1 Health Information Exchange in Washington State LATEST PROJECT UPDATE December 2, 2013 OneHealthPort HIE Community Welcomes New Trading Partners This fall, the OneHealthPort HIE welcomed several new trading partners to the information exchange community. Trading partners include HealthPath Washington-Health Home Lead Organizations, the Medical Information Network-North Sound (MIN-NS) and Pacific Family and Internal Medicine. These organizations joined the HIE to leverage their health information technology infrastructure for reporting and clinical information exchange, and to reach out to a broader range of trading partners using transactions supported by the HIE. HealthPath Washington-Health Home Lead Organizations: Coordinated Care - Established to deliver quality healthcare in the state of Washington through local, regional and community-based resources, Coordinated Care is a Managed Care Organization and subsidiary of Centene Corporation (Centene). Coordinated Care exists to improve the health of its beneficiaries through focused, compassionate and coordinated care. OptumHealth OptumHealth provides the information, tools and solutions that people use to help navigate the health care system. OptumHealth also helps people finance their health care needs and achieve their wellness goals. OptumHealth s approach brings personalized information together with the appropriate services to help address an individual's need. Areas of expertise range from behavioral and clinical health support, online health coaching and counseling for complex medical conditions, to health savings accounts. OptumHealth works with employers, payers, public sector entities and the people who use their products and services every day.

2 Community Health Plan of Washington Community Health Plan of Washington (CHPW) provides affordable comprehensive coverage to more than 300,000 individuals and families throughout Washington State. CHPW is the only health plan in the state founded by local community health centers. The CHPW network brought together shared values for equal access to health care, years of experience in community organizing, and effective health care administration with a commitment to improve the health of communities by making quality health care accessible to all Washington State residents. Southeast Washington Aging and Long Term Care SE Washington Aging and Long Term Care is a designated Area Agency on Aging (AAA) covering an eight-county area consisting of: Asotin, Benton, Columbia, Franklin, Garfield, Kittitas, Walla Walla, and Yakima counties. AAAs were established under the Older Americans Act (OAA) in 1973 to respond to the needs of Americans 60 and over in every local community. By providing a range of options that allow older adults to choose the home and community-based services and living arrangements that suit them best, AAAs make it possible for older adults to remain in their homes and communities as long as possible. Community Choice Healthcare Network - Community Choice is a healthcare consortium composed of regional healthcare organizations including 5 hospitals and 10 clinics with a total of 200+ physician members in North Central Washington. In addition to these member organizations, Community Choice has partnerships and collaborations with various other organizations in the areas of mental/behavioral health, social services and government institutions. Northwest Regional Council - As an association of county governments, the Northwest Regional Council (NWRC) has been serving people of Island, San Juan, Skagit, and Whatcom Counties since As Northwest Washington s Area Agency on Aging (AAA), one of thirteen such agencies in Washington State, NWRC implements state and federal programs for seniors and people who need long-term care. Some of the communitybased care programs NWRC funds are Senior Information & Assistance, Congregate Meals, Meals on Wheels, Respite Care, Adult Day Services, Case Management and Family Caregiver Support. MIN-NS The Medical Information Network-North Sound brings health information exchange technology to hospitals, physicians and patients in Skagit County and the North Sound region, unifying the medical community for better care of patients. Connectivity to the OneHealthPort HIE is helping MIN-NS achieve their goals to expand connections to

3 government agencies, medical providers and payers in the larger trading partner community. MIN-NS anticipates connecting their subscribers to the state Department of Health (DOH), medical providers using the Epic EMR, and payers expecting real time Admit and Discharge notices. Pacific Family and Internal Medicine Pacific Family and Internal Medicine, located in Sequim, Washington is looking to use the OneHealthPort HIE to expand their information exchange capabilities and better coordinate patient referrals with specialty providers in their medical community. One Connection for Many Transactions OneHealthPort HIE and the Washington State Department of Health This fall, the OneHealthPort HIE implemented connectivity with the Washington State Department of Health (DOH). Trading partners participating in the HIE can use this single point of connectivity to exchange information with DOH programs and meet Meaningful Use public health reporting objectives. A description of the current programs and reporting supported by the OneHealthPort HIE is listed below. Prescription Monitoring Program - The Prescription Monitoring Program (PMP), referred to as Prescription Review, is a secure online database used by practitioners across Washington State to improve public health. This centralized database, sponsored by the Department of Health (DOH), holds controlled substance prescription information for patients across the state. Registered practitioners have access to their patient s information before they prescribe or dispense drugs. This allows them to look for duplicate prescribing, possible misuse, drug interactions and other potential concerns. Now, organizations will be able to request and receive their patient s information using secure, encrypted electronic NCPDP query and response transactions generated that can be integrated with their electronic medical record system through their connection to the OneHealthPort HIE. The Department of Health and OneHealthPort are interested in working with organizations who may be interested in testing and operationalizing this transaction. Public Health Meaningful Use Transactions - Organizations looking to meet meaningful use public health objectives now have the option to do so by submitting these transactions using

4 the OneHealthPort HIE. DOH is now able to receive electronic laboratory reporting (ELR) and Syndromic Surveillance messages through the HIE. This connectivity is one of many initiatives DOH has underway to leverage use of information technology to better manage public health in Washington State. The DOH teams managing these transactions have established onboarding processes that describe the steps required to prepare and test the transactions, connect to the statewide HIE and, when validated by DOH, begin submitting this important public health information electronically. Admit-Discharge Notification Health care system soon to come on board The Admit-Discharge Notification transaction continues to generate interest from hospitals and health plans. MultiCare Health System is currently making preparations to test this transaction and is aiming to begin participation late this year or early When live, MultiCare Health System will report admission and discharge information for a number of hospitals across their system Allenmore Hospital, Auburn Medical Center, Good Samaritan Hospital, Mary Bridge Children s Hospital, and Tacoma General Hospital. Current hospital and health plans participating in the Admit-Discharge Notification transaction are listed in the table below. These trading partners are looking for other organizations with whom they can exchange this information to further leverage the value of this administrative transaction.

5 Not familiar with the history behind the Admit-Discharge Notification transaction? The current process hospitals use to deliver admit and discharge notices to health plans is cumbersome and inefficient. With increasing expectations to coordinate care and manage cost, care management information exchanges like this become more important. As the lead organization in the state for administrative simplification and Health Information Exchange (HIE), OneHealthPort has worked with hospitals and health plans to develop a better way to share admit/discharge notifications. OneHealthPort launched the ADN transaction in January 2013 using the statewide HIE to deliver admit and discharge notices electronically from hospitals to health plans. The notifications may be sent by hospitals as either: Individual patient admit and discharge notifications A daily census of all current patients and recently discharged patients The HIE can route the notices to multiple payers for patients with more than one insurer and the hospitals get back an acknowledgement that serves as confirmation the notice has been received by the health plan.
