1 How to obtain a Verizon Wireless Mobile Phone Using the FOP Corporate Account Discount Plan can be downloaded at: If you want to move your existing Verizon cell phone service, or move from another phone carrier, to the FOP s corporate account, please use the ASSUMPTION OF LIABILITY (AOL) form. You must use your My Verizon corporate web account, which you must ask the FOP point of contact to set up 48 hours after your corporate account is activated. Because transferring to a corporate account changes the liability associated with your phone service; o You must remain current in your payments to keep your phone service active. o Equipment and other services (such as downloaded smartphone apps) must be paid with a personal credit card instead of being added to your Verizon bill. After activating your FOP Verizon corporate account, expect to get a final bill for the personal account you are leaving listing all of the charges up to the date of transfer. This final bill must be paid. The final bill also serves as a notice of transfer. The SPOC (Points of Contact) J.R. Pool and Lorraine Thompson are not Verizon employees; they are FOP members who have volunteered to process the paperwork and set up your Verizon Web access. If you want to move your existing non-verizon cell phone service to the FOP s corporate account, before doing so, call your current cell phone service provider to ensure you will not incur an early cancellation fee. If you will incur a fee, determine if it s something you are willing to pay or you may decide to wait until your contract with your current provider has ended before moving to Verizon Wireless. If your existing Verizon account is Bundled with Century Link or any other type of Bundled payment plan you must de-enroll your Verizon account before it can be transferred to the FOP Verizon discount program. If you do not have a cell phone account with any provider, fill out the AOL form. Since you will not have existing Verizon account information, leave those boxes blank.

2 The Following is a STEP-BY-STEP procedure for completing the AOL form. Follow the directions posted in each box. Please note some of the information requested may not apply to you. Please do not go into or call a Verizon store for help or questions. They will not be able to help as they not familiar with corporate pricing and have no access to this information Account Information Relinquishing Customer UNDER THE HEADING ACCOUNT INFORMATION RELINQUISHING CUSTOMER: IN THE BOX Signed: Type your name. IN THE BOX Wireless Number to be transferred: List all the cell phone numbers you want transferred to the FOP account. IN THE BOX Billing Address: Enter your home address. NO PO BOXES ACCEPTED. IN THE BOX Existing Account Number: If you have an existing VERIZON cell phone account list the account number. You can find this on your current invoice. IN THE BOX Current Calling Plan: Enter the name of the calling plan you have (i.e. Nationwide Talk and Text 1400 minutes) you can find this info on your current billing. IN THE BOX Relinquishing Customer Name: Enter your name. IN THE BOX Billing Address City: Enter the name of the city you live in that corresponds to the street address you listed as your BILLING ADDRESS. IN THE BOX Billing Address Zip Code: Enter the Zip Code that corresponds to the street address and city you listed as your BILLING ADDRESS. IN THE BOX Relinquishing Customer Address: Enter your address. IN THE BOX Relinquishing Customer Contact Number: Enter the phone number you can be contacted at. This can be a hardline phone number or can be your cell phone number. UNDER THE HEADING: If transfer involves a WIRELESS NUMBER UNDER A COMPANY NAME: IN THE BOX - Relinquishing Company Name: If the phone NUMBER IS NOT IN A COMPANY NAME, mark the box N/A. If the phone NUMBER IS IN A COMPANY NAME, Enter the company s name. IN THE BOX Name of Authorized SPOC: Leave as J.R.Pool or Lorraine Thompson. Also, if you recently joined the FOP, let us know so we can call a lodge representative to verify your membership. Processing will be delayed until your FOP membership is verified, so give Lorraine and J.R. enough info to help them make the verification. 4

5 The Final Steps. When Lorraine or J.R. receives your copy, she will verify you are an FOP member and send your paperwork to the Government Business Center for processing- This will take between 3-5 business days- Verizon will send you an notifying you your request has been completed. Don t be concerned unless you receive an stating your application is missing a piece of vital information. If that is the case, provide whatever missing information requested. Once Verizon has sent notification to you that your account set up has been completed, We strongly recommend setting up your My Business account 48 hours after your account has been transferred!! You may receive a greater discounts by ordering phones and accessories thru My Business than you would by calling our Government Business Center. You can sign up for online access through Lorraine Thompson 48 hours after your transfer account is complete- Please her at [email protected]. Hover over that and when it pulls down the fly out menu change the little drop down from My Personal Account to My Business Account and click the sign in button. It will change to a different screen and from there it will ask you for your user name and password.
