3 Foreword Dear Reader The Fiish Arthroplasty Register has a log history. The Register was origially fouded by the Fiish Associatio of Orthopaedic Surgeos ad sice 1980 has collected iformatio cocerig orthopaedic edoprosthetic surgery. Juha Nevalaie, M.D., the previous editor-i-chief, started to publish the data collected systematically, thus brigig this iformatio ito cliical use ad to assist decisio-makig. Over the years, the structure of this publicatio has become established ad its readers leared to use it to search for the iformatio they eeded. This is testified by the fact that the last editio is almost sold out, the coutless dowloads from our website ad the may eager requests for a ew Yearbook. Dr. Juha Nevalaie has really doe groud-breakig work ad deserves the warmest thaks from us all. For the ew editor-i-chief it has bee easy to build o the previous publicatios. O the other had, there have bee distict difficulties i brigig some ew perspectives to existig iformatio whilst keepig the readers happy. The aim was to streamlie the book, to make it easier to read ad, perhaps, to provide some ew iformatio. At the same time, the purpose was to preserve iformatio that the readers have become familiar with. A ew feature is that the comparability of a sigle edoprosthesis to other edoprostheses by survival curves is ow possible at a glace. The structure of the tables places more emphasis o the preset data rather tha the historical iformatio that still has bee preserved. I additio, there is a attempt to take a look ahead by assessig future treds i orthopaedic edoprosthetic surgery. This assessmet was based o curret operatio statistics i the populatio over 40 years. The assumptio was that characteristic operatio umbers will remai costat i each age group ad thus the forecast will oly reflect the ifluece of the populatio ageig. It is comprehesible that this type of forecast fails to take ito accout progress made i treatmet methods ad the chage i the icidece of osteoarthritis ad other joit diseases. The assessmet of these issues will remai beyod the scope of this publicatio. Still, this forecast shows that it is ot possible to eglect the growig problem of multiple joit diseases. It is importat to remid the reader that this publicatio is ot a scietific oe but merely a descriptio of the cotet of the Register. Iterpretatio of the figures thus remais solely the resposibility of the reader. Nevertheless, aalytical methods are widely used i medical statistics. The descriptio of some methods that are ot self-evidet ca be dowloaded from our website. Cofidece itervals (95%) are widely used i survival curves ad the upper limit is show as CI_max ad lower limit CI_mi, correspodigly. The reader may become embarrassed by the fact that edoprosthetic survival just after implatatio is worse after 1996 tha it was earlier. This declie is based o the chage i data recordig methods. Before 1996, the early problems after edoprosthetic surgery were cosidered as primary complicatios eve though they resulted i a ew operatio ad revisio arthroplasty. Sice the, ew surgical procedure less tha 4 moths after the primary oe has bee similarly viewed as other subsequet operatios. Cosequetly, the survival curves reveal more clearly the early problems of edoprosthetic surgery tha the previous recordig method. Still, all differet moder edoprostheses are assessed i the same way. We hope this iformatio will give a more realistic picture of the early results of edoprosthetic surgery. Therefore, we have added some ew figures as a survival curve for shoulder, akle ad elbow edoprostheses, too. I have bee pleased to receive may wishes for the future developmet of the Register, its data cotet ad the form of the preset publicatio. Some of these wishes will materialise i the form of scietific published studies. Ufortuately, it is ot possible to fulfil most of these wishes due to the atioal ature of the Register. It is importat to maitai comparability of the curret data to the previous oe. All chages i the Register cotet will ecessitate atioal chages i data collectig ad trasfer systems. Such chages are time ad labour cosumig. It is probably ot possible to make major chages util data collectig, processig ad trasmissio systems have become automatic alog with atioal electroic patiet records. The data cotet of the Register has frequetly bee compared to other cetralised atioal patiet registers i the Natioal Research ad Developmet Cetre for Welfare ad Health (Stakes) by comparig the few overlappig data. The coverage of the Register data cotet has repeatedly bee excellet: some of our data is lackig from Stakes ad vice versa. The Register covers practically 100% of Fiish arthroplasties. Thus, the lackig data does ot affect the essetial iformatio that ca be extracted from the Register data. Such exceptioal coverage i the data cocerig early after surgery data would ot be possible without the work of umerous people workig i operatio theatres ad hospital departmets. However, the data cocerig the later stages of the patiet history is probably ot complete ad should thus be treated with cautio. Co-operatio with Stakes has expaded the data cotet of the preset publicatio. Stakes has e.g. produced iformatio cocerig the time spet i hospital at the time of arthroplasty. It is ecouragig that this time has slightly decreased durig the past 10 years. Whether this is the reaso for the cocomitat decrease i otified early complicatios or whether the complicatio rate really has decreased remais obscure. Mr. Simo Pelateri is ackowledged for his work i processig the data i Stakes. I wat to express my thaks to Mr. Jyrki Kettue, Ph.D. for his help i estimatig future surgical treds, to my colleagues i the Departmet of Medical Devices ad to the Natioal Agecy for Medicies that has agai made the publicatio of this data possible.
