We are thrilled to be hosting the EXTRA Symposium in the beautiful city of Saguenay. It s called the Land of the Giants for good reason. Dramatic cliffs rise above the Saguenay Fjord, sculpted by ancient, powerful glaciers and the surging sea. So impressed was explorer Jacques Cartier that, in 1625, he named the vast region, equal in size to Belgium, The Kingdom of Saguenay-Lac-St-Jean. 10 Biographies G. ROSS BAKER is a professor in the Institute of Health Policy, Management and Evaluation at the University of Toronto and Director of the MSc. Program in Quality Improvement and Patient Safety. Among his recent research projects are a study of patient engagement strategies in a number of leading organizations in Canada, the US and the UK. This study examines the strategies used to create effective patient and family engagement and the linkage of these strategies to efforts to improve care and care experiences. Ross is Program Director for the Masters in Quality Improvement and Patient Safety at the University of Toronto and co-lead for a large quality improvement training program, IDEAS (improving and Driving Excellence Across Sectors) funded by the Ontario Ministry of Health in collaboration with ICES (Institute for Clinical Evaluative Sciences) and HQO (Health Quality Ontario. Ross Baker, together with Dr. Peter Norton of the University of Calgary led the Canadian Adverse Events study which was published in the Canadian Medical Association Journal in Baker and Norton were awarded the Health Services Research Advancement Award for their work on patient safety and quality improvement by the Canadian Health Services Research Foundation. Ross has been a member of the Health Quality Council board in Saskatchewan since He chaired an Advisory Panel on Avoidable Hospitalizations for the Ontario Ministry of Health and Long Term Care whose report was released by the Ministry in November He also served as a member of the King s Fund (London, UK) Commission on Leadership and Management in the NHS in and has been a member of the Improvement Science Development Group of the Health Foundation in London since Leading, Sustaining and Spreading Healthcare Innovation in Canada

11 EXTRA Symposium LUC BOILEAU is a medical graduate of the University of Sherbrooke and a public health specialist. Dr. Boileau holds a Masters in Health Administration from the University of Montreal and is a fellow of the Canadian Foundation for Healthcare Improvement. In addition to having extensive clinical experience, particularly in emergency medicine, over the past 25 years, he has gained considerable experience in the management of health institutions and public health organizations, as well as the regional governance of health systems, all while getting involved and even directing several regional and national projects. On the global stage, he has implemented and managed many study projects and missions, most notably in Africa. From 1993 to 2001, Dr. Boileau was the Regional Director of Public Health and from 2001 to 2008, he was the President and CEO of the Agence de la santé et des services sociaux de la Montérégie, a health region located south-west of Montreal, home to 1.5 million people. In December 2008, the Government of Quebec appointed him CEO of the Institut national de santé publique du Québec, a position he held until January 2015 when he was appointed CEO of the Institut national d excellence en santé et en services sociaux du Québec. Dr. Boileau has also served on numerous boards in Quebec and Canada and currently sits on the Board of Directors of Accreditation Canada. ARMAND BOUDREAU has worked in the health and social services field as a professional, a manager, a senior executive and a consultant for more than thirty five years. He is recognized for his abilities in strategic settings and in developing partnerships with organizations that share the same objectives but have at times very divergent interests. He is particularly active in supporting quality improvement initiatives focussed on the patient or client experience, quality of clinical service issues and organizational performance. Mr. Boudreau has worked as a senior executive in the health and social service system for over twenty years. His most recent executive position was that of CEO of the Régie de la santé et des services de l Outaouais. This regional health authority has since become the Agence de la santé et des services sociaux de l Outaouais and most recently was integrated in the Centre intégré de santé et services sociaux de l Outaouais. He has been active as a management consultant since He offers a variety of services from strategic planning and implementation and coaching, to organizational diagnostics and organizational development. He is also involved in the development and implementation of various projects. He works quite extensively with the Canadian Foundation for Healthcare Improvement (CFHI) as a Quality improvement Coach for Senior Executive teams involved in performance improvement initiatives. He also serves on a panel of experts reviewing improvement projects being implemented in various health organizations in Canada as part of a Fellowship program for senior executives. Mr. Boudreau has Masters Degrees in Social Work and Public Administration. Canadian Foundation for Healthcare Improvement 11

12 Biographies LUC CASTONGUAY has been the Assistant Deputy Minister for the Direction générale de la planification, de la performance et de la qualité (planning, performance and quality assurance branch) under Quebec s ministère de la Santé et des Services Sociaux (MSSS) since Mr. Castonguay holds a bachelor s degree in political science from the Université Laval and a master of public administration from the École nationale d administration publique. His experience spans over twenty years in Quebec s public service, where he served at MSSS as assistant to the Assistant Deputy Minister for planning and assessment; coordinator in the midwifery pilot projects coordination unit; and delegate to the Bureau du Commissaire aux plaintes en matière de santé et de services sociaux (office of the health and social services complaints commissioner). Since 2000, Mr. Castonguay has held various positions within the ministère du Développement économique, de l Innovation et de l Exportation (ministry of economic development, innovation and exports), including as Director General of Recherche Québec and Director of academic research. MARTINE COUTURE has gained significant experience in the areas of health and social services in the Saguenay-Lac-Saint-Jean region. A nurse by training, she served as Director General of the CSSS (health and social services centre) Cleophas-Claveau from 1998 to Ms. Couture was also the Chief Executive Officer of the Agence de la santé et des services sociaux du Saguenay-Lac-Saint-Jean for over ten years. A Canadian Foundation for Health Improvement EXTRA fellow, Ms. Couture holds a bachelor of science in nursing and a master of public administration. 13 EXTRA Symposium JANET DAVIDSON s term as Deputy Minister of Alberta Health ended in August 2015 and she is now serving as Special Advisor in Health to the Government of Alberta. Janet Davidson was appointed Deputy Minister of Alberta Health on September 10, She came into this role with over 30 years of experience in healthcare in the government, voluntary and hospital/community sectors in a number of Canadian jurisdictions. She has also worked internationally in humanitarian relief and development. She was also the Official Administrator of Alberta Health Services. Prior to that, she was the Canadian Executive and Industry Specialist with KPMG s Global Healthcare Practice. From 2007 to 2011, Davidson was President and Chief Executive of Trillium Health Centre in Mississauga, where she led them through to the successful merger with the Credit Valley Hospital creating the largest community academic hospital in the country. Janet has a wealth of experience in all aspects of health care policy and planning, management and operations. She is a well recognized and respected health care executive with a reputation for working with key stakeholders to achieve outstanding results: individually, organizationally, and more broadly at the community level. She has gained a reputation as an outstanding leader and facilitator and has been called upon to lead and/or support major transformational initiatives including: hospital turnarounds, mergers, governance and operational reviews, clinical redesign, development and implementation of health manpower strategies, and design and implementation of health care funding mechanisms. She has also been extensively involved in global humanitarian work and served for 10 years in a senior governance capacity with the International Red Cross and Red Crescent Movement in Geneva, first as a Vice President (Americas) and then as Vice Chair of the Standing Commission, its highest deliberative body. Janet trained as a nurse at Toronto East General Hospital. She received a Bachelor of Nursing Science from the University of Windsor and a Master s in Health Services Administration from the University of Alberta. She is a graduate of the Institute of Corporate Directors Education program, the UC Berkeley School of Public Health s Global Health Leadership program and has an Honorary Doctor of Laws Degree from the University of Windsor. She has received alumnae recognition awards from Branksome Hall School and the universities of Windsor and Alberta. She is an Officer of the Order of Canada and has been named twice to the list of Canada s Top 100 Most Powerful Women. In 2011 and 2012 Janet was also named as one of Canada s Top 25 Most Influential Women. She is Vice Chair on the Board of the Canadian Institute for Health Information and also sits on the Board of the Canadian Foundation for Healthcare Improvement. Other board experience includes service on the Board of the Ontario Institute for Cancer Research and the immediate past chair of the Ontario Hospital Association. 14 Biographies JEAN LOUIS DENIS is Full Professor at the Ecole Nationale d Administration Publique (ENAP) and holds the Canada research chair on governance and transformation of health care organizations and systems at ENAP. He is a visiting professor at Euromed Management (Marseille) and researcher at the Institut de recherche en sante publique de l Universite de Montreal. He pursues research on governance and change process in health care organizations and systems. His current research looks at integration of care and services, health care reforms, medical leadership and leadership in professional organizations, strategies for health care improvement and the role of scientific evidence in the adoption and implementation of clinical and managerial innovations. He is a member of the Royal Society of Canada, fellow of the Canadian Academy of Health Sciences and was chair of the advisory board of CIHR s Institute of Health Services and Policy Research ( ). He was the founding academic coordinator of the FORCES/EXTRA initiative from 2003 to 2007, a training program which aims at developing Canada s health managers competencies in research use. Recent papers have been published in Journal of Health Politics, Policy and Law, Organization Science, Milbank Quarterly and Administration and Society. He was the principal investigator of a CIHR team grant on the theme of Health system reconfiguration ( ). From March 2015, he is appointed as visiting professor at the Department of Management, King s College London. EILEEN DE VILLA is the Associate Medical Officer of Health at Peel Public Health. She has been serving in an acting capacity as Peel s Medical Officer of Health since September 15, Peel Public Health, the local public health agency serving the 1.3 million residents of the cities of Mississauga and Brampton and the town of Caledon, is one of the largest public health departments in the country and is a leader in changing the way communities are designed and built. Dr. de Villa participated in the EXTRA program in (Cohort 9). It was through the EXTRA program that she learned of the Triple Aim Initiative. Dr. de Villa holds an undergraduate degree from McGill University. She continued her studies at the University of Toronto where she completed both her Master of Health Science and her medical degree. She subsequently became a Fellow of the Royal College of Physicians of Canada, specializing in Public Health and Preventive Medicine. During her residency training, she practiced family medicine on a part-time basis at a community health centre in Toronto and completed a Master of Business Administration degree at the Schulich School of Business. 14 Leading, Sustaining and Spreading Healthcare Innovation in Canada

15 EXTRA Symposium HEATHER FRASER is a seasoned strategist, entrepreneur and educator, Heather is a global thought leader in Business Design with a passion for healthcare innovation. She co-founded Rotman DesignWorks with Roger Martin in 2005 and served as its Executive Director through She has cultivated Business Design as a discipline, designed and delivered MBA student curricula and led innovation programs for over 3,000 executives internationally. Her clients include Procter & Gamble, Nestlé, General Electric and Target, and healthcare teams at Pfizer, University Health Network, Princess Margaret Hospital and Medtronic. Heather s work builds on her 25-year track record for insight, innovation and results that combine business acumen and creativity with designing new ways to create value. Her book, Design Works: How to Tackle Your Toughest Innovation Challenges through Business Design, gives a balanced and pragmatic view of how organizations committed to innovation and value creation can infuse design principles and practices into planning and development. Endorsed by Dr. Robert Bell, President and CEO, University Health Network, Business Design, teaches leaders how to engage these principles in tackling chronic problems and initiating change in the public or private sector and discusses why these methods are important. These principles have been successfully applied to solving a wide range of strategic and tactical challenges, including the redesign of the patient experience at Princess Margaret Hospital s Chemotherapy Day Unit. In the words of Sarah Downey, Vice-President of PMH at the time, Business Design allowed us to unlock the creativity of staff and to empower our patients not just in designing a space but rather in developing a new patient and staff experience. In 2012, Heather founded Vuka Innovation to expand this work beyond the academic realm. As the driving force behind Vuka, she ensures that organizations push their thinking and realize their full capacity to innovate. She continues to be active in public education as Adjunct Professor, Researcher and Business Design Expert in Residence at the Rotman School of Management, University of Toronto. CHRIS W. HAYES is a Harkness Fellow in Health Care Policy and Practice and Improvement, a fellow at the Institute of Healthcare Improvement and a critical care physician, and is a Medical Director for Quality and Performance at St. Michael s Hospital in Toronto. He also serves as Medical Officer for the Canadian Patient Safety Institute and is an assistant professor in the Department of Medicine and the Institute of Health Policy, Management, and Evaluation at the University of Toronto. Dr. Hayes has led many quality improvement initiatives and is a core faculty member in the Masters of Quality Improvement and Patient Safety program at the University of Toronto. 16 Biographies MARIA JUDD is a Senior Director at the Canadian Foundation for Healthcare Improvement. She is responsible for developing and implementing strategies, programs and activities in support of CFHI s mission, and leading business development initiatives. Maria s passion for healthcare improvement has evolved from her diverse roles within the health system, such as a community health centre board member, clinician, program manager and researcher. She has extensive experience in knowledge transfer and exchange, establishing and promoting new ventures, creating education programs, developing clinical practice guidelines and convening expert groups. Prior to joining CFHI in 2005, Maria worked in a variety of health-related organizations. As the best practices program manager for the Canadian Physiotherapy Association, she designed a program of activities to support evidence-informed practices among members and their colleagues. She also coordinated the Cochrane Collaboration s Musculoskeletal Group at the University of Ottawa where much of her work focused on the development of evidence-based resources, such as systematic reviews, and supporting knowledge translation activities. Maria holds a Master of Science in Epidemiology and Community Medicine from the University of Ottawa and a Bachelor of Science in Physical Therapy from the University of Toronto. CHRISTINA KRAUSE brings a variety of health care experience to her role as Executive Director with the BC Patient Safety & Quality Council. Her passion and interests include the use of social change models and network theory in efforts to engage and mobilize stakeholders to improve quality of care. More recently, this has expanded to include social media to create enhanced connections and shared learning. Christina was an author on the Canadian Disclosure Guidelines and serves as Master Faculty with the Institute for Healthcare Communication s Disclosing Unanticipated Medical Outcomes. Christina holds a Masters in Science in community health and epidemiology from the University of Saskatchewan, and a Bachelor of Home Economics in family science and nutrition from the University of British Columbia. She is also an EXTRA Fellow with the Canadian Health Services Research Foundation. ANDRÉ NÉRON enjoyed a very successful career in the area public affairs for over 30 years. A patient himself, he chaired the Faculty of Medicine s Expert Patient Committee at the University of Montreal and was appointed Associate Director of the Bureau facultaire de l expertise patient partenaire (Faculty office for patients as partners in care ) which became the University of Montreal s Direction collaboration et partenariat patient (patient partnership and collaboration branch). Currently, Mr. Néron is the Associate Director of the Direction collaboration et partenariat patient and chair of the Expert Patient Committee of the Faculty of Medicine at the University of Montreal. 16 Leading, Sustaining and Spreading Healthcare Innovation in Canada

17 EXTRA Symposium PATTY O CONNOR was formerly the Director of Nursing for the McGill University Health Centre. Ms. O Connor, RN, MScN, CHE, FCCHL, is now a Senior Advisor for Patient Engagement and the McGill Nursing Collaborative. Ms. O Connor is an Associate Professor in the Ingram School of Nursing at McGill, a Certified Health Executive, and Past President of the Academy of Canadian Executive Nurses. She has completed fellowships with the Canadian Foundation for Healthcare Improvement (EXTRA program), the Canadian College of Health Leaders (CCHHL) and the U.S. Commonwealth Fund. Her current efforts are focused on engaging patients and frontline staff in the re-design of care delivery systems, and in supporting evidence-informed practice and administration. In 2012, Ms. O Connor was recognized as one of the Top 8 Women in Business in the Healthcare Sector in Quebec, and in 2014, she was awarded the CCHL National Nursing Leadership Award. MAUREEN O NEIL is President of the Canadian Foundation for Healthcare Improvement. Previous positions include President of the International Development Research Centre, Interim President of the International Centre for Human Rights and Democratic Development, President of the North-South Institute, and Deputy Minister of Citizenship, Government of Ontario. Ms. O Neil is Chair of the Board of the United Nations (UN) Research Institute for Social Development, a member of the board of e-health Ontario, a member of the Board of Directors for the Centre for International Governance Innovation and a member of the board for the Global Development Network. She is also the Chair of the McGill Institute for Health and Social Policy Advisory Board. She is former Chair of the Board of Trustees of the International Institute for Environment and Development, the International Advisory Group, IDRC s Think Tank Initiative, and the Advisory Council on Corporate Social Responsibility of Canada s Export Development Corporation. She has chaired the Board of Governors of Carleton University and has also represented Canada on the UN Commission on the Status of Women and on OECD committees, and has been a member of the UN Committee for Development Planning and the Board of the UN Research Institute for Social Development. Ms. O Neil has chaired or been a member of boards or committees of several not-for-profit organizations. In June 2011, Ms. O Neil was named an Officer of the Order of Canada for public service, recognizing her contributions to international development, gender equality and human rights. Ms. O Neil has a BA in Sociology from Carleton University and Honorary LLDs from Wilfrid Laurier University, Carleton University and the University of Ottawa. 18 Biographies KAYE PHILLIPS is a Senior Director at the Canadian Foundation for Healthcare Improvement. Kaye is responsible for overseeing CFHI s EXTRA program for Healthcare Improvement and a pan-canadian Collaborative on reducing inappropriate prescribing of anti-psychotics in long term care. She also leads the design and integration of strategic evaluation and performance measurement across CFHI s programs. Kaye brings 10 years of experience working as an applied health services researcher, evaluation professional and decision support and change management specialist with an interest in quality improvement and collaborative patient-centered care. Over the course of her career, Kaye has facilitated organization-wide strategic planning and led the design and implementation of program, corporate and cross-regional chronic disease management outcome evaluations, including the North-West Territories Department of Health and Social Services Making the Case for Change: Advancing the NWT Chronic Disease Management Strategy evaluation and CFHI s Atlantic Healthcare Collaboration cross-case evaluation (in progress). Kaye has also led the design of CFHI s performance improvement and stakeholder engagement curriculum and e-workshops and serves as a guest faculty across CFHI s programs. Kaye holds a PhD from the University of Toronto s School of Social and Administrative Pharmacy and is an Ontario Training Center in Health Services and Policy Research fellow and alumna. Her PhD applied network analysis to look at new models for measuring collaborative research and development for neglected global diseases. Previously Kaye worked as a research and policy analyst with the Initiative for Drug Equity and Access at the University of Toronto, the Assessment of Technology in Context lab at Simon Fraser University and with the Institute for Community Health Promotion at the University of British Columbia. JEAN ROCHON has worked in the field of training, research and public health. He served as Director of the Department of Social and Preventive Medicine and Dean of the Faculty of Medicine at Laval University. After heading a Commission of inquiry on health and social services, he worked with the World Health Organization (WHO). As a member of the Government of Quebec, he was the Minister of Health and Social Services, Minister of Research, Science and Technology and Minister of Employment, Social Solidarity and Labour. He holds a law degree (LL.B) from the University of Montreal, a medical degree (MD) from Laval University and a doctorate in public health (DrPH) from Harvard University. 19 EXTRA Symposium STEPHEN SAMIS is Vice-President, Programs at the Canadian Foundation for Healthcare Improvement. He has experience in health research, policy development, knowledge exchange, partnership development and advocacy. Stephen joined CFHI in 2010 to lead the implementation of CFHI s new strategic plan. Since that time he has re-positioned the organization with new programming, new branding and new partnerships. Stephen has played a leadership role in positioning CFHI as Canada s national healthcare improvement organization. Prior to CFHI, he was national director of Health Policy at the Heart and Stroke Foundation of Canada. Stephen moved to Ottawa in 2000 to take on the role of Manager, Research and Analysis, Population Health, at the Canadian Institute for Health Information. R. LYNN STEVENSON is currently the Associate Deputy Minister of Health responsible for the health services portfolio in the British Columbia Ministry of Health. She is responsible for the divisions of health service policy, quality assurance, population and public health and medical beneficiary and pharmaceuticals. Lynn is a registered nurse by profession and has held senior leadership positions in the Vancouver Island Health Authority, British Columbia Cancer Agency and the Fraser Health Authority in British Columbia. Lynn has extensive health care experience in operations, quality and safety, human resources and research. She currently co-chairs the British Columbia Academic Health Council board, is a member of the Royals Roads University Leadership Advisory Board and the University of Victoria Human and Social Development Advisory Committee. Lynn has a PhD with a research focus on organizational change, practice and leadership. She is a Fellow of the Canadian College of Health Leaders (CCHL), a CFHI EXTRA Fellow, certified corporate director (ICD-D) and holds Adjunct Professor positions in the Schools of Nursing at the Universities of British Columbia and Victoria. She has received awards from the Canadian College of Health Leaders, the University of Victoria and the College of Registered Nurses of British Columbia. 20 Biographies TERRENCE SULLIVAN is the independent board chair of the Canadian Agency for Drugs and Technologies in Health (CADTH). He also chairs the board quality committee of the Hospital for Sick Children and the board governance committee for Exactis Innovation (a federal NCE). From 2001 to March 2011 he occupied successively responsible positions at Cancer Care Ontario (CCO), the final seven years as President and CEO during which period the entire organization transformed its business model to performance measurement and improvement of cancer services. From he was the founding president of the Institute for Work & Health (IWH), North America s leading research center on work-related injury. He has played senior roles in the Ontario Ministries of Health, Cabinet Office and Intergovernmental Affairs. He served as Assistant Deputy Minister, Constitutional Affairs and Federal-Provincial Relations during the Charlottetown negotiations, and he served two successive First Ministers of Ontario as Executive Director of the Premier s Council on Health Strategy including a period as Deputy Minister (1991). A behavioural scientist he is also adjunct professor in the department of Oncology at McGill University and he has provided a range of professional advisory services to governments and health care organizations in Canada and internationally. LESLEE J. THOMPSON has extensive strategic, operational and transformational leadership experience in both the public and private sectors. Starting out as a critical care nurse, Leslee s passion for improving patient care has fueled a career that spans from the bed side to the board rooms of top tier organizations. Leslee started as a critical care nurse, and is now the President and CEO of Kingston General Hospital. Leslee is an energetic, systems thinker who has led many teams through periods of significant change. She has been Vice President, Cancer Care Ontario, Executive Vice President & Chief Operating Officer of Sunnybrook & Women s Health Science Center; Chief Operating Officer of Toronto Western Hospital at University Health Network, and Senior Operating Officer of Royal Alexandra Hospital in Capital Health, Alberta. In the private sector, Leslee was Canadian Vice President Health System Strategies for Medtronic, a global medical devices company where she had national and international responsibilities in areas of strategy, health policy and strategic account management. She was also on the Board of Directors at Shoppers Drug Mart and is a Certified Corporate Director (ICD.D). Leslee has an MBA from University of Western Ontario, a Masters of Nursing from University of Toronto, and a Nursing degree from Queen s. Leslee is Chair of the Council of Academic Hospitals of Ontario and Board member of Ontario Institute for Cancer Research. 21 EXTRA Symposium ANIQUE TURGEON is an Improvement Lead at the Canadian Foundation for Healthcare Improvement. Over the past four years, she has been responsible for managing and operationalizing CFHI s flagship Executive Training program. Anique brings experience in leading and redesigning programs arising from her eight years working in various CFHI portfolios. JENNIFER VERMA is a Senior Director at the Canadian Foundation for Healthcare Improvement. She is responsible for the start-up of large-scale health system improvement partnerships across regions, provinces and territories in Canada. Jennifer brings expertise in applied health services research and experience in sharing and spreading evidence-informed healthcare solutions. She has developed and led a variety of knowledge exchange products with the Canadian Foundation for Healthcare Improvement, most notably, Mythbusters and the Mythbusters Teaching Resource. Previously, Jennifer worked in the Department of Quality and System Improvement at Eastern Health, the largest regional health authority in Newfoundland and Labrador. She also worked in communications and broadcast journalism as a producer and reporter. She holds a Master of Science in Medicine in Applied Health Services Research from Memorial University of Newfoundland.
