This online tool can be used by school administrators/health personnel to submit their annual reports summarizing the immunization status of all enrolled students. The School Immunization Law (Minnesota Statutes, Section 121A.15) requires school administrators to submit their AISR each year to the Minnesota Department of Health by December 1. 3 After navigating to the login screen please enter your user name ( address) and password which was supplied in an you should have received. If you did not complete the AISR report last year or have a new address you will not have received this . You will need to register as a new user by clicking on the New User Registration button on the login page. If you have misplaced this please use the Forgot Password button to receive a new password. Once you are registered you will receive an confirmation with a password, use this to log in. Steps to Complete the AISR Step 1: Verify Contact Information Click on the Browse Schools link to see a list of all schools in your district. Select your school from the school list. o If you do not see your school please health.aisr@state.mn.us or call Lisa Harris at (metro) or (greater MN). Please review and update your contact information by clicking on the Edit School link. o Note: You will not be able to save your data unless all of the fields are filled in on the school contact information section. 4 Step 2: Select Grades After clicking on the Add Report link you will be directed to a page where you select the grades that you currently have enrollment for. Select each grade that you have enrollment for by clicking on the corresponding check box. Double check to make sure you have selected the correct grades before clicking. You will not be able to go back and edit grades after you have pressed Step 3: Complete Enrollment Information Enter enrollment numbers for each grade in the Diphtheria, Tetanus and Pertussis (DTaP) section. o Enrollment numbers will auto fill for all of the remaining vaccine sections after they are entered into the DTaP section. o If you need to change the enrollment numbers you will have to modify the enrollment in the Diphtheria, Tetanus, and Pertussis (DTaP) section and then click Enrollment numbers should include the total number of students for each grade at the time of reporting. After saving the enrollment data the rows in the vaccine tables will be highlighted purple. This indicates that the row still needs to be completed, is missing data, or there is an error in the row. See the example on page 5. o Your data will still be saved if the row is highlighted purple. o The AISR application is a real time system. Once your data has been saved MDH will be able to view any data that you have entered, and you are able to return and update your data at anytime. Step 4: Complete the Vaccine Tables Fill out the tables for each specific vaccine. You can refer to the Are Your Kids Ready? document to see which vaccines are required for enrollment into school and when they are required: Each student should be accounted for in this report. Students that have all required doses of a vaccine should be counted in the All Req. Doses column for that vaccine. Students that are missing any or all required doses, and DO NOT have an exemption on file for the vaccine, should be counted in the In Progress/Missing Doses column for the given vaccine. Students that have the proper documentation showing that they have a history of varicella should be counted in the History of Disease column. Students that have a legal conscientious objection exemption on file for a vaccine should be counted in the CO column for that particular vaccine. Students that have a legal medical exemption on file for a vaccine should be counted in the ME column for that particular vaccine. o Note: Include students in the CO column even if they started a series but didn t finish it because they filed an exemption. 5 NOTE: Please save your data regularly. You can come back and update your information after it has been saved. Example Refer to the example below. The sum of All Required Doses + InProgress/Missing Doses + CO + ME must be equal to the enrollment for each grade. If you have correctly entered the number of children for each grade the row will turn white after the button is clicked. Once you have pressed your data will be available to MDH. Note: The rows that remain highlighted purple after saving have an error or are missing data. The total enrollment is 50, but there are 57 students entered into the columns. The entry boxes are highlighted purple because there is a data entry error. Data are missing for fifth graders NOTE: Please save your data regularly. 6 Step 5: Complete the Exempt to All Tables Note: The exempt to all totals only need to be completed for Kindergarten and seventh graders. The number of Kindergarteners and seventh graders that have a legal exemption for either conscientious objection or medical exemption for every single required vaccine should be recorded in the Exempt to All table under the CO and ME columns respectively. Your report is complete! There is not a formal submit to MDH button (as in the past). Anytime you click on the save button MDH can see your data. Remember you can go back and edit your report as needed.
