9 School Required Vaccines K through 12 th grades
10 Vaccines for Kindergarten Entry - 3 Hep B - 5 DTaP (unless 4 th dose is given on or after the 4 th birthday) 2013: 4 month interval between dose 3 and 4 is acceptable (previously a 6 month interval was required) - 4 Polio (unless 3 rd dose given on or after 4 th birthday) - 2 MMR ( if single antigen given, 2013: 1 dose Rubella acceptable) - 2 Varicella (The intervals between doses can be found in the BOH rules under the Table 1 footnotes. Varicella doses found in Table 2)
11 Vaccines 6 th thru 12 th grades Hep B 3 doses (2 dose series for yr olds) * Tdap - If a student, 7 yrs of age or older, did not receive a full DTaP series they must have evidence of 3 appropriately spaced tetanus/diphtheria containing vaccines (DTaP, DT, Td, Tdap). 2013: The administration of Tdap to complete this 3 dose series, will meet the Tdap requirement for 6 th through 12 th grades.
12 Vaccines 6 th thru 12 th grades 4 doses Polio (A fourth dose is not necessary if the third dose was administered at age 4 years or older and at least 6 months after the previous dose). Note: Students who were series complete before 7/1/09, have met the polio requirement. 2 doses MMR (or if single antigen given,1 dose of Rubella meets the requirement). 2 Varicella K through 7 th grade 1 Varicella - 8 th through 12 th grades
13 Vaccines 6 th thru 12 th Grades 1 dose Tdap required for students yrs of age or at 6 th grade entry. There is no interval between Td and any tetanus & diphtheria containing vaccine (including DTaP) ACIP Recommended Immunization Schedule for persons aged 0 through 18 years 2013 Tdap can be administered regardless of the interval since the last tetanus and diphtheria toxoidcontaining vaccine

16 Certificate of Immunization All students must have a CI in their school record. A student is in compliance if they have received all required immunizations for their age/grade.

19 In Process Within 14 days after receiving direct personal notification that the certificate of Immunization is not up to date, the student is to provide documentation that the next required immunization(s) has been given and/or a plan to receive the required immunization(s) will occur.
20 School Required Immunizations: Some Basic Rules No record, no attendance In Process: The parent has 14 days after notification to receive the shot(s) or to make a plan to receive the shots(s) Exemptions (medical, personal, religious) (Must have a HCP or parent signature) All students must have on file an official Certificate of Immunization McKinney/Vento Act No homeless (indigent) child will be denied school attendance

22 Board of Health Work Group Participants - provide well rounded & varied perspectives (Public Health, School Nurses, Child Care Nurse Consultant, Physicians, Coalitions) Goal of the Work Group - to assess the need for additional vaccines as well as rule changes based on epidemiological evidence Convenes Meet annually to discuss school immunization requirements/changes prior to presentation before the BOH
