2 What is a Patient Portal? A password protected website for patients to electronically access information in their electronic medical records.
3 Why Patient Portals? To help patients manage their own healthcare. To make information more conveniently accessible. To comply with Meaningful Use requirements.
4 What is Meaningful Use? Part of healthcare reform. Intended to get healthcare facilities to meaningfully use their electronic medical records. Each year, Medicare payments to healthcare facilities will be impacted by how hospitals and clinics comply with the measures the government has identified.
5 What Are the Portal Requirements for Meaningful Use? For Clinics that have implemented EHR in 2013 or earlier: More than 50% of patients seen in clinic must be given electronic access to their medical information. More than 5% of the patients seen in the clinic must log in and view their health information. More than 5% of the patients seen in the clinic must send a message via the portal. Will track these statistics in July, August & September 2014.
6 What Are the Portal Requirements for Meaningful Use? For Clinics that will implement EHR in 2014: More than 50% of patients seen in clinic must be given electronic access to their medical information. Will track these statistics in May, June and July 2014.
7 What Are the Portal Requirements for For Hospital Meaningful Use? More than 50% of patients admitted as an Inpatient or in Observation must be offered electronic access to their medical information. More than 5% of the patients admitted as an Inpatient or in Observation must log in and view their health information. Will track these statistics in January, February and March 2014.

Patient name Admit and discharge date and location Reason for hospitalization Care team including the attending of record as well as other providers of care Procedures performed during admission Current and past problem list Current medication list and medication history Current medication allergy list and medication allergy history Vital signs at discharge Laboratory test results Summary of care record for transitions of care or referrals to another provider Care plan field(s), including goals and instructions Discharge instructions for patient Demographics maintained by hospital (sex, race,ethnicity, date of birth, preferred language) Smoking status Information from all Mercy Health Network North Iowa locations
