2 CATT (Corrective Action Tracking and Trending) Is written in Microsoft Access Office 2000 version and is compatible with Office The license is open architecture so you can make any desired modifications yourself Contains an OSHA compliance protocol, VPP assessment protocol, an ISO protocol, workplace inspection form, incident investigation criteria and more Permits Tracking of Issues, Findings, Actions, Closure, Open Items, etc. Permits Trending of Findings, Root Causes by Area, etc. 2
3 CATT (Corrective Action Tracking and Trending) With the database OSHAScreen : Over 800 questions taken from OSHA s selfinspection checklist VPPProfile Screen: Provides the criteria needed to determine status with respect to OSHA Voluntary Protection Program expectations ISO14001 Screen: A set of over 200 criteria based on ISO and ANSI requirements Incident Investigation & National Safety Council Causal Factors: 56 potential causal factors to consider in four categories: Equipment, Environment, People and Management Area Inspection Format: An inspection checklist and over 200 editable items 3
4 CATT (Corrective Action Tracking and Trending) Other items that can be tracked: Scorecard: Can be used by supervisors or by EHS to track training requirements, safety disciplinary action, accident/illness, inspections, investigations, participation and action plans. Goals & Objectives: Used to track plan of action and milestones for committee activity. Suggestions: Used to track EHS suggestions and issues submitted by the workforce. 4
5 Main Menu for CATT. Please preload Names, Groups, Buildings, Departments, Areas, Training Course Names first to facilitate use. After preloading the database, date input is greatly improved.
6 Inspections or suggestions may result in a finding that action is needed to comply with an OSHA regulation. This permits documentation and tracking to closure.
7 An assessment for OSHA VPP expectations for safety and health programs may show gaps.these can be identified and actions tracked.
8 Similarly, ISO issues gaps can be tracked to closure.
9 There are numeric goals that sites identify
10 Numeric goals often are set for individual departments for the issues shown on this screen. These are set for the year.
11 Goals are set for any group (team, committee, etc.) and actions to meet the goals are tracked to closure here.
12 Actions needed to address the issues raised by suggestions are tracked to closure with this form.
13 Generally, this is the data that is usually collected for who, what, when, where, and why. This is the top half of the investigation screen. The bottom half permits identification of causal factors, root causes and assignment of actions needed to prevent reoccurrence.
14 Results of inspections are documented here and actions to correct issues are assigned and tracked to closure on the bottom half of the form.
15 This data would be updated by the user on a monthly basis and it is used for a formal report from CATT.
16 The raw data on number trained by department should be updated as needed and actions to handle gaps defined and tracked too.
17 A questionnaire for an abbreviated audit against OSHA standards is included in CATT. The line items are the pull down issues on the OSHA Issues input form
18 This protocol can be printed out and used for a self-audit against the usual and customary OSHA regulations. Can also be exported to Word.
19 A blank form for Incident Investigation can be printed from CATT to serve for the collection of field notes, etc.
20 A comprehensive inspection form is included in CATT. It can be exported to Word and customized for any building, department, area.
21 The request form is completed first and the Preview Report button then clicked.
22 This generates a report for every building and department.
23 More of the report. The report bottom summarizes Action Plans by subject.
24 Trending capabilities of CATT CATT has built in trending for two issues: Incident Causal Factors Area Inspection Findings Microsoft Access permits you to create your own queries and search for trends on any subject. 24
