1 Highlights of our more popular offerings include: Wealth Accumulation positions you for a better tomorrow through Profit Sharing, Company match on 401(k) contributions, plus a match on the Employee Stock Purchase Plan. Profit Sharing On an annual basis, active employees may receive contributions from the Bank s profits. The contribution is based on the Bank meeting or exceeding specified financial goals and ranges from 0-10% of Profit Sharing Eligible Pay with a target of 4%. The contribution is placed in a retirement account set up for each employee. Profit Sharing contributions vest after three years of service. Eligible compensation for Profit Sharing is accumulated from January 1 to December 31 of each year. 401(k) Plan All employees are eligible to defer 1 to 50% of their eligible earnings, subject to IRS limitations, to the 401(k) Plan after 30 days of employment. Employees may choose to defer pretax or Roth 401(k) contributions, or a combination of the two. 401(k) Match Fifth Third offers a 100% match on the first 4% of pretax or Roth 401(k) contributions made by employees on a per pay period basis. The match vests after three years of service. Employee Stock Purchase Plan Fifth Third offers employees the opportunity to purchase Fifth Third Bancorp Common Stock through payroll deductions. The Bank provides a 15% match on employee contributions. Note: Fifth Third Bank reserves the right to terminate or change plans at any time by action of the Human Resources Department. As always, the Plan Documents, not this summary, are controlling Insurance Plans offer assurance that you or your loved ones will be taken care of in your time of need whether it is medical, dental, life or disability. Fifth Third Bank helps you offset these costs by giving you Benefit Choice Dollars. Benefit Choice Dollars* After 30 days of employment, eligible employees receive Benefit Choice Dollars, which help offset the cost of elected benefits and are equal to 4% of benefits salary. Benefit Choice Dollars are paid as additional cash compensation over the 26 pay periods of the calendar year. Medical Insurance* Fifth Third offers medical coverage to eligible employees after a 30-day waiting period. Employees are offered an EPO/HMO, PPO and HSP (a high-deductible health plan with the option to contribute to a Health Savings Account). Dental Insurance* The Dental Plan offers two levels of coverage: Basic and Premium. Basic coverage provides preventive and basic services. The Premium Plan offers major services in addition to preventive and basic. Coverage begins after a 30-day waiting period.
2 Vision Insurance* The Vision Plan offers coverage on routine eye exams as well as other eye care needs. Coverage begins after a 30-day waiting period. Life Insurance Basic* The Bank provides basic life insurance coverage for full-time and part-time employees hired to work more than 20 hours/week. Coverage is effective after 30 days of service. The provided coverage for full-time employees is one times benefits salary, up to a maximum of $1,000,000. The provided coverage for part-time employees is $10,000. Supplemental Life Insurance Coverage Full-time employees have the opportunity to purchase up to five times their benefits salary, up to a maximum of $1,000,000. Spouse and/or dependent children may also be covered at a flat benefit amount. Other Insurance Fifth Third offers its employees a variety of other insurance plans, including: Disability Insurance Voluntary Personal Accident Insurance* Business Travel Accident Insurance Pet Insurance * Part-time employees must work more than 20 hours/week to be eligible for benefits. Flexible Spending Accounts Employees may elect to have an amount deducted from their pay before taxes are withheld and deposited into any of the following reimbursement accounts: Health Care, Dependent Care, or Parking/Transit. Health Care This account allows reimbursement for out-of-pocket medical expenses. Employees may contribute a maximum of $10,000. Dependent Care This account allows reimbursement for dependent care expenses. Employees may contribute up to $5,000. Parking/Transit These accounts allow for reimbursement for work-related parking/transit expenses. The monthly limits are: Parking $230, Transit $230. Limited Purpose This account allows for a contribution up to $10,000 annually for dental, vision and preventive care services, plus any co-insurance, co-payments and other medical expenses after you meet the deductible. A Bank-wide Incentive Plan is in place for all employees, because when we succeed, you succeed. Team Fifth Third is a Bancorp-wide incentive program that rewards Fifth Third employees for new business referrals. This program encourages employees to cross-sell a range of products and services and offers the potential to earn unlimited incentive pay for qualified referrals.
3 Having Work/Life Balance is important for you to lead a productive life at Fifth Third and at home. Alternative Work Arrangements* Fifth Third Bank understands that allowing you to have more control over where and when you work is a way to increase job satisfaction and retention. Experienced and engaged employees ensure the Bank s competitive edge for the future. Fifth Third Bank offers Alternative Work Arrangements to help you with work/life balance. Our Alternative Work Arrangement options are: Compressed Work Week Flextime Job Sharing Part-time Exempt Remote Work Telecommuting *Implementing an Alternative Work Arrangement for eligible employees includes careful consideration of each employee s situation on a case-by-case basis. Decisions regarding Alternative Work Arrangements will be made based on what best serves both the employee and the business. Vacation Regular full-time and part-time employees hired to work more than 20 hours/week earn vacation based on length of service. Vacation time will accumulate on an hourly basis each pay period. Vacation Purchase Employees may be eligible to purchase up to five days of additional vacation. This election can be made only during open enrollment. Holidays Fifth Third provides holiday pay for all regular employees upon hire. Full-time employees receive eight hours of pay per holiday. Part-time employees receive holiday pay based on that day's normally scheduled hours. Ten national holidays are observed. Sick Days Eligible employees are eligible for sick days. These days are incremented on a per pay basis. Current days are available for use in the current year when a personal or family illness prevents an employee from working. Medical Appointments Fifth Third recognizes that employees may need time off occasionally for a medical appointment. Therefore, full-time employees may be eligible for up to two hours for a medical appointment per quarter. Jury Duty Full-time and part-time employees summoned for jury duty are paid for the time taken to complete this obligation. Employees may keep any money earned while on jury duty. Fifth Third is proud to offer other employee benefits such as discounts and educational assistance. Educational Assistance The Bank offers a Forgiveness Loan Program to help employees finance tuition costs. The loan need not be repaid if the employee remains with the Bank for one year after completing the course and receiving reimbursement. Full-time employees are eligible to receive $5,200 per calendar year, and part-time employees are eligible to receive $1,200. Employee Assistance Program The Employee Assistance Program provides the opportunity for employees to receive confidential assistance regarding a variety of personal, emotional and/or situational concerns that might affect them and their family. This free and confidential tool is available to all employees starting on their first day of employment.

Participation in the Fifth Third Bank options does not guarantee that contributions plus earnings will be adequate to cover future college expenses; contributions and earnings are not guaranteed by the state of Ohio or the Ohio Tuition Trust Authority. For a current copy of the Offering Statement, go online at call AFFORD-IT ( ) or ask a Fifth Third Bank representative. Before you invest, consider whether your home state offers a 529 plan that provides its taxpayers with state tax and other benefits not available through this plan **** Terms are available during the period of employment with Fifth Third Bank. Upon termination, not including retirement, the rewards component of the credit card account, balance transfer rate and future balance transfer fees will change to the then current Platinum Rewards point earning calculation and expiration, balance transfer rate and balance transfer fee. Employees will be notified of any changes to the terms of the card as outlined in the Card Agreement. The 3.9% Introductory Annual Percentage Rate (APR) for Purchases is valid until 01/01/2012. After that, your APR will be prime %, currently 5.24%. The 3.9% APR is for the life of the balance transferred and applies to Balance Transfers only. If you no longer qualify for this introductory APR, your APR will be the standard APR for Purchases and will be 5.24%. Both the APR for Purchases and Balance Transfers will vary with the market based on the WSJ Prime Rate. We may end your introductory APR for Purchases and Balance Transfers and apply the APR in effect or the Penalty APR if you make a late payment, exceed your account credit limit, make a payment that is returned or use a convenience check that is not honored. Cash Advance APR: 18.24%. This APR will vary with the market based on the WSJ Prime Rate. Delinquency APR: 24.99% and may be applied to your account if you make a late payment, go over your credit limit, make a payment that is returned or use a convenience check that is not honored. Once your APR are increased for any of these reasons, the Penalty APR may remain in effect indefinitely. The APR s for Purchases, Balance Transfers and Cash Advances are based on the WSJ Prime Rate plus a margin. The rate is calculated monthly by adding the margin applicable to the account to the WSJ Prime Rate. The WSJ Prime Rate means the prime rate published in the "Money Rates" section of the online version of The Wall Street Journal. We determine the WSJ Prime Rate two days prior to the closing date of your Billing Cycle. Changes to the Monthly Periodic Rate and corresponding APR for Purchases will be applied to your existing Account balance and to subsequent transactions effective the first day of the Billing Cycle in which we determine the interest rate. If the Wall Street Journal does not publish a prime rate, or if it changes the definition of the prime rate, we may, at our sole discretion, substitute another index. There is a 21- day grace period for the repayment of the balance of purchases after the close of your billing cycle, provided that the previous account balance is paid in full by its payment due date. We use the Average Daily Balance method, including new purchases, to compute the balance. Minimum finance charge: $1.00. Balance Transfer fee: $5.00 or 4.0% of the total dollar amount of the balance you are transferring, whichever is greater. Convenience Check Fee: $ 5.00 or 4.0% of the total dollar amount, whichever is greater. Cash advance fee: $5.00 or 3.5% of the total dollar amount, whichever is greater. Expedited Card Delivery fee: $ Replacement card fee: $10.00; Closed account fee: $2.50 per month if closed voluntarily. Late Payment Fee: $19.00 if your New Balance is less than $250.00, or $ if your New Balance is $ or greater. Returned Payment Fee: $35. For all transactions made outside the United States or a foreign currency, a fee of 3% of the U.S. Dollar amount of each foreign transaction that posts to your account will be assessed. Terms and conditions subject to change without notice. Fifth Third and Fifth Third Bank are registered trademarks of Fifth Third Bancorp. Member FDIC. Equal Housing Lender
