The faculty, administration, and support staff are committed to assisting you as you proceed through the program to graduation. 7 CERTIFICATE IN PRACTICAL NURING PROGRAM INFORMATION Nursing is concerned with the individual, focusing its efforts on the promotion, conservation and restoration of the individual s patterns of behavior. Nursing seeks to facilitate achievement of an optimum level of physical, social, psychological and spiritual health. ince patient nurse relationships are based upon mutual trust and respect, a nurse s competence facilitates development of trust and respect, a high level of accountability is required when the nurse assumes patient care responsibilities. The Ohio Board of Nursing assures the public of the graduate nurse s competence through the National Council Licensure Examination for Practical Nursing (NCLEX PN) test, taken by all nursing students at the conclusion of the nursing program. Furthermore, the Ohio Board of Nursing continues to monitor the practice of nursing after graduation and upon entry into an employment setting as a Licensed Practical Nurse. To prepare students for the NCLEX PN and for the tasks of a Licensed Practical Nurse, the Practical Nursing Program at Cuyahoga Community College combines nursing theory and practice with science courses and general studies. tudents are given clinical experience throughout the Cleveland area caring for individuals of all ages in medical, surgical, pediatric, obstetric and geriatric settings. The Practical Nursing Program meets the high standards of competence demanded by the nursing profession. The Practical Nursing Program is approved by the Ohio Board of Nursing. In order to facilitate the successful completion of the Practical Nursing Program, to assist with preparation for the National Council Licensure Examination, and to support preparation for entry into the practice of Licensed Practical Nursing, the Nursing Administration and faculty have developed the Practical Nursing tudent Handbook. The handbook includes a description of the program, the curriculum, rules, regulations, guidelines and related information. The handbook is designed to facilitate the achievement of academic and nursing excellence. TUDENT REPONIBILITY TATEMENT The Practical Nursing student is responsible for receiving this Handbook and adhering to the standards and procedures contained herein. Any information contained herein is subject to modification, deletion, or change. When there is a change in program or policy, it will be communicated via the student s Tri C , along with an announcement on the group site. TUDENT E MAIL COMMUNICATION POLICY The Cuyahoga Community College has established e mail as an official means of communication with students. An official Tri C e mail address is issued to each student. ince e mail has been adopted as a primary mechanism for sending official communications to students at Tri C, students must check e mail regularly in order to read important messages and notifications. Certain communications may be time sensitive. Failure to read college communications sent to students official Tri C e mail addresses does not absolve students from knowing and complying with the content of those official communications. tudents must also ensure that there is sufficient space in their e mail inboxes to 1

8 allow for e mail delivery. To avoid junk or spam filters, students who are forwarding mail should add their Tri C address to their address book. 9 TECHNICAL TANDARD FOR ADMIION, PROGREION AND GRADUATION Physical and Mental Qualifications The following are the minimal physical and mental qualifications necessary to be considered for admission into and progression through the Cuyahoga Community College Associate Degree Nursing Program. 1. The ability to assist in lifting or moving patients of all age groups and weights. 2. The ability to move around the patient s room and in work areas. 3. Visual acuity sufficient to observe and assess patient behavior, prepares and administers medications, and accurately read monitors. 4. Auditory acuity sufficient to hear instructions, patient requests and monitoring alarms; and, to auscultate heart sounds, breath sounds and bowel sounds. 5. Gross and fine motor abilities sufficient to provide safe and effective nursing care. Tactile ability sufficient for physical assessment. 6. The ability to provide effective written, oral and nonverbal communication with patients and their families, colleagues, healthcare providers and the public. The ability to speak, write and comprehend the English language proficiently. 7. Critical thinking ability sufficient for clinical judgment. 8. Interpersonal abilities sufficient to interact with individuals, families and groups from a variety of social, emotional, cultural and intellectual backgrounds. 9. Compassion, integrity, interest and motivation. 10. The ability to adhere to ethical standards of conduct as well as applicable state and federal laws. 11. Emotional maturity and stability to approach highly stressful human situations in a calm and rational manner. 12 PHILOOPHY The Division of Nursing Education, Practical Nursing Program is an integral component of Cuyahoga Community College. The philosophy of the Practical Nursing faculty is consistent with the general mission of the College, which is to provide high quality, accessible, and affordable educational opportunities and services to all members of the community. The educational environment of the College is designed to promote individual development and to improve the overall quality of life in a multicultural community. The philosophy of the Practical Nursing faculty at Cuyahoga Community College encompasses beliefs concerning the individual, society, health, teaching/learning, nursing practice, nursing education and the role of the Practical Nurse. These concepts are an integral part of the Practical Nursing Program. We believe: Individuals are a unit who can be viewed as functioning biologically, spiritually, and socially and who initiate and perform self care activities on their own behalf in maintaining life, health, and wellbeing; self care activities that deal with air, water, food, elimination, activity and rest, solitude and social interaction, prevention of hazards to well being, and promotion of human functioning. ociety is composed of complex, multi cultural groups of individuals, families, and communities who possess different beliefs, values, and behaviors. These cultural differences shape a person s thoughts and behaviors. Within society, individuals and groups attempt to meet self care needs, maintain homeostasis, and achieve self fulfillment. Culture is a design for living and it provides a set of norms and values that offer stability and security for members of society. Health is a state that is characterized by soundness or wholeness of developed human structures and of bodily and mental functioning. It includes physical, psychological, interpersonal, and social aspects. Well being is used in the sense of individuals perceived condition of existence. Well being is a state characterized by experiences of contentment, pleasure, and certain kinds of happiness; by spiritual experiences; by movement toward fulfillment of one s self ideal; and by continuing personalization. Well being is associated with health, with success in personal endeavors, and with sufficiency of resources. Nursing is a scientific and humanistic body of knowledge guided by nursing principles, nursing education, and the continuous development of the profession. Nursing practice entails helping individuals across the life span, and groups to meet their self care needs, maintaining optimal health, detecting aberrations from health, assisting in restoring health, and support during life or to a peaceful death. These responsibilities are accomplished through a systematic and deliberate process. Licensed Practical Nurses contribute to the nursing assessment by collecting data; participate in the development of the nursing component of the care plan, provide direct basic nursing care in accordance with the Licensed Practical Nurses scope of practice set forth in the Nurse Practice Act, and contribute to the evaluation of patient responses to nursing care. 6

13 Licensed Practical Nurses work interdependently with other health care team members to effectively enhance the health status, health assets, and health potentials of individuals, groups, families, and communities. The Teaching/Learning Process provides opportunities for planned educational experiences to produce a measurable change in the behavior of the learner. Learning is enhanced when consideration is given to individual differences in cognitive style. Discussions, lectures, seminars, audiovisuals, demonstrations, reading assignments, student presentations, planned laboratory practice, applicable computer experience and clinical experience are utilized to meet the needs of the individual learner. Learning is most effective when theory and skills are taught concurrently, utilized promptly, and throughout the program. Nursing Education involves a learning process that requires interaction and joint responsibility of teacher and learner working toward a common goal. It is an ongoing process that promotes growth and development in cognitive, psychomotor, and affective behavior. The simple to complex instructional approach is responsive to a multi cultural student population with varying learning styles and needs. The Practical Nursing Program is committed to providing educational opportunities to a non traditional student body. It is built on nursing concepts and skills that enable the nurse to adapt in a changing society. Based on a cooperative relationship with community agencies, students are provided with opportunities to develop clinical skills in diverse health care settings. Upon successful completion of the program, the graduates are prepared for entry level nursing positions and are eligible to sit for the National Council Licensure Examination for Practical Nurses. Education as a Life Long Process is essential in today s society. Many changes are rapidly taking place due to advancements in technology and the availability of information. In order for individuals to develop their potential, increase their knowledge and skills, and be accountable for their actions, it is necessary to participate in continuing education programs. 7

14 ORGANIZATIONAL FRAMEWORK The faculty of Cuyahoga Community College, Division of Nursing Education, Practical Nursing Program has developed an organizational framework to provide direction for the nursing curriculum and opportunity for optimal student learning. The organizational framework is derived from the philosophy and provides a basis for the content and sequence of courses. Cuyahoga Community College is located in a large, industrial, urban setting. It serves the residents of the Greater Cleveland area and draws students from this area. These college students are adult learners from multicultural backgrounds. Opportunities for student learning are diverse and enriched by exposure to community based health care facilities. tudents progress through a curriculum designed to increase knowledge based upon concepts of self care for individuals, families, groups and society. Upon completion of the program, graduates are prepared to provide nursing care to patients or groups of patients at home or in institutions, at the direction of a registered nurse or licensed physician, dentist, optometrist or podiatrist. Graduates are eligible to sit for the National Council Licensure Examination for Practical Nurses. The organizational framework is based on Dorthea Orem s elf Care Deficit Theory of Nursing that states that individuals function and maintain life, health, and wellbeing through self care. Practical Nursing focuses on the identification of self care deficits and health deviations. Emphasis is on contributing to the assessment, planning, implementation and evaluation of the person/client s ability to meet self care needs. Care may be provided by family members or others until the person/client is able to meet their own self care needs. ix major themes have been identified and incorporated into the curriculum through the organizational framework. These themes are Nursing Process, Critical Thinking, Communication, Human Development, Cultural Diversity, and the Role of the Practical Nurse. These themes are defined as follows: 1. Nursing Process is central to nursing practice and is the basis for making decisions regarding nursing care. The Practical Nurse contributes to the five steps of the nursing process: assessment, analysis, planning, implementation, and evaluation, providing the framework for the delivery of patient care and is the structure for course content in the Practical Nursing Program. 2. Critical thinking/clinical Judgment is the process of purposeful, self regulatory judgment. This process gives reasonable consideration to evidence, contexts, conceptualizations, methods, and criteria. Critical thinking/clinical Judgment, in conjunction with the nursing process, is the basis for making clinical decisions. 3. Communication is a dynamic interactive process that encompasses the teaching/learning process. Communication is essential in meeting self care needs. Effective oral, written, and non verbal communication skills are necessary to apply the nursing process, to promote selfcare, and to guide and teach patients. Teaching and learning techniques are incorporated throughout the program. Practical Nursing students communicate with individuals, families, and groups to further the goal of self care. 8

15 4. Human Development: All human beings have specialized growth and developmental needs and related tasks to accomplish at various stages of the life span. Individuals differ in their ability to meet self care needs. The amount of assistance necessary to meet self care needs is affected by the developmental stage and adverse effects of illness. Practical Nurse students are provided with opportunities to assist individuals to meet self care needs at different developmental stages. 5. Cultural Diversity: Cultural and socioeconomic background factors influence the ability to meet self care needs. Variations of self care needs result from unique life experiences. Cultural norms are learned first in the family and then in societal groups. Practical Nursing students are exposed to the impact of socioeconomic and cultural diversity both in the classroom and in clinical agencies. Knowledge of cultural variation is incorporated when planning nursing care. Role of the Practical Nurse: The Practical Nurse is a provider of care utilizing specialized knowledge and skills that meet the health needs of people in a variety of settings under the direction of qualified health professionals. The Practical Nurse utilizes leadership skills while adhering to ethical and legal standards of the nursing profession. EDUCATIONAL OUTCOME This program is designed to prepare students to demonstrate the following program outcomes: 1. Collects, prioritizes, organizes, and records patient information in an accurate and appropriate manner for continuity of patient care. 2. Integrate interpersonal skill concepts and professional behavior standards into the practice of Practical Nursing. The ability to utilize therapeutic communication skills effectively with members of the health care team, patients, and families. 3. Apply the principles of medication administration, utilizing the nursing process to affect a positive and safe outcome. Also, utilize the nursing process while implementing scientific principles of nursing, consistently, to safely provide technical care. 4. Delegate and supervise within LPN scope of practice, unlicensed personnel in the performance of appropriate skills while adhering to facility policies and procedures. 5. Demonstrate a theory based practice when planning, implementing, and evaluating the nursing care of individuals and groups across the lifespan, including end of life care. 6. Utilize critical thinking in a clinical environment, applying the nursing process to meet self care and self care deficits across the life span, including end of life care. A practical nurse should be able to use critical thinking skills to: Assist RN with patient assessment Prioritize patient care among patients Recognize when a patient is in trouble and seek assistance Delegate tasks with scope of practice 7. Effectively teach patients and families self care to attain, maintain optimal level of wellness or to a dignified death in accordance with patient s wishes. 9

16 EQUENCE, PROGREION, WITHDRAWAL, AND READMIION POLICIE AND PROCEDURE COURE EQUENCE IN NURING The Practical Nursing Program consists of 2 emesters and 1 ummer ession. tudents must be eligible to register for ENG 1010 and Math 1141 during the Fall emester, possess an accumulative GPA of 2.0, and successfully pass the entrance examination and background check. Nursing courses and support courses must be completed in a specified sequence. TNA students may be granted credit for PNUR 1300 upon successful completion of written and skills competency exams. tudents that have been state tested for at least one year and have a current TNA certificate with a minimum of one year clinical experience may be eligible articulation and/or bypass credit. Please note that there is a $25.00 administrative fee to apply for the bypass credit. Additional costs for bypass credit will apply. o tudents will be required to complete the Award of Comparable Credit: Assessment of Prior Learning form requesting bypass credit for PNUR 1300 (2 credit hours) or if eligible apply for Credit Award Agreement for Cuyahoga Community College s TNA Program. o The process must also be completed and credit awarded before the student can progress to the next course level. 10

17 Courses used as pre requisites or core courses for the Health career and Nursing programs must have a traditional letter grade. The Pass/No Pass grading option for pre requisites and core courses will not be accepted by the Health Career and Nursing Programs. uccessful completion of a nursing course is contingent upon a C grade or better in the theory portion and a atisfactory evaluation of performance in the nursing skills lab and clinical setting. (ee Grading tandards) An Incomplete grade in any nursing course must be successfully removed and replaced with a C grade or better and approved by the faculty member prior to progressing to the next nursing course. tudents who fail a nursing course must retake the specific course in its entirety (lecture, lab and clinical components) in order to obtain credit for the course. The course must be successfully completed with a C or better grade before the student can process to a subsequent nursing course. (ee Readmission Policy, page 12) Please note that if you are unsuccessful in a nursing or concurrent course and are registered for the next successive course(s) and you do not withdraw or drop, you will be administratively dropped from the next successive courses. A letter grade of C or better is required in the nursing courses: PNUR 1300, PNUR 1321, PNUR 1330, and PNUR ONLY ONE PNUR clinical nursing course (PNUR 1321, 1330, and 1341) may be repeated to improve a letter grade of less than C. hould a student earn a failing grade in a PNUR course, the student must retake the specific course in its entirety (lecture, nursing skills lab, and clinical) in order to obtain credit for the course. The course must be successfully completed with a letter grade of C or better before the student can progress to a subsequent nursing course. tudents may be readmitted on a PACE AVAILABLE BAI ONLY. Upon completion of these requirements, the student will be eligible for readmission ON A PACE AVAILABLE BAI. PROGREION PROCEDURE 1. All health, CPR and ACEMAPP requirements must be completed, and maintained throughout the entire program (please refer to page 29). 2. Must have a completed, reviewed and cleared BCI. 3. A cumulative GPA of 2.0 is required each semester to progress in the Practical Nursing Program. 4. tudents who accept a slot in the nursing program (with a specific starting date for nursing courses) and do not show up for orientation or the first day of class will have to re start the application process after ept. 1 st. 5. A grade of C or better is required for BIO 1050, ENG 1010, MATH 1141, PY 1010, PY 2020, BIO 1100, PNUR 1300, 1321, 1330, and A grade of less than C for a nursing course or concurrent course(s) results in automatic dismissal from the program. Once dismissed from the Practical Nursing Program the student must apply for readmission. 6. Concurrent Courses: Each semester and summer session concurrent course have been assigned and must be taken as scheduled or prior to the designated semester/session as outlined in the PN course sequence. Concurrent courses are to be completed with a grade of C or better; 11

18 students with a grade less than C in a concurrent course cannot continue in the program. If a concurrent course was not taken prior to the designated semester/session then the student must be registered in the course(s) the first day of the term; any student not registered in a concurrent course will be administratively terminated from the program. a. Any withdrawal from a nursing course with a grade of less than C after the deadline for withdrawal will be considered equivalent to a final course grade of less than C for readmission purposes. 10. GPA (Grade Point Average) Policy for tudents with Transfer Credit tudents enrolled at Cuyahoga Community College (CCC) who do not possess a CCC GPA*and are seeking to enter the Practical Nursing Program with transfer credit for support course from another accredited institution** may use that institution s cumulative GPA if it is 2.0 or greater. tudents with a cumulative GPA below 2.0 must complete a minimum of 9 credit hours at CCC and achieve a minimum cumulative GPA of 2.0. The 9 credit hours must come from courses in English, Math or the ciences that are at the 1000 level or above (no developmental courses). For the student that has a degree from other institution and nine or more CCC credit hours, the degree GPA will take precedent over the CCC GPA if that GPA is higher than the CCC GPA. An official transcript must be submitted to CCC s Enrollment Office (Metropolitan, Eastern or Western Campus). * Cumulative GPA 2.0 or greater from a minimum of 9 credits. **chools accepted by Cuyahoga Community College WITHDRAWAL PROCEDURE Withdrawal from the Practical Nursing course may be initiated by a student. Notice of the withdrawal must be in writing on the withdrawal form provided by the Division of Nursing Education Office. tudents officially withdrawing from a course prior to the last day of the first week of the course will have no notation made on their permanent record. Please note readmission policy. The student must complete the withdrawal form in the back of the Practical Nursing tudent Handbook, and have the form signed by the appropriate Faculty Member. The student must also submit a written statement outlining the reasons for withdrawal from any nursing course. 12

19 Upon withdrawal from any component of a clinical nursing course, the student will have withdrawn from all components of that nursing course. Any student who continues to act in the role of a nursing student, after withdrawal from a clinical nursing course, will be dismissed from the nursing program and will become ineligible for readmission to the Practical Nursing Program. **NOTE: In conjunction with completion of the Division of Nursing withdrawal forms, students must also withdraw from the course(s) in accordance with the college withdrawal process. For the college withdrawal policy, regarding non nursing courses, refers to the Academic tudent upport Information in the College Catalog. ADMINITRATIVE WITHDRAWAL A student will be administratively withdrawn from a nursing course if any of the following circumstances exist: 1. A student has not submitted initial and/or annual health and CPR requirements. A student has not submitted completed ACEPAPP requirements when applicable. 2. A student has not completed and/or is not enrolled in ALL concurrent courses. 3. A student is enrolled in a nursing course and has not completed a required co requisite course(s) with a grade of C or higher (as outlined in the program sequence). 4. A student in enrolled in a nursing course and does not maintain enrollment throughout the semester in ALL CO REQUIITE COURE. 5. A student is enrolled in a nursing course and is dropped by the College for non payment of tuition and fees. 6. Faculty may administratively drop a student who exceeds the maximum absences for lecture, nursing skills lab and clinical. READMIION PROCEDURE tudents seeking readmission to the Practical Nursing Program must submit in the written request form Request for Readmission to the Nursing Education Program. Readmission shall be subject to the following requirements and limitations: 1. Any student, who violates the OBN or Tri C s tudent Conduct Code or the PN tudent Conduct Policy and is dismissed from the program due to that violation, will not be readmitted into the program, even if they would otherwise be eligible. 2. tudents who have either failed a clinical nursing course (not including failures based upon the clinical calculation competency examination policy or ACEMAPP) or withdrawn from a clinical nursing course regardless of the reason are eligible to be readmitted one time only. Failures regarding clinical calculations competency examinations shall be governed, and readmission determined, by the Clinical Calculations Competency Examination Procedure. tudents noncompliant with ACEMAPP requirements are eligible for re admittance one time only. 3. In addition, students who have failed a non clinical nursing course or withdrawn from a non clinical nursing course regardless of the reason are eligible for one additional readmission. 4. An exception to # 2 and 3 above: Any student who has been dismissed from the nursing program for any reason, including, but not limited to, unsafe clinical performance, violations of HIPAA or patient confidentiality, ethical violations, violations of the College s tudent Conduct Code, excessive absences/tardiness, and 13

20 violations of the social media policy must petition the Admission and Progression Committee when seeking readmission into the nursing program. This procedure must be followed even if a student has not been readmitted previously. In these cases, readmission may or may not be granted by the Admission and Progression Committee. The Committee s decision is final. 5. tudents who fail a course or withdraw due to failing are advised to seek assistance through the Academic Counseling Center. tudents are also encouraged to seek assistance through tudent upport ervices. 6. tudents seeking readmission must have a minimum of 2.0 cumulative GPA at Cuyahoga Community College, inclusive of all nursing courses. 7. ubject to the limitations set forth below, students who have been recommended for a Withdrawal Exception by the tudent Affairs department (reserved for severe extenuating circumstances) shall be subject to the readmission limitations of paragraph tudents requesting an exception to paragraph 2 must submit to the Nursing Education Division Admission and Progression Committee, a letter of petition that includes a written explanation for the request and ALL supporting documentation of extenuating circumstances to substantiate the petition. Readmission to the nursing program for a second time shall be approved only upon demonstration of extenuating circumstances beyond the reasonable control of the student which may include emergency or unanticipated medical condition of the student or a member of the student s immediate family; military service in which the student is called to active duty; court orders impacting the student where the student is not a party to the court action, or other extenuating circumstances beyond the reasonable control of the student which the Committee, upon consideration of the petition and supporting documentation, determine warrants readmission of the student. DOCUMENTATION UBMITTED AFTER THE ORIGINAL LETTER OF PETITION I UBMITTED WILL NOT BE CONIDERED. The decision of the Division of Nursing Education Admission & Progression Committee to approve or deny a request for readmission is final. 9. tudents who have not attended Cuyahoga Community College s nursing education program for two academic years must seek admission to the nursing education program as a new student. 10. tudents who were dismissed from the College for academic reasons must petition their Campus Appeals Board for readmission three weeks prior to registration. These students must also see a counselor or faculty advisor to establish goals and objectives, and they must complete an Application for Admission form and a Campus Appeals Board form before being considered for readmission. Readmission to the College does not mean readmission to the nursing education program. 11. All readmissions, regardless of the reason, shall be on a space available basis. PLACEMENT ON READMIION LIT tudents who withdraw from a nursing course and are eligible for readmission to the nursing program must submit the appropriate forms to the Division of Nursing. All readmissions will be based on the availability of openings. tudents who withdraw from a nursing course prior to the official college withdrawal date, need to submit the following completed forms (form in appendix). 21 Placement onto the readmission list for all students withdraw prior to the withdrawal date will be placed in the order in which the completed forms have been received in the Nursing Office. tudents who withdraw from a nursing course after the withdrawal date will be required to submit a request for readmission. Placement onto the existing readmission list will be based upon a random selection through a lottery process of all readmission request forms by the deadline date and time selected for the semester. Any student submitting a Division of Nursing Request for Readmission Form after the deadline date and time will be placed on the readmit list following those students who were part of the lottery process in the order they are received. *Note: In order to be placed on the readmission list, all three forms (withdrawal forms two pages and readmission form one page) must be submitted at the same time. 15

22 CLAROOM, KILL LAB AND CLINICAL POLICIE AND PROCEDURE GRADING TANDARD The grading scale for the Practical Nursing Program is as follows: 91% to 100% = A = 4.0 quality points 84% to below 91% = B = 3.0 quality points 75% to below 84% = C = 2.0 quality points Below 75% = F = 0.0 quality points Withdrawal = W Incomplete = I The final theory grade is computed by totaling the number of points earned on a unit and final exams and determining the corresponding percentage. To pass, a student must achieve at least 75%. cores will not be rounded up to the next whole number. There will be no retakes of examinations. Extra credit points will not be awarded. If a atisfactory evaluation is earned in the college or clinical laboratory, but an F grade is earned in theory, the grade for the course is an F. If a C grade or better is achieved in theory, but an Unsatisfactory evaluation is earned in the nursing skills or clinical laboratory, the grade for the course is an F. Unsatisfactory clinical performance that jeopardizes client safety will result in student s dismissal from the clinical areas by the instructor with a grade of F for the course. An incomplete grade in any nursing course must be successfully removed with a C grade or better and approved by the faculty member prior to progressing to the next nursing course. In order to remain in, progress through, and complete the Practical Nursing Program, the student must have a C grade or better in all nursing courses. A 2.0 GPA ( C ) cumulative average must be maintained in all other courses. tudents who fail a nursing course must retake the specific course in its entirety (lecture, lab and clinical components) in order to obtain credit for the course. The course must be successfully completed with a C or better grade before the student can progress to a subsequent nursing course. (ee Readmission Procedure). TUDENT NOTIFICATION OF PERFORMANCE Faculty will appraise students of their performance in lecture, lab, and clinical. Methods of notification include the use of the tudent Progress Appraisal Form, anecdotal notes, and a formative (midterm) and final clinical evaluation. When faculty determine the need for additional support, students can be referred for special tutoring and/or remediation in the nursing skills laboratory. 16

23 ATTENDANCE REQUIREMENT Attendance in all classes, nursing skills laboratories and clinicals are required. ince theory, laboratory practice and actual clinical experience are vital to success in nursing, unexcused absences and lateness are unacceptable in all instances. IT I THE TUDENT REPONIBILITY TO NOTIFY HI/HER INTRUCTOR (ONE HOUR BEFORE CLA, NURING KILL LAB, OR CLINICAL) OF AN ABENCE. If a student does not notify his/her instructor within the minimum of one hour prior time frame, it will be considered a no call, no show, resulting in the student s dismissal from the program. ince attendance is mandatory, in the event of an unavoidable absence, the following will apply: 1. Absence equal to or greater than 10% or unmet objectives will result in failure. 2. Upon an absence, a mandatory conference regarding the reason for the absence must be completed within one week. The conference must be documented on the clinical absence form. Copies will go to the student s file, clinical faculty, course coordinator and the student. (Form in appendix). TET Attendance for tests (examinations) is mandatory. Examinations taken will not be repeated. The student is responsible for completing make up examination(s). If this procedure is not followed, an F grade may be earned for the missed tests. tudents are held accountable for adherence to the College conduct code during test taking. Disciplinary action for academic dishonesty (cheating and/or breach of confidentiality) may result in a failing grade for the course. tandardized tests may be required throughout the nursing program as course requirements. 1. Five percent of total points in the lecture component of class may come from pop quizzes and/or activities administered by faculty at any point during the lecture day. If a student is not present for a pop quiz or activity, no matter what reason, the student will not have an opportunity to take a make up quiz or participate in a make up activity for those points. 2. On exam days, the door to the classroom will be closed at the start of the exam. Once the door is closed, the door is considered locked and a student may not enter the classroom until after the exam has been completed, no matter what reason, the student was late. If a student misses taking an exam on a scheduled test date, s/he will take a make up exam at the end of the semester on a single date, time, and place to be determined by the faculty of that course. If a student misses taking more than one exam throughout the semester, s/he will take all of their make up exams on that single established date, time, and place. If a student misses taking their make up exam(s) on the established date, the student will receive a score of zero points for the make up exam(s). If a student is no call/no show to a make up exam, they will not be given another opportunity to take the exam. 3. Any student who fails to contact their faculty member via telephone or e mail, one hour before the scheduled exam, will be considered no call/no show and will not be permitted to take the makeup exam on the end of the semester date referenced above, receiving zero points for that exam. During test taking, NO electronic equipment (cell phones and tape recorders including ear devices, etc.) may be used, and may not be kept on the desk top. Upon completion of the Practical Nursing Program, the student is required to take the LPN Assessment Test. tudents are required to pay for this test. 17

24 GRADE DIPUTE POLICY Any student with a grievance about a course grade has the option to pursue a grade dispute. The student should first discuss the dispute with the faculty member/course instructor. tudents who wish to formalize the dispute should follow the Grade Dispute Policy and Procedure in Grade Dispute in the CCC tudent Handbook. CLINICAL CALCULATION COMPETENCY EXAMINATION PROCEDURE Clinical calculation competency is defined as the student s ability to correctly perform clinical computations on material previously taught. The following nursing courses will administer 3 consecutive clinical calculations competency examinations. PNUR 1321, PNUR 1330 and PNUR Clinical Calculation Competency examination grade will NOT be calculated as part of your overall course grade. BUT, a Practical Nursing student who fails the clinical calculation competency examination three (3) times in any Practical Nursing course will earn a grade of F in the current nursing course. The guidelines for the clinical calculations competency examination are as follows: 1. The first two clinical calculation competency examination will be comprised of 10 questions. The third will have 5 multi step calculation questions. 2. No multiple choice; that is, the student will be expected to show all work indicating how an answer was derived. 3. Tests are to be administered on appropriate campus and will not be take home, or open book. 4. tudents may use a non programmable basic calculator for computation in the following activities: clinical calculations competency examinations, unit tests, laboratory test outs and final examinations. The term non programmable basic calculator shall mean limited to the following processes: addition, subtraction, multiplication, division and percentages. A student will be prohibited from using a calculator with other capabilities. 5. Correct test item responses (answers) must include, in writing, all sequential steps to problemsolving and mathematical formulas. No credit will be given for answers that omit problem solving steps and formulas. 6. A score of 9 correct answers out of 10 possible correct answers will be necessary to pass the mathematics examination. 7. Any student who does not pass the first (1 st ) and/or second (2 nd ) clinical calculation competency examination, in any semester must meet with a Practical Nursing Program Faculty Member regarding a written remediation plan. 8. If unsuccessful in attaining a passing grade, a student is permitted to take two (2) additional mathematics competency tests. All three (3) mathematics competency examinations must be completed per the time schedule in the course syllabus. 9. A maximum of three (3) examinations may be administered per course; if upon completion of the third (3 rd ) examination the student is not successful, the student will be withdrawn and will have earned a failing grade for the course. 10. One time, during his/her entire tenure in the Practical Nursing Program, a student may fail 3 consecutive mathematics competency examinations; and, will be considered for readmission to the Practical Nursing Program under the same readmission criteria applied to all other students. This one readmission opportunity is based solely on mathematics competency failures and is not subject to Criterion #1 of the Readmission Policy. 11. tudents who are not present at the date and time of a scheduled math competency test will receive a score of 0 for that math competency test. 18
