Interpreting/captioning services will only be provided to a student who has a verifiable disability and whose disability related functional limitations prevent him/her from hearing regular speech and lectures. Student must have an audiogram on file with the DSPS office. 2. Students are required to meet with a DSPS counselor each semester to begin or continue receiving services from our office. 3. This form must be signed by the DSPS counselor prior to the initiation of interpreter services. 4. The DSPS office will contact contracted interpreter agencies. 5. If a student does not attend the first class meeting, services for that class will be suspended. 6. Interpreters will wait only 20 minutes outside of the classroom for a student. 7. If the interpreter does not show or you cannot attend class, the student should contact the DSPS Office: ext. 7220, or by to: dspsoffice@contracosta.edu 8. It is the student s responsibility to notify the DSPS office as soon as it is known he/she will be unable to attend class and at least 96 hours (4 days) prior to the class starting (when possible), so the interpreter can be notified of the cancellation. 9. The student must notify the DSP&S office in writing of any class changes to this application for interpreter services; and those changes must be approved by the DSP&S office prior to implementing modifications. 10. Students are not allowed to change services with the interpreter. 11. Students must maintain an active account and provide DSP&S office with their address. 12. Students must complete 75% or more of your units through to the last day in order to enroll in classes. 13. Students must maintain a 2.0 grade point average in all classes. 14. In the event that interpreting is not available we will provide captioning. 15. Students must sign all Interpreter Confirmation of Service forms and all other required forms in order to receive services from the DSPS office. (sample forms are included in packet). Important: If you miss three (3) classes without informing the DSPS staff, in writing, DSPS staff will suspend interpreting services for that class until you meet with the DSPS counselor to discuss the problem. If you miss four (4) classes without informing the DSPS staff, in writing, DSPS will suspend interpreting services for that class for the rest of the semester. 2 INTERPRETING/CAPTIONING Instructions STUDENT RESPONSIBILITIES BEFORE the semester starts: 1. See your DSPS counselor early to request your interpreting needs for each new class. The DSPS office is located in the Student Services Center, Room You must report any schedule changes to your DSPS counselor immediately. AFTER the semester starts: 1. If you change your class schedule, you must see your DSPS counselor immediately. 2. If you add a new class, see your DSPS counselor immediately. 3. You must sign an Interpreter Confirmation of Service Form at each class meeting. If you are absent: 1. CANCEL your interpreter or captioning service 96 hours (4 days) before the beginning of your class. 2. DSPS office at: dspsoffice@contracosta.edu If you are ABSENT and DO NOT CALL before class starts, your services may be suspended. If your Interpreter is LATE or ABSENT or the real time captioning does not work: 1. Use NCR paper and an in-class volunteer note-taker and/or 2. Use a tape recorder and tape 3. TELL the DSPS office, by at: dspsoffice@contracosta.edu. (They need to know!) If you need real time captioning for a MEETING, COUNSELING, etc.: 1. For special events please contact the DSPS instructional assistant at least 2 weeks (14 days) in advance, so the proper paperwork can be filled out and processed in a timely manner. 2. Return the form to the DSPS office at least 10 working days before the event. Student Responsibilities Form 6/2008

3 Inappropriate Interpreter Behavior Interpreters will not engage in inappropriate behavior with students. These behaviors include but are not limited to: 1. Getting involved with the student on a personal level 2. Enrolling in an assigned class 3. Counseling or advising the student 4. Tutoring the student 5. Interpreting or explaining personal understanding/knowledge of what the instructor is trying to explain 6. Making special arrangements with the student regarding work schedule 7. Becoming a distraction for the other students or the rest of the class 8. Expressing their ideas or feelings about the topic/project being discussed 9. Becoming an aide for the student 10. Taking class notes for the student 11. Participating in class activities 12. Gathering or providing materials for the student that are needed for a class assignment or project 13. Explaining anything to the instructor for the student when the student is not present in class (i.e. what the student said or did, why they are missing class, etc.) 14. Explaining anything said by the instructor while the student was not present. 15. Staying late for a student without contacting the DSPS office first 16. Attending class on a day not scheduled for interpreting without telling the DSPS office 17. Making plans with the student without approval from the DSPS office 18. 5 The Interpreter Confirmation of Service Form Verification signatures from the Instructor, the Interpreter, and the Student confirm that the Sign Language Interpreters are/were present in the classroom, will assist Disabled Students Programs and Services (DSPS). THIS FORM MUST BE SIGNED AT EACH CLASS MEETING. 1. To provide prompt and consistent interpreter services for Deaf students; 2. With prompt processing of invoices from the Interpreter Referral Services Agency. Without the signature form a delay in payment may occur; and interpreting services may be suspended. 3. 8 Contra Costa College Disabled Students Programs & Services (DSPS) Missing Classes Policies and Services for Deaf and Hard of Hearing Students Services I understand that if I do not attend the first class session of the semester, and I do not inform the DSPS office, in writing, by , or by phone, in advance of my absence, my interpreting/captioning services will be suspended. I understand that if I will miss class because I am ill, I must call the DSPS office at (Voic ext. 7220), or send an to: dspsoffice@contracosta.edu, at least one hour before class meets. I must submit a note from my doctor. Otherwise, it will be considered an unexcused absence and I will receive a warning. I understand that if I miss class for any other reason, I must notify the DSPS office by at least 4 days in advance, so that interpreters/captioning can be notified. 1. The first time I do not notify the DSPS office at least 4 days in advance, I will receive a warning. 2. The second time I do not notify the DSPS office at least 4 days in advance, my interpreting/captioning services for that class will be suspended. I will have to schedule a meeting with a DSPS counselor to resolve the problem before my services are reinstated. 3. The third time I do not notify the DSPS office at least 4 days in advance, my interpreting/captioning services will be cancelled for the rest of the semester. In spite of all our efforts, we do not have enough interpreters to meet the demand for the fall 2008 semester. I regret to inform you that we are not able, but will continue to look for an interpreter to provide interpreting services for: Please meet with your counselor to discuss the following options: 1. Drop the class and take the same class next semester. 2. Join a class with interpreting services already established. 3. Remain in the class and use a note taker or captioning service.
