2 Access Partnership A nonprofit community collaboration comprised of individuals and organizations dedicated to improving access to health care Established to mobilize efforts and eliminate major gaps in care for Greater Hampton Roads' uninsured and underinsured residents Focused on strengthening existing programs & implementing new services for identified gaps Current collaborative membership includes ~1000 individuals representing >300 organizations 2

4 How Collaboration Began Formed as a direct result of the Norfolk Turning Point planning initiative, part of a national effort to change and strengthen the ways in which states and local communities protect and improve public health (1999). Access to health care for financially disadvantaged uninsured adults was recognized as a major concern. Key stakeholders began collaborating to address access issues Sentara Health Foundation s Community Health Assessment confirmed access to health services (2004) October 2004 Access Partnership was launched 4

13 ODU Dental Voucher Program Individuals without dental insurance can receive dental cleaning at ODU School of Hygiene for $15 Program coordinated by Access Partnership 13

14 Rotary Smiles Program January 2014 Provides dentures (complete and partial) to 100 eligible adults 14
