1 Original Page 1 6. Usages for Sprint WATS and Toll Free Services 1. Service Hours Service is available 24 hours a day, seven days a week. Rate periods are applicable as indicated in the chart below (except for international calls and are based on the time at the point of origin of the call, except for Sprint Toll Free Services, where calls will be rated according to the time at the point of termination (including Canadian Toll Free calls). Calls that begin in one rate period and terminate in another will be prorated accordingly. Calls that begin in one rate period and continue into another will be prorated accordingly. For example, the charge for a 10 minute call beginning at 4:55 PM and lasting until 5:05 PM on a Monday will be the sum of 5 minutes times the appropriate distance-sensitive Day Rate plus 5 minutes times the corresponding Evening Rate. 2 2nd Revised Page 2 6. Usages for Sprint WATS and Toll Free Services (Continued) In addition to the rates, monthly recurring and non-recurring charges set forth in this section may apply. 2. Per-Minute Rates The rate charged is determined by the day and time call originates at the Sprint WATS service location. For Sprint Toll Free services, the rate charged is determined by the day and time the call terminates at the Sprint toll free service location. Pricing Example for SAW Plus A customer incurs 110 hours of usage which is broken out as 60 hours of day usage, 40 hours of evening usage and 10 hours of night/weekend usage. The customer will be billed as follows: (1) for the day usage, the customer will be billed the applicable day per minute rates in the 0 to Hours rate taper for the first 25 hours of usage, the applicable per minute rate in the 25 to Hours rate taper for the next 25 hours of usage and the applicable per minute rate in the 50 to Hours rate taper for the last 10 hours of usage; (2) for the evening usage, the customer will be billed the applicable evening per minute rates in the 0 to Hours rate taper for the first 25 hours of usage and the applicable per minute rates in the 25 to Hour rate taper for the last 15 hours of usage; (3) for the night/weekend usage, the customer will be billed the applicable per minute rates in the 0 to Hours rate taper for the 10 hours of usage. Pricing Example for Dial "1" WATS* and Dial "1" WATS Transport* A customer incurs 60 hours of usage (40 hours of day usage and 20 hours of evening usage). The customer will be billed the applicable day per minute rates in the 0 to 4.99 Hours rate taper for the first 5 hours of usage, and the applicable per minute rates in the 5 to Hours taper for the last 35 hours of usage. For the 20 hours of evening usage, the customer will be billed the applicable evening per minute rates in the 0 to 4.99 Hours rate taper for the first 5 hours of usage and the applicable evening per minute rates in the 5 to Hours rate taper for the last 15 hours of usage. * Effective September 29, 1995, this service will no longer be available to new subscribers Issued: September 13, 2012 Effective: September 15, 2012

Per-Minute Rates Dial "1" WATS ADVANTAGE* Example For discount purposes, total Dial "1" WATS ADVANTAGE* intrastate, interstate and international gross usage is aggregated from all customer locations within a customer's Dial "1" WATS ADVANTAGE* account. The total gross usage (including FŌNCARD ADVANTAGE usage and surcharges) is then used to determine a discount percentage from the Dial "1" WATS ADVANTAGE* Discount Schedule, the resulting percentage will then be applied to total intrastate, interstate and international Dial "1" WATS ADVANTAGE* and FŌNCARD ADVANTAGE gross usage. Directory Assistance, Operator Services and optional feature charges will neither contribute towards nor be eligible for volume discounts. Use the following steps to determine an example customer's volume discount percentage, resulting dollar discount and resulting invoice amount under the Dial "1" WATS ADVANTAGE* service: Step 1: To determine the volume discount, use the Total Gross Usage (including FŌNCARD ADVANTAGE usage and surcharges) of all locations ($6,400) to look up the volume discount percentage in the Dial "1" WATS ADVANTAGE* Volume Discount Schedule (8.5%).
