4 How the Fundraising Program Works Students may sell TaxSlayer Bowl tickets at a discounted rate as a fundraiser for their school or organization. For each $25 ticket sold (regularly $60), $10 is retained by the participating school Funds raised are unrestricted and available to the school immediately Each organization is responsible for the sale and collection of all monies and distribution of tickets Tickets sold by each participating school will be issued together in an individual block This special offer expires December 1, 2015 (prior to the announcement of the participating teams) Sales are not restricted to members of the school community. Anyone is eligible to purchase! At the conclusion of the fundraiser, each organization shall submit one single payment of $15 per ticket sold to the TaxSlayer Bowl along with a final weekly update. To participate in the program, submit the Registration Form and Waiver to the TaxSlayer Bowl via mail or fax. TaxSlayer Bowl Attn: School Fundraising Program One Gator Bowl Blvd. Jacksonville, FL Fax: (904) How honor roll students can earn more A/B * honor roll students may receive a FREE ticket to the 2016 TaxSlayer Bowl when one parent / legal guardian purchases a $25 school program ticket to join them at the game. *Primary Grades equivalent: All S s All that is required for your honor roll students to receive their FREE ticket is a list of all eligible students names and proof that a parent / legal guardian purchased a ticket with the Program Coordinator s and Principal s signature, (see page 8). Your school s Honor Roll Ticket Form must be submitted no later than 5:00 pm on December 1, 2015 to ensure seating. Parent tickets and any additional tickets are available for $25 ($10 retained by organization). Tickets sold by each participating school will be issued together in an individual block 4

6 WAIVER PLEASE NOTE: This form must be signed and returned to the TaxSlayer Bowl office. Once the TaxSlayer Bowl office receives the completed Registration Form (page 5) and this Signed Waiver, your school is officially registered to participate in this ticket program! Please send via fax or mail to: TaxSlayer Bowl Attn: School Fundraising Program One Gator Bowl Blvd. Jacksonville, FL Fax: (904) Please review the following and complete the portion below: Only 5,000 seats have been reserved for this exclusive program. To ensure that the appropriate number of tickets is reserved for my school, I will submit a ticket sales report each week from my program start date throughout the duration of our school s sales program regardless of the quantity of ticket sales. I will submit payment ($15 per ticket sold) to the TaxSlayer Bowl office by the deadline, 5:00 pm on December 1, I understand that the TaxSlayer Bowl will not fulfill ticket orders if payment is received past the deadline. I understand that any organization participating in the TaxSlayer Bowl School Fundraising Program is prohibited from selling tickets to brokers. The TaxSlayer Bowl reserves the right at their sole discretion to request the actual ticket order forms for review and/or rejection of any ticket sales from an organization that they believe has participated in the sale of tickets to brokers and any ticket sales outlets. I understand that personal checks will NOT be accepted by the TaxSlayer Bowl. I, (First and Last name), have read and agree to adhere to these responsibilities as the Program Coordinator at (School). 7 Reporting Your Progress Each organization is required to submit the Weekly Update Form (page 9) each week throughout the program s duration to ensure the appropriate number of tickets is reserved for your school. At the conclusion of the fundraiser, each organization shall submit one single payment of $15 per ticket sold to the TaxSlayer Bowl along with a final ticket sales update. A check must be made out from the school or organization to the TaxSlayer Bowl. NO PERSONAL CHECKS WILL BE ACCEPTED. Don t Forget: Honor Roll Students get a FREE TICKET when a parent purchases a companion ticket. Verify and request authorization of Honor Roll eligibility from school principal using the Honor Roll Ticket Form (page 8). Forms and payments must be submitted to the TaxSlayer Bowl offices no later than December 1 st, $10 from each ticket sold will be retained by the school organization. Game tickets will be available for pick-up at the TaxSlayer Bowl office on December 3, 2015 and December 4, 2015 between 8:30 am and 5:00 pm. 7

10 SEAT LOCATION MAP Seat location will be prioritized by the following: 1. Schools who submit regular weekly reports who have previously participated in the program. 2. Schools who submit regular weekly reports who have not previously participated in the program. 3. Schools who submit irregular weekly reports 4. Schools who submit no weekly reports 10

11 MASCOT REQUEST FORM PATCHES is the official mascot of the TaxSlayer Bowl. Request PATCHES to come visit your school at least 2 weeks in advance, so that the TaxSlayer Bowl has adequate time to schedule PATCHES for your visit. Mascot visits typically last between minutes. Although PATCHES cannot speak, he will be accompanied by a member of the TaxSlayer Bowl staff who will be happy to discuss the program and answer any questions. Please be sure to specify the type of event PATCHES will be attending. (IE. Fall festival, classroom visits, PTA meeting, etc.) Mascot appearances are limited to one visit per school per year. All mascot visits are subject to PATCHES availability To request a mascot visit at your school, simply complete the Mascot Request Form. A member of the TaxSlayer Bowl Staff will contact you to discuss logistics of the visit. 12 FREQUENTLY ASKED QUESTIONS HOW DOES THE WORK? Students can sell 2016 TaxSlayer Bowl tickets to family, friends, and the community for the discounted rate of $25. For each $25 ticket sold, the school or organization keeps $10. a) Unlike other fundraisers, the $10 you earn with each TaxSlayer Bowl ticket sold is immediately available for your use. b) Unlike other funding sources, the $10 you earn with each TaxSlayer Bowl ticket sold is unrestricted to be used at your discretion. c) The savings are a significant value to your supporters. TaxSlayer Bowl tickets typically sold at $60 are sold exclusively by local schools for $25. WHAT DO YOU MEAN BY IMMEDIATELY AVAILABLE? Since you are responsible for the collection of all monies, you may begin using it as soon as you receive it. At the conclusion of your sales program, you submit only the TaxSlayer Bowl s portion in one single payment of $15 per ticket sold. The TaxSlayer Bowl will never handle your school s fundraising monies. WHAT DO YOU MEAN BY UNRESTRICTED? The participating school clubs/organizations who utilize the fundraising element will determine how, when, where, why and on what the money will be spent with no questions asked by the TaxSlayer Bowl. The funds are not affected by, nor do they affect other funding sources. HOW DOES THE HONOR ROLL TICKET WORK? A/B * honor roll students may receive a free ticket if at least one parent / legal guardian purchases a $25 honor roll program ticket to join them at the TaxSlayer Bowl, January 2 nd. The participating school/school organizations keep $10 for each $25 TaxSlayer Bowl ticket sold (Regularly $60). It s that simple! HOW LONG IS THE AVAILABLE? Registration to participate in the school fundraising program ends on October 16, This special ticket offer expires on December 1, Each participating school club / organization will determine the timing and length of the program s sales within the allotted time frame (Now December 1, 2015). IS THERE A CERTAIN NUMBER OF TICKETS THAT MY GROUP MUST SELL? No. You may sell any number of tickets. No matter the number sold, you keep $10 per ticket to spend on your project. Only 5,000 tickets have been allocated to this program when that number has been reached, the TaxSlayer Bowl will cease ticket sales. 12

13 FREQUENTLY ASKED QUESTIONS HOW DO I HANDLE HANDICAPPED SEATING NEEDS? To purchase a ticket in the handicapped seating area, the ticket purchaser should mark the Handicapped Seating option on the Ticket Order Form. You, (the Program Coordinator), are their contact to reserve handicapped seats. (Please write your name and phone number on the Ticket Order Form) Please note: In order to accommodate all handicapped persons, the TaxSlayer Bowl asks that each handicapped person be limited to purchasing one additional companion seat in the handicapped section. When you remit your final ticket order form to the TaxSlayer Bowl office on December 1st, please include a list of all handicapped seating needs. HOW DO I PARTICIPATE IN THE? To participate in the program, each organization is asked to register with the TaxSlayer Bowl by completing the enclosed Registration Form and Waiver. Registration Form and Waiver must be submitted to the TaxSlayer Bowl by October 16, If you have any questions, feel free to call the TaxSlayer Bowl at (904) WHEN IS THE $15 PER TICKET DUE TO THE TAXSLAYER BOWL? The $15 portion collected per ticket must be remitted by 5:00 pm, Friday, December 1, 2015, at the TaxSlayer Bowl office, located on the East Side of EverBank Field at One Gator Bowl Blvd., Jacksonville, FL No personal checks will be accepted. HOW ARE THE TICKETS DISTRIBUTED? Each participating school club / organization will be responsible for the distribution of game tickets to their customers. Game tickets will be available for pick-up at the TaxSlayer Bowl office located at EverBank Field on December 3, 2015 and December 4, 2015 between 8:30 am and 5:00 pm. WHEN AND WHERE IS THE TAXSLAYER BOWL? January 2, 2016 EverBank Field WHO IS PLAYING IN THE GAME? Teams will not be announced until after the first weekend in December. The TaxSlayer Bowl receives a selection from the Big Ten Conference or the Atlantic Coast Conference who will face off against a representative from the Southeastern Conference WHERE ARE THE SEATS LOCATED? Exact seat location will be determined after December 1, (See map for School Program sections). WILL ALL OF OUR SEATS BE TOGETHER? Yes! Tickets sold by each participating group will be issued together in an individual block as long as you send in your weekly update form from your fundraising start date every week throughout the fundraiser s duration. Handicapped seating may not be with the rest of the group. CAN ONLY STUDENTS/PARENTS BUY TICKETS? No! Tickets through your program can be purchased by anybody who wants to help your cause or sit in your school s block. 13
