Collect curriculum data, document progress and continue to provide technical assistance (through curriculum action team). Continue outreach to existing and developing clinical education programs. OBJECTIVE: Create and convene an oral health education action team of state champions to share ideas and best practices and act as a resource to health institutions by January 30, Created action team of students and faculty from different schools to champion oral health and drive curriculum integration and interprofessional (IP) learning at their academic institutions. Continue to support students and faculty and initiate outreach to other programs. OBJECTIVE: Partner with at least one school from each discipline to plan four IP educational opportunities annually that include dental professionals by June 30, Medical Schools PA Programs Nursing Schools Pharmacy VCU Shenandoah University VCU VCU VT/Carilion Jefferson Shenandoah University EVMS EVMS Hampton University VCOM JMU Appalachian College UVA *Note: Schools listed in green are those that currently provide IP opportunities that include dental students and/or providers, either through curriculum or a specific event/activity.

Created action team of students and faculty from different schools to lead development of opportunities for IP learning that include dental students and faculty at their academic institutions. Formed partnership with VCU Inter-Health Professionals Alliance (IHPA), a student lead program focused on service to the community and the delivery of interdisciplinary team-based care. Collect data, document progress and continue to provide technical assistance as needed (through action team). Continue to facilitate new partnerships between academic institutions. Continue to work with IHPA student group to engage new students and share information about student-lead IP programs and activities.

3 EDUCATION AND TRAINING: EXISTING PROVIDERS OBJECTIVE: At least eight of the major state health professional associations will promote the Smiles for Life oral health curriculum through their websites, association literature and as part of their regular meetings and conferences by June 30, (See table below for list of identified associations) Developed Smiles for Life materials and continuing education (CE) resources for provider associations. Initiated dissemination of materials to association contacts. Continue to disseminate materials to associations (webcopy, newsletter templates, articles). Collect data through Smiles for Life website. OBJECTIVE: One professional association from each discipline (medicine, nursing, pharmacy and physician assistant training) will provide one or more interactive (in-person) continuing education opportunity focused on oral health each year by June 30, OBJECTIVE: Partner with professional associations to provide four interprofessional (IP) networking and educational opportunities that focus on interdisciplinary approaches to oral health care by June 30, Medicine Nursing Pharmacy PA Dental Medical Society of Virginia Virginia Academy of Family Physicians (July 2013) American Academy of Pediatrics, Virginia Chapter Virginia Nurses Virginia Council of Nurse Practitioners (March 2013) NAPNAP, Virginia Virginia Pharmacists Virginia Academy of Physician Assistants (July 2013) Virginia Dental Virginia Dental Hygienists *Note: s listed in green are those scheduled to provide in-person CE focused on oral health. Collect data and provider feedback from events. Continue to work with association contacts to provide oral health CE and IP opportunities. Work with contacts to schedule in-person IP gatherings/sessions. Expand outreach to additional provider associations and local/regional professional groups.

4 OBJECTIVE: Create Virginia specific quality improvement training program for primary care providers (pediatricians) to incorporate oral health prevention services into practice that meets Maintenance of Certification (MOC) criteria by January Received planning grant from AAP and identified champion to lead project. Developed and disseminated survey to pediatricians via Virginia AAP listserv. Met with pediatric practices to learn about how oral health is incorporated into practice. Continue to collect and synthesize survey and in-person interview data. Continue outreach to pediatric practices regarding oral health prevention in primary care. Create training template and develop implementation plan.

5 PRACTICE ENVIRONMENT: REIMBURSEMENTS & INCENTIVES OBJECTIVE: Develop recommendations and initiate advocacy activities for proposed legislation to include reimbursement for referral as part of the Bright Smiles for Babies program by June 30, OBJECTIVE: Partner with private insurer to pilot program that reimburses/incentivizes medical providers for prevention services and referral to a dental home by age one by June 30, Convened early dental home action team to focus on implementing strategies to increase the number of children receiving oral health prevention services and that see a dentist by age one. Collected data on financing models for preventive oral health services for young children in other states. Initiated outreach to one private insurer to participate in pilot/demo project. Work with early dental home action team members and provider champions to carry out strategies. Continue to collect data and develop recommendations for referral strategies. Continue to collect data on prevention base oral health models and create case statement(s). Continue to work with providers and insurance carrier(s) on pilot/demo project. PRACTICE ENVIRONMENT: INTEGRATED CARE MODELS OBJECTIVE: Partner with healthcare organizations and other community groups to share information and resources and provide technical assistance to primary care facilities (i.e. community health centers, free clinics, etc.) incorporating oral health into the patient centered healthcare models by June 30, Scheduled webinar on managing oral health for patients with chronic disease (expected date September 2013). Work with partners to convene stakeholder group to set direction, outline strategies and identify opportunities to support integration of oral health into patient centered care models. Continue to work with individuals and organizations to carry out specific activities and support stakeholder group as necessary. OBJECTIVE: Partner with ConnectVirginia to educate 75% of licensed practicing dental providers who accept Medicaid about the use of direct messaging* to share patient health information by June 30, 2014.

6 OBJECTIVE: Partner with the Virginia Health Information Technology (VHIT) Regional Extension Center to educate 75% of licensed practicing dental providers who accept Medicaid about meaningful use of electronic health records by June 30, Met with Virginia Health Information Regional Extension center (VHIT) representative to discuss strategies to partner to educate providers about meaningful use and technology that facilitates collaboration among the professions. Initiate outreach to ConnectVirginia to develop communication plan. Continue to work with VHIT to support provider education and outreach.
