1 LPN to ADN FLEX OPTION Application Requirements for Spring 1. Attend a Health Information Program (HIP) Session to obtain admission information and application materials for the application year. If you prefer, you can attend an online HIP Session by going to On the front page, under Admissions, click on Nursing & Health Sciences and then Health Information Sessions. Click on Online at the bottom of the page. 2. OCtech students or those transferring from other colleges must possess a 3.0 cumulative GPA from the last college where 12 or more credit hours (does not include developmental/remedial courses) were attempted. Prior to acceptance into the PN-ADN Flex Option, applicants must have completed ALL of the following required general education courses with a grade of C or better prior to acceptance into the PN-ADN Flex Option. (MAT 155 or higher, BIO 210, BIO 211, BIO 225, PSY 201, PSY 203, ENG 101, and a humanities course). NOTE: AHS 210 (offered online), must be competed with a C or better in the fall when accepted into the program. Applicants must be a high school graduate or possess a GED. (Certificate of completion does not qualify). 3. Meet one of the following test criteria: 1) TEAS V, composite (average of math & reading) score range between with a minimum Reading score of 65. Students are allowed to take the TEAS test a maximum of two times for the year in which they are applying. There is a minimum waiting time requirement of 15 days between each TEAS test attempt. 2) SAT scores of 480 verbal and 480 Math or ACT Reading of 19 and Math of 19 may be used in lieu of TEAS V test requirements. 3) Completion of a Bachelor s Degree or higher from a regionally or nationally accredited institution with a cumulative GPA of 2.75 or higher may be used in lieu of the TEAS V test requirements. 4. Once testing and GPA criteria meet minimum standards, applicants can submit a completed application packet that must include the following in order to be considered for admission: 1) Application for Nursing/Health Science Program; 2) State County of Residency; 3) TEAS V/SAT/ACT scores; 4) Official high school transcript or GED transcript indicating proof of graduation. 5) Official transcripts from ALL colleges attended. All college transcripts must be complete and current. 6) Health Sciences Application Packet Checklist with Declaration of U.S. Citizenship attached; 7) Copy of your driver s license. 5. Applicants who have had two unsuccessful (failed) enrollments in any RN Program will not be eligible to apply for admission. (See an admission counselor for further information). Applicants must have graduated from a program approved by the Accreditation Commission for Education in Nursing, (formerly the National League of Nursing and Accrediting Commission). Program accreditation may be verified at If program is not accredited, special testing may be required (see program coordinator). 7. Applicants must be a U.S. citizen or lawful permanent resident for at least one year prior to the date of application. 8. Admission into the LPN to ADN FLEX OPTION is a one-time-only opportunity. If unsuccessful on the first attempt, applicants must apply for the Associate Degree Nursing Program. *Qualified applicants who attended college outside of the United States and who meet citizenship requirements must provide an official translation transcript(s) from World Education Service. Fees for English translation of international transcripts must be paid by the applicant. (See Registrar for details). Misrepresentation of information on any required forms or a willful attempt to misrepresent residency will make an applicant ineligible for admission into either the LPN to ADN FLEX OPTION or into the Associate Degree Nursing curriculum. 2 LPN to ADN FLEX OPTION Spring Additional Requirements Prior to acceptance into the program, applicant must provide the following information to the Program Coordinator: 1. Copy of active unencumbered PN license ( or be a graduate of an accredited nursing program ( within the last 6 months and have a license by the spring semester start date. 2. Submit letter of recommendation from employer or program coordinator of previous nursing program. Also, applicant must demonstrate prior knowledge and expertise in the following: 1. Dosage Calculation Test with a score of 80% or higher. 2. Successful demonstration of selected nursing skills: i.e. Foley Catheter Insertion, Injections, etc. 3. Take the NLN Foundation of Nursing Exam with a score of 60% or higher; unless the applicant has passed the NCLEX in the last 6 months. Applicant will be notified of date and time for testing by Program Coordinator. Admitted students who are not able to perform ALL Core Performance Standards and the Additional Requirements will have the admission offer for spring rescinded. July 7, 2014

3 Core Performance Standards for the Didactic and Clinical Environment LPN- ADN FLEX OPTION APPLICANTS WILL BE NOTIFIED OF THE CORE PERFORMANCE STANDARDS FOR THE CLASSROOM AND CLINICAL ENVIRONMENT REQUIRED FOR THIS OCCUPATION. Applicants who need to be evaluated for reasonable accommodations should make an appointment with the Coordinator for Services for Students with Disabilities, Tracy Dibble, by calling between 8:30 AM and 3:00PM, Monday thru Thursday. Purpose: To communicate the physical demands and communicational skills required for Nursing Students. Issues Examples Critical thinking Identification of cause/effect relationships in clinical situations Use of the scientific method in the development of patient care plans Evaluation of the effectiveness of nursing interventions Professional Relationships Establishment of rapport with patients/clients and colleagues Capacity to engage in successful conflict resolution Peer accountability. Communication Mobility Explanation of treatment procedures, initiation of health teaching. Documentation and interpretation of nursing actions and patient/client responses Movement about patient's room, work spaces and treatment areas Administration of rescue procedures-cardiopulmonary resuscitation Motor skills Calibration and use of equipment Therapeutic positioning of patients Hearing Ability to hear monitoring device alarm and other emergency signals Ability to discern auscultator sounds and cries for help Visual Tactile Sense Ability to observe patient's condition and responses to treatments Ability to palpitate in physical examinations and various therapeutic interventions

4 LPN to ADN FLEX OPTION ADMISSION OVERVIEW Admission of applicants is competitive. Applicants seeking admission to the spring LPN to ADN FLEX OPTION must meet the admission requirements as outlined in this handout. Qualified applications are reviewed on a rolling admission basis and will be accepted throughout the year until the class is filled. Applicants are strongly encouraged to apply early. Applicants who are permanent residents of Orangeburg or Calhoun counties and established students at Orangeburg-Calhoun Technical College are given priority consideration. However, we do accept students from outside our service area. Applicants must be U.S. citizens or lawful permanent residents. ALL interested applicants must attend a Health Information Program Session to obtain an application for admission, which will be submitted to the college Admissions staff. All documents that are submitted with the application packet, including but not limited to official transcripts and test scores, become the exclusive property of the college and will not be returned to the applicant. A national background check and drug test will be required upon acceptance to the program and before starting class in the spring. Positive results on background check and/or drug test may prohibit participation in clinical experiences and may necessitate removal from the program. Note: Prior criminal convictions may affect eligibility to take the State Board Examination for Licensure. The LPN- ADN Flex Option is awaiting approval from the Accreditation for Education in Nursing (ACEN).
