6 2 Minnesota Department of Human Services Treatment Providers Below is a list of the most well-known treatment providers, specifically detailed for each of the Phase 1 SBIRT sites. Providers are located in or near the individual clinic communities. To find all substance use treatment providers listed by county/city, refer to the Minnesota Department of Human Services/DHS website, provided below in the Comprehensive Treatment Providers section. SBIRT sites should consider contacting and/or visiting the treatment providers in their communities to learn more about their services. In addition, consider inviting a representative from a treatment program to your clinic to provide further information on their specific services. See Targeted Treatment Provider list below for Phase 1 Sites. Instructions to access the site: The MN Rule 31 Substance Use Disorder Treatment Provider list is updated as April 30, 2012 and changes periodically throughout the year. To access the list refer to the below instructions: Visit the DHS website, Click on General Public tab and Licensing Click on Licensed programs, then on Chemical Dependency treatment programs Select the licensed program lists, then CSV and Excel Chemical Dependency Treatment Programs (Rule 31) and sort by county/city.
