algorithms, FAQs, calculators, and other important information. If you have a question or need to report an issue, submit a ticket to the Survey Help Desk by selecting the Help Desk link on the survey website. April 1, 2015 Page 9

10 10 Print to a PDF It s important to keep a record of your saved and submitted responses. Once you start to save and submit sections of the survey, you ll see two buttons appear on the survey dashboard: Print Last Saved Survey and Print Last Submitted Survey. When you select these buttons, the online survey tool will generate a PDF of your last saved or last submitted survey. You can save the PDF for your records and print a copy to review. Saved vs. Submitted: Saved responses have been entered and saved, but have not been submitted. Only submitted sections are scored and publicly reported. Always review a copy of your Last Submitted Survey to ensure that: 1. Each section is complete and accurate (no typos, etc.) 2. Each section is affirmed (if the affirmation is blank, the section did not get submitted, will not be scored, and will not be publicly reported) April 1, 2015 Page 10
