1 Culture Grants Online: Applying for a grant using the Miami-Dade County Department of Cultural Affairs CGO online grant system To apply for a grant using the CGO grant system, applicant organizations MUST complete ALL of the items listed below by the program s deadline date via the CGO online grant system. Make sure to allow sufficient time to complete each of the following: 1) FIRST complete the Grant Eligibility and Request Form AND be deemed eligible by the program s Grant Administrator BEFORE accessing Part 1 of the grant application FormSet. 2) Fully complete and successfully SUBMIT Part 1 of the grant application FormSet. 3) Fully complete and successfully SUBMIT Part 2 of the grant application FormSet. Step 1 - Complete the Grant Eligibility and Request Form: Note: these instructions are for applicants who have already attended the CGO Basics workshop, and have been properly registered in the CGO system. 1) Go to: 2) Click on the Log In tab in the upper right hand corner of the page. Enter your User Name and Password. Passwords are case sensitive. If you have forgotten your password, click on the Forgot Your Password? hyperlink. 3) You will be directed to the Available Applications tab. 4) In the row for the grant program to which you would like to apply, click on: Apply. Grant programs are listed alphabetically. In the case of applying to our quarterly programs (CG or TDC), make sure you apply to the correct quarter. 5) The grant Eligibility and Request Form will display. 6) Complete the grant Eligibility and Request Form. Answer all required questions and upload all required Eligibility support documents listed on the Form. 7) To upload new documents from your computer s hard drive: a. Click on the Upload button. The Portfolio Media Uploader box will appear. b. Enter a Title for the file to be uploaded. c. Enter a Description for the file to be uploaded. CGO 2.0 Application Instructions (Rev ) P age 1
2 d. Click on the gray Browse button. e. Navigate your computer s document folder to find the file to be uploaded. f. Select the file and click Open. g. Click the gray Submit button to upload the file to the portfolio folder. h. On the Form page, click on the support documents check boxes to indicate which items have i. Click on the Save button. To COPY files uploaded previously to your CGO portfolio: a. Click on the Manage Folders button. b. Click on the folder that contains the file you wish to copy. c. Hover over the file you wish to copy. d. Click on the Move/Copy Material icon (blue arrow) found below the thumbnail of the file you wish to copy. e. Select [GRANT PROGRAM] Application: Part 1 [FISCAL YEAR] Folder from the drop down list. f. Select Copy File. g. On the Form page, click on the support documents check boxes to indicate which items have h. Click on the Save button. The document thumbnail will then appear on the Form page. 8) When all required questions have been completed and all required Eligibility support documents have been uploaded, click on the Submit button. If properly submitted, a pop-up confirmation will display on screen and the primary account holder will receive an confirmation. 9) Please allow up to 3 business days for your Eligibility and Request Form to be reviewed by the grant program administrator. You will receive an from the grant program administrator once your request has been reviewed and/or approved. CGO 2.0 Application Instructions (Rev ) P age 2
3 Step 2 - Fully complete and successfully SUBMIT Part 1 of the grant application FormSet: 1) Go to: 2) Click on the Log In tab in the upper right hand corner of the page. Enter your User Name and Password. Passwords are case sensitive. If you have forgotten your password, click on the Forgot Your Password? hyperlink. 3) Click on: My Grants 4) In the row for the grant program to which you have requested access, click on: Link to FormSet. Grant programs are listed first alphabetically, then by fiscal year. 5) Click on the [GRANT PROGRAM] Application: [FISCAL YEAR] Part 1 folder hyperlink. The Organization Info UNIV Form will display. 6) Complete/update the required fields on the Organization Info UNIV Form page. Click the Save or Save & Continue button at the bottom of the Form page. 7) Complete/update the required fields on the Other Contacts UNIV Form page. Click the Save or Save & Continue button at the bottom of the Form page. 8) Complete/update the required fields on the ADA Questionnaire UNIV Form page. Click the Save or Save & Continue button at the bottom of the Form page. 9) Complete/update the Organization Support Material Form page. Click the Save or Save & Continue button at the bottom of the Form page. To upload new documents from your computer s hard drive: a. Click on the Upload button. The Portfolio Media Uploader box will appear. b. Enter a Title for the file to be uploaded. c. Enter a Description for the file to be uploaded. d. Click on the gray Browse button. e. Navigate your computer s document folder to find the file to be uploaded. f. Select the file and click Open. g. Click the gray Submit button to upload the file to the portfolio folder. h. 4 To COPY files uploaded previously to your CGO portfolio: i. Click on the Manage Folders button. j. Click on the folder that contains the file you wish to copy. k. Hover over the file you wish to copy. l. Click on the Move/Copy Material icon (blue arrow) found below the thumbnail of the file you wish to copy. m. Select [GRANT PROGRAM] Application: Part 1 [FISCAL YEAR] Folder from the drop down list. n. Select Copy File. o. On the Form page, click on the support documents check boxes to indicate which items have p. Click on the Save button. The document thumbnail will then appear on the Form page. 10) Read the instructions on the Final Submit: Part 1 UNIV Form page and click on the box stating Yes, I have read the instructions. Click the Save button. 11) Click the Submit button. If properly submitted, a pop-up confirmation will display on screen and the primary account holder will receive an confirmation. 12) Congratulations! You are now ready to complete Part 2 of the grant application FormSet. You do not have to wait for permission from the grant program administrator to proceed to Part 2. 5 Step 3 - Fully complete and successfully SUBMIT Part 2 of the grant application FormSet: 1) Go to: 2) Click on the Log In tab in the upper right hand corner of the page. Enter your User Name and Password. Passwords are case sensitive. If you have forgotten your password, click on the Forgot Your Password? hyperlink. 3) Click on: My Grants 4) In the row for the grant program to which you have requested access, click on: Link to FormSet. Grant programs are listed first alphabetically, then by fiscal year. 5) Click on the Application: Part 2 folder hyperlink. The Organization Info UNIV Form will display. 6) Complete/update all required fields on each of the Forms in the Application FormSet. Remember to click the Save or Save & Continue button at the bottom of each Form page. 7) Complete/update the Organization Support Material Form page. Click the Save button. 9) Click the Submit button. If properly submitted, a pop-up confirmation will display on screen and the primary account holder will receive an confirmation. 10) Congratulations! You have completed Part 1 and Part 2 of the grant application. 11) The grant program administrator will review properly submitted applications for technical sufficiency and provide comments/suggestions to aid applicants in improving the content and quality of the application. An application checklist will be posted in the CGO system or sent via , fax and/or mail to applicants indicating the recommended corrections, corrections procedures and deadline date to submit corrections and required materials for panel distribution. The application FormSet will remain read only until the grant program administer unlocks the application for the corrections period. FINAL APPLICATION SUBMISSION: In order to formally submit the final application for panel review, applicants must open the application FormSet during the corrections period, make all necessary corrections and click SUBMIT on the Final Submit: Part II UNIV Form page.
