1 Contemporary RN Case Manager Certificate Program 2015 Program Schedule Online RECOMMENDATIONS FOR COMPLETING THE COURSE: From the first day the course starts, you have 60 days (8 weeks) to complete. This course is designed so that you are able to move through the course at your own pace coming in and out as your time allows. Time to complete may vary depending on individual variation in pace and interest in course topics and learning activities. You may continue working on the course at any time during the 60 days that the course is open. To assure completion of the program, we recommend you plan to spend a minimum of 4 hours per week. All materials for the course are posted by topic. Topic modules are posted sequentially to build on each content area covered. Each topic contains various learning activities, articles and resources. Click on each to access the learning activity. When you complete the content in each topic module, you will take a short quiz, and the next topic will be available to open up. Although we do not grade work completed in the program, it is expected that as an adult learner you are self-motivated and will thoughtfully complete the course learning activities. At the end of each topic section you will have the opportunity to take a short quiz designed to help you self-assess your understanding of the content. Each topic module in the course includes a powerpoint presentation, assigned self-study, videos and links to resources and tools. Completion of the quizzes and the Course Evaluation Tool demonstrates successful completion of this course and is required for participants to be awarded 35 Continuing Education hours. Below is a suggested schedule to use as a guide for completing the course within the 6o day time frame. 5 Week 8 Long-term Care, Skilled Nursing and Hospice Explore the role of the RN Case Manager in long term care settings including skilled nursing and hospice Examples of effective care coordination in these settings Evidence-based practice models in these settings are described Case application: develop a care plan specific to transitional care in these settings Total Continuing Education Hours for Part 2: Contemporary RN Care Coordination Across Patient Care Settings 12 This activity has been approved by Wisconsin Nurses Association to award 35 Continuing Education contact hours. WNA is an accredited approver of continuing nursing education by the American Nurses Credentialing Center s Commission on Accreditation (ANCC). ANCC is a recognized CE provider by states requiring CE for RN licensure. [Provider Approval Numbers: , ] The Systems, Policies and Roles online component has also been approved by the Commission for Case Manager Certification (CCMC) to provide board certified case managers with 23 clock hours of continuing education. The 35 CE Contemporary RN Case Manager Certificate Program requires completion of both course sections.
