ROYAL NATIONAL ORTHOPAEDIC HOSPITAL NHS TRUST RNOH Patient Group Action plan from ward and department fact finding visits /2011 REHABILITATION WARD (Visit on 15 August 2010) Staff to follow the procedures for the reporting of accidents, and must not assume any accident is trivial. All nursing staff are aware of the incident reporting procedure and undertake it as appropriate The ward could rehabilitate more patients if they were able to use more physiotherapy and occupational therapist time. Patients should be told that they can ask for a snack if they are hungry during the evening, as exercise increases their appetites and portions need to be larger, particularly on this ward where patients are taking charge of their own progress. Patients should be empowered and encouraged to talk to the staff if they need anything rather than suffering in silence. Ward manager to have input to the design of the accommodation before the new build design is finalised, to ensure the particular needs of the patients will be met. Disused areas could be locked to improve the appearance of the ward. There has been an increase in the number of therapists caring for upper limb patients and rehab/pain management programmes. Now short of beds so are in the process of implementing hotel admissions for patients. The provision of therapists for orthopaedic and bone tumour patients requiring rehab remains the same. This has not been an issues since the last adult When the hospital is rebuilt the rehab service will move into the pre-existing Spinal Cord Injury Centre There is one area of the ward that is disused (an old shower) but at present this is being used for storage so is not locked as such June2011 1
ORTHOTICS DEPARTMENT (Visit on 27 August 2010) RNOH Patient Group strongly support the view of the orthotics team that the manufacture of orthoses should remain on the hospital site with the clinicians to provide the best possible and efficient service to their patients Signage to the unit to be re-reviewed. If the shuttle bus service is not working it is important that porterage is available for people with mobility problems and that information on this is available at the front entrance as well as on the website. Still awaiting plans for the development of the OPD as to where the Orthotics production will be moved to Signage has been improved and now there are new yellow signs directing patients to orthotics. Request to have an inclusion on the Trust website to advise if the shuttle bus is not available that patients can report to the main entrance porters lodge and ask for transportation to the dept. Kelvin Smith/ Pauline Lodwick Kelvin Smith Kelvin Smith July2011 ANGUS MACKINNON WARD (Visit on 17 August 2010) Patients would welcome being told about what they can do while on the ward e.g. asking for more food if hungry; visiting the Aspire garden; being able to sit outside The housekeeper offers beverages with biscuits or cakes between meals. Ward Manager is working on an admission information leaflet which will advise on recreational facilities and general timetable of the ward. Kathy Coultry Update X RAY DEPARTMENT STANMORE (Visit on 9 August 2010) Provide wheel chairs to reduce delays in the arrival of patients from the out-patients department. Superintendent Radiographer to review. More wheelchairs would help but this means having more porters available. Update 2
Another work-station for the reporting room and new chairs to reduce waiting for reporting. Staff should not need to eat lunch while reporting and should have a proper break. Patients changing in the scanning rooms August be limiting through-put. If changing cubicles are not needed they could be replaced with proper storage. If changing cubicles are needed, they should be dignified and not exposed to public gaze. There is only one secretary serving three managers, impacting on workload. Superintendent Radiographer to review but believes that having more fellows and consultants would reduce waiting times Superintendent Radiographer to review but advises that Radiographers have proper breaks but Radiologists tend to eat while working. Superintendent Radiographer confirmed changing cubicles are needed. Redevelopment work in the department should resolve this situation. Update On progress Update on progress Under review Update One toilet has no alarm cord or disabled facilities Under review in re-development plan Update Poor condition of flooring, covered in tape, trip hazard and impossible to keep clean Standard of cleaning in the department Under review in re-development plan Update Superintendent Radiographer has implemented a new cleaning rota Update on progress MARGARET HARTE WARD (Visit 12 August 2010) Shortage of storage space and adequate staff facilities. Limited space between beds Review of nurse/patient ratio Communication between patients and staff i.e. inadequate information regarding epidurals and pain control. Loss of 2 beds due to patient handling bed space assessment and reassessment of fire exits. The loss of beds affects the nurse/patient ratio. All ward becoming involved in teaching at the Joint School to improve communication/patient education in the long term for patients. 3
Long shifts, Real time survey, medication/pharmacy Ongoing long day survey being undertaken in the Trust. The Ward is working positively to achieve the target for the Real Time Survey. Pharmacy is going through changes and is a work in progress. COLEMAN UNIT (Visit on 9 September 2010) Signage issue as the name of the unit does not appear on site maps so the unit difficult to find, Suggest the map on the website be searchable to make it more useful Magazines and children s toys would alleviate the waiting periods Toilets should be provided at both ends of the unit so facilities for each gender are properly separated Facilities to review maps. New visitor maps and car parking/external maps are now on order. Communications to review maps on website. Lynn Wilson Jackie Stephen Ward Manager to review Ward Manager Not an issue now that the unit is single sex n/a n/a OUTPATIENT DEPARTMENT, STANMORE (Visit on 6 October 2010) Overcrowding on some clinic days is unsatisfactory Service improvement review is ongoing Pauline Lodwick for all concerned. More space is required for treatment and consulting rooms as well as larger rooms for the staff (training, changing, etc) Disabled parking required to replace spaces lost to shuttle bus stop Refurbishment of the department and resiting of limb-fitting is planned OPD Car park has been re-designated and redesigned so problem now addressed Pauline Lodwick/Sarah Webb WARD 4 (Visit on 14 October 2010) Staff arriving in their own clothes have nowhere to change or take a shower and only small lockers To be reviewed Ward Manager Update 4
All wards are meant to be able to provide extra food on request. Can the ward be given keys to the main fridge? Trust equipment, such as wheelchairs left by discharged patients takes up valuable storage space on the ward. Could the porters keep them somewhere centrally instead? To be reviewed Ward Manager Update To be reviewed Lynne Wilson Update HOSPITAL SCHOOL (Visit on 26 November 2010) Computers and other electrical equipment brought in from home need to be PAT tested to ensure compliance and safety Not all children who come in are funded to be tutored it depends on the council involved. RNOH to consider providing the service for all admitted children. Staff not qualified to teach A-level but students still need to be encouraged and supported in their studies whilst in hospital Ward staff should be aware that the Estates Help Desk has to be contacted for personal patient equipment to be checked School lead to look into Gail Burgess Update School lead to look into Gail Burgess Update (PROSTHETICS) LIMB FITTING (3 December 2010) Brightening up fitting rooms and a fitting room specifically for teenage patients would improve environment Access to refreshments, internet access to give immobilised patients with extended waits a distraction, Call alarm to enable staff attention Bins to be relocated somewhere less obtrusive. Recommendation will be implemented as far as possible within the relocation from OPD to Louis Fleishmann building Recommendation will be implemented as far as possible within the relocation from OPD to Louis Fleishmann building Recommendation will be implemented as far as possible within the relocation from OPD to Louis Fleishmann building Sue Walker Sue Walker Sue Walker 5
Toilet doors need closers to stop wasteful continual operation of extractor fans Limb fitting patients find parking difficult Estates investigated but the doors operate a two way entry as they are disabled toilets so cannot have door closures fitted This should have been resolved with new parking restrictions now in force PLASTER THEATRE (17 February 2011) Although all suggestions from 2009 visit have been resolved, hand washing facilities is still a high priority for both staff and patients There is no issue with facilities as there are 9 sinks available for hand washing all located in appropriate areas. Hand washing is a high priority in the dept to prevent cross infection Cathy Evans 6
